The middle latency somatosensory evoked potential (SEP), in particular the N60 of median nerve SEP, can be recorded in patients with severe peripheral neuropathy even in the absence of a recordable compound muscle action potential (CMAP). However, clinical utilization of N60 is hampered by its inter-trial variabilities that depend on the level of consciousness, habituation, stimulus rate and/or other unknown factors. We have now obtained a stable N60 SEP when the subject stays awake and alert using a slow stimulus rate. We have applied this method to 17 hands of 15 patients with severe carpal

The value of vibrometry in the evaluation of carpal tunnel syndrome is controversial. Several investigators have suggested that vibrometry screening with multiple frequencies would improve the correlation and increase the sensitivity when screening for carpal tunnel syndrome. One hundred sixty-nine industrial workers from two manufacturing plants in southern Michigan were screened for median nerve impairment using both vibrometry (seven frequencies, 8-500 Hz) and electrophysiologic testing in each hand. The vibratory threshold at each frequency, as well as composite measures-Jetzer index and n

The pathophysiology of spinal stenosis and segmental instability is discussed. The critical magnitude of the cross-sectional area of the cauda equina in the lower part of the vertebral canal is related to the dynamic loading of the spine, nerve impulse conduction and nerve root nutrition. Present knowledge of segmental instability is discussed, and an account is given of findings obtained during dynamic measurements with an intervertebral motion device (IMD), which indicate pathological motion patterns to differ from one pair of vertebrae to another.

Although conduction block indicates dysfunction of peripheral nerve, it may occur in patients with clinically typical motor neuron disease. There are no universally accepted criteria to identify conduction block, so diagnosis may be difficult. In some peripheral neuropathies, conduction block persists over long periods of time. If conduction block persists in motor neuron disease, then a more reproducible means for identification would be available. We repeatedly studied 9 patients with different forms of motor neuron diseases; conduction block was suspected because of excessive loss of the am

Twenty-eight patients with a clinical diagnosis of carpal tunnel syndrome underwent extensive neurographic examination. The results were compared with those in 86 normal subjects who had no signs or symptoms. The widely used methods of measuring distal motor latency from wrist to thenar and the sensory conduction velocity from the first and third fingers to the wrist gave highly significant differences between the carpal tunnel syndrome group and the control subjects. There was, however, a large overlap of values between the two groups and the discriminative power of the tests was not sufficie

A patient with aosmia and night blindness had repeated clinical and electrophysiological examinations during a 21-year period. Within the first 11 years, he experienced two subacute episodes of numbness with weakness and ataxia. Over the 10 ensuing years, no additional functional disability appeared. We looked retrospectively for a correlation between repeated manual muscle scoring and different nerve conduction parameters. Further studies were done in order to estimate reinnervation in proximal and distal muscles and to assess autonomic functions. A direct relationship was found between the m

Motor conduction velocity (MCV) alone cannot separate all the cases with types I and II of hereditary motor and sensory neuropathy (HMSN). However, sensory conduction velocity (SCV) in sural nerve distinctly separated types I and II of HMSN (8). As in most of our patients with HMSN sural nerve was unexcitable, we introduced SCV estimation in the distal segments of median and ulnar nerves. So, we studied 124 patients from families with typical and uncomplicated cases of Charcot-Marie-Tooth disease (CMTD): 68 patients had the "hypertrophic" form (type I) and 56 cases the "neuronal" form (type II

Lateral ankle or foot pain may have various etiologies including entrapment of or injury to the distal sural nerve or early peripheral neuropathy. Antidromic distal sural conduction studies have not been used in clinical neurophysiologic evaluation, however, for technical reasons. The purpose of this study was to determine if by modifying recording parameters and using electronic averaging adequate responses could be obtained to permit standardization of distal sural conduction. Standard surface recording electrodes with an inter-electrode separation of 37 mm were used. The distal sural nerve

Clinical heterogeneity within the group of spinal muscular atrophies (SMA) in children can pose practical problems of diagnosis, prognosis and genetic counselling. In addition to the classic forms, unusual cases have been described. Five children with an atypical type of SMA are reported, characterised by early involvement of cervical muscles contrasting with initially mild or absent affection of peripheral muscles. Secondary involvement of respiratory muscles was responsible for the death of three children in the second to fourth year. The disorder may be recessively inherited.

