Prognostic value of perineural invasion in gastric cancer: a systematic review and meta-analysis.

BACKGROUND: The prognostic role of perineural invasion in gastric cancer is controversial. Here, we present a systemic review and meta-analysis of the association between perineural invasion and survival in resectable gastric cancer patients. METHODS: A comprehensive literature search for relevant reports published up to April 2013 was performed using PubMed, Embase, Web of Science and Wanfang Data. Studies that investigated the role of perineural invasion with a sample size greater than 100 were included and analyzed. RESULTS: A total of 30,590 gastric cancer patients who had undergone cura

A joint frailty model to estimate the recurrence process and the disease-specific mortality process without needing the cause of death.

In chronic diseases, such as cancer, recurrent events (such as relapses) are commonly observed; these could be interrupted by death. With such data, a joint analysis of recurrence and mortality processes is usually conducted with a frailty parameter shared by both processes. We examined a joint modeling of these processes considering death under two aspects: 'death due to the disease under study' and 'death due to other causes', which enables estimating the disease-specific mortality hazard. The excess hazard model was used to overcome the difficulties in determining the causes of deaths (unav

A Bayesian autoregressive three-state hidden Markov model for identifying switching monotonic regimes in microarray time course data.

When modeling time course microarray data special interest may reside in identifying time frames in which gene expression levels follow a monotonic (increasing or decreasing) trend. A trajectory may change its regime because of the reaction to treatment or of a natural developmental phase, as in our motivating example about identification of genes involved in embryo development of mice with the 22q11 deletion. To this aim we propose a new flexible Bayesian autoregressive hidden Markov model based on three latent states, corresponding to stationarity, to an increasing and to a decreasing trend.

Forecasting depression in bipolar disorder.

Bipolar disorder is characterized by recurrent episodes of mania and depression and affects about 1% of the adult population. The condition can have a major impact on an individual's ability to function and is associated with a long-term risk of suicide. In this paper, we report on the use of self-rated mood data to forecast the next week's depression ratings. The data used in the study have been collected using SMS text messaging and comprises one time series of approximately weekly mood ratings for each patient. We find a wide variation between series: some exhibit a large change in mean ove

Tuberculosis in sheltered homeless population of Rome: an integrated model of recruitment for risk management.

The authors show the results of an integrated model for risk management of tuberculosis in a sample of sheltered homeless in Rome. Tuberculin skin test (TST) was used for evaluating the prevalence of latent infection (LTBI). In TST positives, expectorate was collected and chest X-ray was achieved. Multiple logistic regression analysis was performed to investigate determinants of infection. Out of 288 recruited subjects, 259 returned for the TST reading; 45.56% were positive and referred to a specialized center; 70 accessed the health facility and completed the clinical pathway. The risk factor

FEM-based 3-D tumor growth prediction for kidney tumor.

It is important to predict the tumor growth so that appropriate treatment can be planned in the early stage. In this letter, we propose a finite-element method (FEM)-based 3-D tumor growth prediction system using longitudinal kidney tumor images. To the best of our knowledge, this is the first kidney tumor growth prediction system. The kidney tissues are classified into three types: renal cortex, renal medulla, and renal pelvis. The reaction-diffusion model is applied as the tumor growth model. Different diffusion properties are considered in the model: the diffusion for renal medulla is consi

Development of artificial neural network based metamodels for inactivation of anthrax spores in ventilated spaces using computational fluid dynamics.

Linear, quadratic, and artificial neural network (ANN)-based metamodels were developed for predicting the extent of anthrax spore inactivation by chlorine dioxide in a ventilated three-dimensional space over time from computational fluid dynamics model (CFD) simulation data. Dimensionless groups were developed to define the design space of the problem scenario. The Hammersley sequence sampling (HSS) method was used to determine the sampling points for the numerical experiments within the design space. A CFD model, comprised of multiple submodels, was applied to conduct the numerical experiment

Evolutionary dynamics from a variational principle.

We demonstrate with a thought experiment that fitness-based population dynamical approaches to evolution are not able to make quantitative, falsifiable predictions about the long-term behavior of some evolutionary systems. A key characteristic of evolutionary systems is the ongoing endogenous production of new species. These novel entities change the conditions for already existing species. Even Darwin's Demon, a hypothetical entity with exact knowledge of the abundance of all species and their fitness functions at a given time, could not prestate the impact of these novelties on established p

An SEI model for sarcoptic mange among chamois.

We consider a simple model to study the dynamics of sarcoptic mange in a population of chamois. The epidemiological patterns observed during an epidemic in Italy are reconstructed and key parameters of the model are estimated from field data. In particular, we calculate the basic reproductive ratio R (0), a threshold value for chamois density for the occurrence of an epidemic and the speed of propagation of the epidemic wave. The model is then used to obtain indications on the effect of culling as a possible control measure in a closed population and extended to analyse the spatial diffusion o

Heterogeneous aspirations promote cooperation in the prisoner's dilemma game.

To be the fittest is central to proliferation in evolutionary games. Individuals thus adopt the strategies of better performing players in the hope of successful reproduction. In structured populations the array of those that are eligible to act as strategy sources is bounded to the immediate neighbors of each individual. But which one of these strategy sources should potentially be copied? Previous research dealt with this question either by selecting the fittest or by selecting one player uniformly at random. Here we introduce a parameter u that interpolates between these two extreme options

A general neuropsychological model of delusion.

