Neuroschistosomiasis.

Schistosomiasis is an infection caused by digenetic trematode platyhelminths of the genus Schistosoma. These blood flukes use man and other mammals as definitive hosts and aquatic and amphibious snails as intermediate hosts. Of the schistosomal species, S. mansoni, S haematobium and S. japonicum are the most important to man and the most widely distributed. The infection affects about 200 million individuals in 74 countries of Latin America, Africa and Asia. Far less commonly, schistosomes reach the central nervous system (CNS). This may occur at any time from the moment the worms have matured

Diagnosis and antibody status awareness in neuromyelitis optica spectrum disorder and MOG-associated disease: A cross-sectional study.

INTRODUCTION: Neuromyelitis optica spectrum disorder (NMOSD) and MOG-associated disease (MOGAD) present high risk of disability. Early testing for anti-aquaporin-4 (AQP4) and anti-myelin oligodendrocyte glycoprotein (MOG) antibodies is crucial for timely diagnosis and better outcomes. However, patient awareness of these antibodies and their understanding about their disease remains poorly characterized. OBJECTIVES/AIMS: To assess the frequency of antibody serostatus awareness and diagnosis recognition among patients with NMOSD and MOGAD in a tertiary outpatient clinic. METHODS: In this cross

Serum and CSF cytokine profile impact clinical phenotype in pediatric MOGAD.

BACKGROUND: Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) has a broad spectrum of clinical phenotypes, including acute disseminated encephalomyelitis (ADEM), optic neuritis (ON), transverse myelitis (TM), and cerebral cortical encephalitis (CCE). Although these conditions exhibit diverse outcomes and require different treatment options for relapse prevention, no biomarkers have been identified to distinguish the clinical phenotypes. This study aimed to investigate the properties of cytokines and chemokines in the serum and cerebrospinal fluid (CSF) of patients with MO

MOGAD-Like Encephalitic Syndromes Without MOG Antibodies in Children.

BACKGROUND AND OBJECTIVES: Although acute disseminated encephalomyelity (ADEM) can be myelin oligodendrocyte glycoprotein-IgG (MOG-IgG) positive or negative, it is unclear whether other MOG-antibody-associated disease (MOGAD)-like encephalitis syndromes occur without MOG-IgG. We aimed to define the frequency, clinico-radiologic features, outcomes, and antibody associations of such cases in children. METHODS: Prospective cohort study of children (<18 years) with encephalitis meeting clinico-radiologic MOGAD criteria, regardless of MOG-IgG status. Serum from all patients and CSF when available

Risk of neurologic or immune-mediated adverse events after COVID-19 diagnosis in the United States.

INTRODUCTION: Neurologic or immune-mediated conditions have been evaluated as potential adverse events (AEs) in coronavirus disease 2019 (COVID-19) vaccine safety surveillance. To contextualize United States (US) surveillance findings, it is important to quantify the association of AEs with COVID-19 diagnoses among US adults before the introduction of COVID-19 vaccines. METHODS: Cohort and self-controlled risk interval (SCRI) designs were used in 2 US administrative claims data sources-Merative™ MarketScan® Commercial Database (ages 18-64 years) and Medicare fee-for-service data (ages ≥ 65 ye

Early clinical response and complications of therapeutic plasma exchange in central nervous system demyelinating diseases.

BACKGROUND: Appropriate treatment reduces the severity and duration of relapses in demyelinating diseases of Central Nervous System (CNS). If high-dose corticosteroids treatment fails, therapeutic plasma exchange (TPE) is considered as a rescue treatment. OBJECTIVES: This study aimed to investigate early clinical response and complications of TPE and prognostic factors in CNS demyelinating relapses. DESIGN: This prospective observational study was designed in a tertiary center during one year. METHODS: All adult patients diagnosed corticosteroid-resistant Multiple Sclerosis (MS), NeuroMyeli