Comparison of sensory nerve conductions in palmar cutaneous branch (PCB) and digit I nerves was applied in 50 patients suspected of having carpal tunnel syndrome (CTS) and 40 healthy persons. The abnormalities were defined as the differences in latencies and sensory nerve conduction velocities (SNCVs) of more than mean plus 2.5 SD of controls. Comparing these 2 sensory nerves, statistical differences were seen in 83.7% (36 of 43 patients) of their sensory latencies and 76.7% (33 of 43 patients) of their SNCVs. This excluded 7 patients who had unobtainable sensory nerve action potentials in dig

Using "inching technique" we recorded antidromic sensory nerve action potentials from the little finger and compound muscle action potentials from the abductor digiti minimi, first dorsal interosseous and flexor carpi ulnaris muscles in 30 entrapped ulnar nerves. In cubital tunnel syndrome, localized conduction delay occurred most commonly at a point 2 to 4 cm distal to the medial epicondyle. In other ulnar neuropathies, with the exception of cubitus valgus deformity, conduction block or delay was noted at a site just distal to the medial epicondyle. These conduction abnormalities were most co

Muscle hypertrophy following denervation is an uncommon phenomenon. Calf hypertrophy in man with S1 radiculopathy is here reported. Electrophysiological examination of hypertrophic muscles revealed a neurogenic pattern with spontaneous bizarre repetitive discharges. Posterior tibial nerve pharmacological block failed to abolish these discharges. During the following three years the hypertrophy decreased while the spontaneous repetitive discharges persisted unchanged. Relationship between radiculopathy, 'neurogenic' hypertrophy and bizarre repetitive discharges is discussed.

Although it has been known for a long time that in awake cats, natural stimulation of the skin induces short latency responses in rubrospinal cells, the pathway possibly involved has been identified only recently (Padel et al. 1988). This tract, which was described in acute, chloralose anaesthetized cats, ascends in the ventromedial spinal cord and is activated via collaterals of primary afferent fibres running in the dorsal columns of the spinal cord. The present study demonstrates that this newly described spino-rubral tract is able to send detailed somaesthetic information to the red nucleu

Employees from a large midwestern automobile manufacturing plant completed a preemployment evaluation which included a personal and family history, physical examination, and wrist ratio determinations obtained by dividing the anteroposterior diameter by the mediolateral diameter of the wrist. Over a 3-year period, 80 of these employees who developed symptoms compatible with carpal tunnel syndrome within 4-12 months of employment were entered into the study and were evaluated with standard electrodiagnostic techniques. The symptoms included nocturnal hand pain, paresthesia and weak grasp. Thirt

Fourth finger stimulation has been used to obtain the compound nerve action potential (CNAP) of the median and ulnar nerves by a single cutaneous bipolar recording electrode placed in some specific sites of the upper limb. In normal subjects, the response was a combination of both action potentials which could be seen as separated peaks only when the recording was made at midarm with the elbow flexed at 90 degrees. This finding is mainly attributed to the longitudinal sliding of nerves according to the joint movements. In patients with a carpal tunnel syndrome, there were a striking separation

We identified 35 patients who had electrodiagnostic evidence of mononeuritis multiplex and did not have diabetes or multiple nerve compressions. Their charts were reviewed to determine the etiologies of the mononeuritis multiplex and to determine how often the laboratory examination revealed a rheumatic disease in patients whose initial history and physical examination did not suggest that a rheumatic disease was present. In 11/35 (31%; CI = 17-49) a disorder capable of causing mononeuritis multiplex was diagnosed before the symptoms of mononeuritis multiplex began. Ten had a rheumatic disease

1. Single-unit electrical activity has been recorded from fifty-five neurones whose recording sites were located in or immediately adjacent to the marginal zone (lamina I) of the lower thoracic spinal cord (T8-T12) of anaesthetized or decerebrate cats. Their responses to stimulation of somatic and visceral afferent fibres and the sizes of their cutaneous receptive fields have been analysed and compared with the responses and receptive fields of neurones recorded throughout the spinal grey matter. 2. Neurones were classified according to their responses to innocuous stimulation of their somatic

The effects of acetylsalicylic acid, salicylic acid, sodium salicylate and paracetamol on the compound action potentials of the isolated left phrenic nerve of the rat were observed, and compared to that of lidocaine. All drugs depressed the compound action potential in a reversible, dose-dependent way. Lidocaine (inhibition between 0.05 and 10 mM) was more potent than the other drugs and the depression occurred faster. The inhibitory effects of acetylsalicylic acid and salicylic acid were similar to each other (between 1 and 20 mM). Sodium salicylate was less potent (inhibition between 5 and 7