INTRODUCTION: Neurocognitive accounts of delusion have traditionally highlighted perceptual misrepresentation, as the primary trigger in addition to other cognitive deficits that maintain the delusion. Here, a general neurocognitive model of delusional disorder (DSM-IV) is proposed, not so much based on perceptual or cognitive deficits after right hemisphere damage as on cognitive propensities, specifically excessive inferencing (especially jumping to conclusions) and excessive reference to the self, due to left hemisphere overactivity. METHOD: The functional imaging, topographic EEG, and exp

Heartburn and other related symptoms are independent of body mass index in irritable bowel syndrome.

BACKGROUND: increasing body mass index (BMI) is a risk factor for GERD but little is known about this association in the irritable bowel syndrome (IBS). AIMS: to determine the presence of heartburn and other related symptoms in relation with BMI in IBS. METHODS: volunteers (n = 483) answered the Rome II-Modular Questionnaire, and were divided into IBS and non-IBS (controls) groups. The frequency of heartburn, chest pain, epigastric pain, nausea, vomiting and belching was compared between the groups in the study sample and within three BMI categories. RESULTS: the IBS (23.7%) and controls (7

Influence of growth modulation on the effective permeability of the vertebral end plate. A porcine experimental scoliosis model.

BACKGROUND: Abnormal mechanical loading occurs in scoliosis as compared to normal spines. Intervertebral disc degeneration has been correlated with alteration of bone density in adjacent vertebral bodies. How vertebral end plate remodels in scoliosis and the consequences on disc homeostasis are not well understood. Permeability is a relevant physical measure to quantify mass transport in porous media. We hypothesized that effective permeability of the vertebral end plate was modified by growth modulation in a scoliosis animal model. METHODS: Flexible asymmetric posterior instrumentation was u

Optimal control of vaccine distribution in a rabies metapopulation model.

We consider an SIR metapopulation model for the spread of rabies in raccoons. This system of ordinary differential equations considers subpopulations connected by movement. Vaccine for raccoons is distributed through food baits. We apply optimal control theory to find the best timing for distribution of vaccine in each of the linked subpopulations across the landscape. This strategy is chosen to limit the disease optimally by making the number of infections as small as possible while accounting for the cost of vaccination.

Tetragonal distortions of some tetragonal Cr3+ centers in fluoroperovskite ABF3 crystals.

The tetragonal distortions [characterized by the displacement DeltaR of the F(-) ion intervening in Cr(3+) and V(B) (B(2+) vacancy) or M(+) ion in B(2+) site] for Cr(3+)-V(B) and Cr(3+)-Li(+) centers in Cr(3+)-doped fluoroperovskites KBF(3) (B=Mg, Zn) and ACdF(3) (A=K, Rb, Cs) and also for Cr(3+)-Na(+) center in KZnF(3) have been studied by calculating the EPR parameters D, g(parallel) and g(perpendicular) from the crystal-field theory. From the studies, an interesting trend is suggested, i.e., when the size of the co-doped monovalent impurity M(+) in the nearest neighbor B(2+) site is compara

Modelling the incubation period of anthrax.

Models of the incubation period of anthrax are important to public health planners because they can be used to predict the delay before outbreaks are detected, the size of an outbreak and the duration of time that persons should remain on antibiotics to prevent disease. The difficulty is that there is little direct data about the incubation period in humans. The objective of this paper is to develop and apply models for the incubation period of anthrax. Mechanistic models that account for the biology of spore clearance and germination are developed based on a competing risks formulation. The m

Predicting meningococcal disease outbreaks in structured populations.

Rational decision making on whether some form of intervention would be necessary to control the spread of a meningococcal epidemic is based on predictions concerning its potential natural progression. Unfortunately, reliable predictions are difficult to make during the early stages of an outbreak. A stochastic discrete time epidemic model was applied to adaptively predict the development of outbreaks of meningococcal disease in 'closed' populations such as military garrisons or boarding schools, which are further divided into subgroups called 'units'. The performance of the adaptive method was

Prospects for elimination of bancroftian filariasis by mass drug treatment in Pondicherry, India: a simulation study.

LYMFASIM, a microsimulation model for transmission and control of lymphatic filariasis, was used to simulate the effects of mass treatment, in order to estimate the number of treatment rounds necessary to achieve elimination. Simulations were performed for a community that represented Pondicherry, India, and that had an average precontrol microfilariae (MF) prevalence of 8.5%. When ivermectin was used, 8 yearly treatment rounds with 65% population coverage gave a 99% probability of elimination. The number of treatment rounds necessary to achieve elimination depended to a large extent on covera

Detection of genes influencing economic traits in three French dairy cattle breeds.

A project of QTL detection was carried out in the French Holstein, Normande, and Montbéliarde dairy cattle breeds. This granddaughter design included 1 548 artificial insemination bulls distributed in 14 sire families and evaluated after a progeny-test for 24 traits (production, milk composition, persistency, type, fertility, mastitis resistance, and milking ease). These bulls were also genotyped for 169 genetic markers, mostly microsatellites. The QTL were analysed by within-sire linear regression of daughter yield deviations or deregressed proofs on the probability that the son receives one

Developmental robustness.

Developmental robustness, the capacity to stay "on track" despite the myriad vicissitudes that inevitably plague a developing organism, is, I argue, a prerequisite for natural selection and key to our understanding of the evolution of developmental processes. But how is such robustness achieved? And how can we reconcile this property with the delicate precision that seems to characterize so many developmental mechanisms, with what Michael Behe calls "irreducible complexity"? By looking at context, I argue. Developmental mechanisms must be robust with respect to the kinds of insults they are mo
