Stigma Attitudes Toward HIV/AIDS From 2011 Through 2023 in Japan: Retrospective Study in Japan.

BACKGROUND: Stigma associated with HIV/AIDS continues to be a major barrier to prevention, management, and care. HIV stigma can negatively influence health behaviors. Surveys of the general public in Japan also demonstrated substantial gaps in knowledge of HIV/AIDS. Tweets from the social networking service X (formerly known as Twitter) have been studied to identify stigmas in other disorders but have not yet been used to study HIV stigma in Japan. OBJECTIVE: The aim of the study is to examine the variations in stigma related to HIV over an extended period using tweets from X and to investiga

Acceptability and Potential Effectiveness of eHealth Tools for Training Primary Health Workers From Nigeria at Scale: Mixed Methods, Uncontrolled Before-and-After Study.

BACKGROUND: The in-service training of frontline health workers (FHWs) in primary health care facilities plays an important role in improving the standard of health care delivery. However, it is often expensive and requires FHWs to leave their posts in rural areas to attend courses in urban centers. This study reports the implementation of a digital health tool for providing video training (VTR) on maternal, newborn, and child health (MNCH) care to provide in-service training at scale without interrupting health services. The VTR intervention was supported by satellite communications technolog

Application of Behavioral Theories to Disaster and Emergency Health Preparedness: A Systematic Review.

BACKGROUND: Preparedness for disasters and emergencies at individual, community and organizational levels could be more effective tools in mitigating (the growing incidence) of disaster risk and ameliorating their impacts. That is, to play more significant roles in disaster risk reduction (DRR). Preparedness efforts focus on changing human behaviors in ways that reduce people's risk and increase their ability to cope with hazard consequences. While preparedness initiatives have used behavioral theories to facilitate DRR, many theories have been used and little is known about which behavioral t

Clinician Decision-Making Around Offering Home Video Telehealth Visits: Qualitative Study.

BACKGROUND: Use of in-home video telehealth rapidly expanded in response to the COVID-19 pandemic, including at Veterans Affairs (VA), a forerunner in telehealth. Despite this uptick, differences in use by patient age and rurality created a digital divide that persists to this day. While clinicians frequently cite patients' older age and lack of technical skills as barriers to in-home video telehealth, it remains unclear how clinicians decide whether to offer video visits to patients and to what extent these beliefs may hinder offering video visits to older adults. Gathering perspectives from

Privacy Leakage in Federated Learning in Radiology Reports: Comparative Evaluation of Tokenizer and Batch-Size Privacy Risks.

BACKGROUND: Federated learning (FL) enables multi-institutional model training on clinical text without sharing raw data; however, gradient inversion methods can reconstruct sensitive information from shared model updates. The extent of such privacy leakage in FL applied to radiology reports, and the role of tokenizer design, remains unclear. OBJECTIVE: This study aimed to quantify gradient-based reconstruction of radiology report text in an FL setting and to compare privacy risk across 3 transformer tokenization strategies in a controlled, tokenizer-aware evaluation. METHODS: Six FL clients

Impact of Rural Trauma Team Development Education on Prehospital Time, Referral-to-Dispatch Interval, and Neurological and Musculoskeletal Injury Outcomes: Cluster Randomized Controlled Trial.

BACKGROUND: Scarce human resources for health and high injury-related mortality coincide with inequities in accessing quality trauma education programs in low- and middle-income countries. Existing observational studies restrict assessments of trauma training program impacts on providers' knowledge. Evaluation of trauma education programs outside clinical trial settings hinders their effectiveness in influencing clinical practice and policy changes for patient outcomes. OBJECTIVE: This study aimed to assess the impact of the Rural Trauma Team Development Course (RTTDC) on clinical processes a

Support Strategies and Interventions for eHealth Inclusion: Scoping Review.

BACKGROUND: Policymakers increasingly promote eHealth as a way to improve health care efficiency. However, digitalization risks excluding individuals and groups who cannot fully engage with eHealth, for example, due to limited digital literacy or restricted access to resources. Targeted support, such as skills training, personalized guidance, or system-level initiatives, may help, but evidence on how such support is organized and on their outcomes for eHealth inclusion remains limited. OBJECTIVE: This scoping review aimed to map proposed strategies to promote eHealth inclusion, identify concr
