Artificial intelligence outperforms a nomogram for osteoradionecrosis prognostication following fibula free flap reconstruction in oral cancer patients.

BACKGROUND: Osteoradionecrosis (ORN) is a serious complication in oral cancer patients undergoing tumor excision and mandibular reconstruction, particularly after radiotherapy. This study compared a logistic regression-based nomogram with five machine learning (ML) algorithms to identify the most clinically useful ORN prognostication tool. METHODS: We retrospectively analyzed 275 irradiated oral cancer patients who underwent segmental mandibulectomy and immediate fibula flap reconstruction at a single center. Patients were split 75:25 into training and test cohorts. 34 patient variables were …

Effect of Segment Length and Number of Osteotomy Sites on Cancellous Bone Perfusion in Free Fibula Flaps.

BACKGROUND: Indocyanine green (ICG) videoangiography is routinely used to evaluate skin and organ perfusion and to assess patency rates of microvascular anastomoses. This study uses ICG angiography as a novel approach to qualitatively and quantitatively evaluate bone perfusion of microvascular fibula grafts intraoperatively and to assess the effect of fibula segment length and number of osteotomies on bone perfusion. METHODS: All patients planned for mandible reconstruction using a microvascular fibula graft between January 2013 and May 2017 were considered for this study. ICG videoangiograph

Evaluation of alternative airway management strategies in surgical repair of severe cranio-maxillofacial trauma.

BACKGROUND: Severe cranio-maxillofacial trauma requires the utilization of alternative airway management techniques when oral and nasal routes of tracheal intubation are either not possible or contraindicated. METHODS: This retrospective study evaluated the techniques of submental intubation, retromolar intubation and tracheostomy in terms of their frequency of use, time required, costs involved and associated complications in a total of 200 patients. RESULTS: Frequency of utilization of alternative airway management techniques was 3.63%. Tracheostomy was associated with the most severe comp

Bone Regeneration and Dental Implant Surgeries in Florid Cemento-Osseous Dysplasia: A Case Report.

Cemento-osseous dysplasia (COD) is a benign fibro-osseous pathology in which fibrous connective tissues, osteoid and cementum-like materials, replace bone. Concerning the hypovascularization and increased mineralization of bone that occurs in these patients, the clinician may face 2 types of problems: infectious complications, such as osteomyelitis, and increased implant failure. The present study aims to report the successful and innovative management of a COD patient complicated by mandibular osteomyelitis and the implant rehabilitation of this area. We report the management of a 54-year-old

A Case Report of Primary Intraosseous Carcinoma of the Mandible Initially Diagnosed as Mandibular Osteomyelitis.

Primary intraosseous carcinoma (PIOC) is a rare malignant tumor of the jaw, often diagnosed as odontogenic cysts or osteomyelitis. We report a case of a 53-year-old woman who was initially diagnosed with mandibular osteomyelitis resulting from pericoronitis of the left lower third molar. The patient underwent tooth extraction under general anesthesia, and the granulation tissue from the extraction socket was sent for histopathological evaluation, revealing moderately differentiated squamous cell carcinoma. Postoperative imaging revealed significant bone resorption, invasion of the mandibular c

Treatment of metastatic apocrine sweat gland adenocarcinoma in a cat with association of surgical resection and angular oris reconstruction, extensive lymphadenectomy, electrochemotherapy and systemic chemotherapy.

Apocrine sweat gland adenocarcinoma (ASGAC) is a rare skin carcinoma in cats. In some cases, this tumor occurs in anatomical sites of challenging wide surgical resection, which increases the need for adjuvant therapies for residual disease. These include radiotherapy or electrochemotherapy (ECT), local treatments for invasive tumors in companion animals often associated with surgery. However, the current literature for ASGAC treatment is limited and there are no reports of ECT as an adjuvant therapy. In this case report, we account for the case of an 11-year-old cat with a history of a non-ulc

A case series on a new reference in mandibular angles harmonization: the Frontozygomatic-Infraorbital Line (FZ-IOL).

the aim of this paper, is to propose a new reference line: the Frontozygomatic-Infraorbital Line (FZ-IOL). This reference line can guide the surgical team planning mandibular angle harmonization, based on the patient's skeletal proportion. The Frontozygomatic-Infraorbital Line has been adopted for symmetrization surgery, masculinization surgery, and in unsatisfactory results of previous orthognathic surgery. From March 2021 to December 2022, 3 patients were treated for severe facial asymmetry affecting mainly the lower third of the face. All cases were planned with the reference FZ-IOL. Patien

Exposed weapons: A revised reconstruction of the facial anatomy and life appearance of the saber-toothed cat Megantereon (Felidae, Machairodontinae).

Megantereon was a widespread saber-toothed felid from the Pliocene and Pleistocene of the Old World and North America, but its rarity in the fossil record makes it complicated to restore its life appearance. Lack of complete specimens makes it necessary to combine information from fossils of different individuals to reconstruct their facial anatomy. In this work, we combine the analysis of skulls and mandibles of Megantereon from various fossil sites with the study of extant carnivorans through dissection, 3D scans, and the observation of live individuals. Megantereon combined very elongated u

The pressure in the temporomandibular joint in the patients with maxillofacial deformities.

BACKGROUND: Temporomandibular disorder (TMD) symptoms were found to be common in the patients with maxillofacial deformities. The mandibular structure was in relation with the stress within temporomandibular joint (TMJ). However, the current studies on the TMJ stresses in the patients with different maxillofacial deformities are not comprehensive enough. PURPOSE: The aim of this study was to investigate the compression and morphology of the TMJ in the patients with different maxillofacial deformities under central occlusion. METHODS: 24 patients and 10 asymptomatic individuals were included

Anatomical similarity between the Sost-knockout mouse and sclerosteosis in humans.

Sclerosteosis, a rare autosomal recessive genetic disorder caused by a mutation of the Sost gene, manifests in the facial skeleton by gigantism, facial distortion, mandibular prognathism, cranial nerve palsy, and, in extreme cases, compression of the medulla oblongata. Mice lacking sclerostin reflect some symptoms of sclerosteosis, but this is the first report of the effect on the facial skeleton. We used geometric morphometrics (GMM) to analyze the deformations of the murine facial skeleton from the wild-type to the Sost gene knockout. Landmark coordinates were obtained by surface reconstruct

Mesiolingual root rotation for horizontal mandibular third molar extraction: position classification and surgical simulation.

Extracting horizontal mandibular 3rd molars face considerable difficulty due to the large bone and adjacent tooth resistances. This study aims at evaluating the effectiveness of a novel method-mesiolingual root rotation to extract wisdom teeth of this type. In this study, 73 horizontal teeth extracted using piezosurgery were reviewed and classified based on impaction depth: position I, II, III refers to the highest portion of the crown on a level with upper 1/3, middle 1/3, lower 1/3 of the 2rd molar's root. Based on the surgical simulations on their 3D CBCT reconstructions, traditional method

A clinical evaluation of a bioresorbable membrane and porous hydroxyapatite in the treatment of human molar class II furcations.

BACKGROUND: The ultimate goal of periodontal therapy is predictable regeneration of a functional attachment apparatus destroyed as a result of periodontitis. Reconstructive procedures have been used with varying success during the past decades to accomplish this goal. AIM: To evaluate whether the use of porous hydroxyapatite alone or a bioresorbable membrane alone would enhance the clinical results in the treatment of class II furcation defects in human lower molars. MATERIALS AND METHODS: Fifteen patients with chronic periodontitis, aged between 39 and 49 years, with a pair of similar bilat