A comparison of the efficacy of tocilizumab versus azathioprine for neuromyelitis optica spectrum disorder: A study protocol for systematic review and meta-analysis.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) is a chronic inflammatory disease of the nervous system, which is frequently accompanied by a pathological humoral immune response against aquaporin-4 water channel. The most common feature of the disorder is recurrent episodes of longitudinally extensive transverse myelitis and optic neuritis. Frequent relapse leads to the gradual accumulation of neurological dysfunction. Azathioprine (AZA) is an empirical attack -preventive immunotherapies drug to prevent the relapse of NMOSD, and tocilizumab (TCZ) has been also reported reduce the a

[Pregnancy-associated neuromyelitis optical spectrum disorder combined with primary Sjögren's syndrome: A critical illness case report].

Central nervous system involvement in primary Sjögren's syndrome (pSS) is less common and usually presents as white matter lesions, neuromyelitis optica spectrum disorder (NMOSD), or transverse myelitis. NMOSD is an immune-mediated inflammatory demyelinating disease of the central nervous system with a high rate of relapse and significant disability. Studies have shown that patients with pSS combined with NMOSD have more severe symptoms and poorer prognosis. Here, we present a case of critical illness in pregnancy-associated NMOSD combined with Sjögren's syndrome. The patient was a 30-year-old

Rapid myelin water imaging for the assessment of cervical spinal cord myelin damage.

BACKGROUND: Rapid myelin water imaging (MWI) using a combined gradient and spin echo (GRASE) sequence can produce myelin specific metrics for the human brain. Spinal cord MWI could be similarly useful, but technical challenges have hindered routine application. GRASE rapid MWI was recently successfully implemented for imaging of healthy cervical spinal cord and may complement other advanced imaging methods, such as diffusion tensor imaging (DTI) and quantitative T1 (qT1). OBJECTIVE: To demonstrate the feasibility of cervical cord GRASE rapid MWI in multiple sclerosis (MS), primary lateral scl

Extensive hemispheric lesions with radiological evidence of blood-brain barrier integrity in a patient with neuromyelitis optica.

Anti-aquaporin-4 antibody (NMO-IgG) is used as a diagnostic marker for neuromyelitis optica (NMO). Although the mechanism of spinal cord lesions in NMO has been investigated, that of extensive hemispheric lesions with brain edema remains unclear. Here we report a 36-year-old woman with NMO positive for NMO-IgG, who developed an acute disseminating encephalomyelitis (ADEM)-like episode after Mycoplasma pneumoniae infection. Brain MRI T2-weighted images demonstrated asymmetric tumefactive hyperintense lesions in the subcortical white matter. Importantly, no lesions on T1-weighted images were enh

Between Disability and Illness in Ancient Rome - The Case of Emperor Claudius.

Claudius, the fourth emperor of Rome, suffered from multiple health issues, including an unsteady gait, persistent tremors in his arms and head, and a speech impediment that caused him to stutter and mispronounce words since childhood. Despite possessing average or even above-average intellectual abilities, he was considered unfit for public office (cursus honorum) due to these physical conditions. Based on remaining historical records, modern scholars have proposed several possible diagnoses, including cerebral palsy, dystonia, transverse myelitis, or Tourette syndrome. While earlier scholars

Stage-dependent immunity orchestrates AQP4 antibody-guided NMOSD pathology: a role for netting neutrophils with resident memory T cells in situ.

Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune disease of the CNS characterized by the production of disease-specific autoantibodies against aquaporin-4 (AQP4) water channels. Animal model studies suggest that anti-AQP4 antibodies cause a loss of AQP4-expressing astrocytes, primarily via complement-dependent cytotoxicity. Nonetheless, several aspects of the disease remain unclear, including: how anti-AQP4 antibodies cross the blood-brain barrier from the periphery to the CNS; how NMOSD expands into longitudinally extensive transverse myelitis or optic neuritis; how multiphasic
