First-stage scapholunate fusion for the treatment of a chronic lunate dislocation: A case report.

RATIONALE: Lunate dislocation is a rare injury to the wrist caused by high-energy trauma, and poor prognosis is often associated with missed initial diagnosis. To date, there is no consensus regarding an effective treatment plan for such cases. PATIENT CONCERNS: Here, we describe the case of a 36-year-old man who presented with lunate dislocation following a delay in diagnosis of over 7 weeks, and further illuminate its diagnosis and treatment. DIAGNOSIS: The diagnosis of chronic lunate dislocation was rendered. INTERVENTIONS: The patient was treated using reduction via the dorsal approach

Does the "three-ligament tenodesis" procedure restore carpal architecture in static chronic scapholunate dissociation?

The effect of the three-ligament tenodesis on the scapholunate angle and scapholunate gap was studied. A comparison was made between these angles preoperatively and postoperatively in 12 patients. There was a significant decrease in the scapholunate angle from 77 to 68 degrees, and the scapholunate gap was reduced from 4.25 mm to 3.29 mm. We also studied the correlation between scapholunate angle and scapholunate gap postoperatively in a cohort of 25 patients and found a significant correlation. This suggests that the principle of the procedure is correct, but the aims are not fully achieved,

Total scapholunate dislocation with complete scaphoid extrusion: case report.

A case of complete dislocation of the lunate and scaphoid resulting from a fall is reported. We are unaware of any previously reported case of simultaneous dislocation with the scaphoid completely extruded from the wrist at the time of injury. The patient was treated with a proximal row carpectomy.

[Atypical Fenton syndrome].

The case of a patient 41 years old is reported. He sustained a posterior open carpal fracture dislocation transcapho-trans-capitate with radius styloid process fracture. The original aspect of this observation is that the lunatum was enucleated. The treatment was not usual. Capitatum bone was first fixed and associated with an external fixation. Proximal row carpectomy was performed in a second procedure when removing internal and external fixation, capitatum being healed. Follow up is now at four years with a very good radiological and clinical result. The patient is working with an intermitt

Postoperative Imaging of the Wrist and Hand.

In this article we will do an overview of the general and specific complications that occur after the most common wrist and hand surgeries. Knowledge of the different surgical techniques is essential for postoperative imaging evaluation. General complications include infection, complex regional pain syndrome, problems related with the surgical approach (open or arthroscopic) and bone healing problems. The most frequent fractures of the wrist with specific complications are distal radius fractures and scaphoid fractures as associated with tendon ruptures secondary to friction, nonunion or secon

A Rare Presentation of Extensor Pollicis Longus Tendon Rupture After Nondisplaced Scaphoid Fracture.

Extensor pollicis longus tendon rupture is a reported complication after nondisplaced distal radius fractures. These are thought to occur secondary to mechanical irritation or compromised blood supply. We present a case of extensor pollicis longus rupture after a healed nondisplaced scaphoid fracture, which may have involved a similar attritional process. We are unaware of any prior reports of extensor pollicis longus rupture after this type of injury.

Isolated Scaphoid Dislocation With Radial-Axial Instability: A Treatment Strategy Utilizing Spanning Wrist Plates.

Isolated dislocation of the scaphoid is a rare injury with only a few case reports in the literature. We report on 2 complex scaphoid dislocations demonstrating concomitant axial instability with disruption of the capitohamate articulation as well as the long-ring metacarpal relationship. Both of these patients underwent reduction and fixation using a wrist spanning plate, which was removed approximately 2 months after injury. Follow-up of these patients demonstrated maintenance of reduction, axial stability, and return of painless range of motion.

Three-and-a-half to 18 year outcome data for costal osteochondral grafts for salvage of the proximal pole of the scaphoid.

Twelve patients who had undergone costal osteochondral graft reconstruction of the proximal pole of scaphoid were evaluated with clinical examination, patient-reported outcome scores and radiographs with an average follow-up of 10 years (range 3.5-18). The range of wrist motion was not significantly changed compared with the preoperative range of motion and functional outcomes scores were acceptable. The patients reported low pain scores despite the universal presence of radiographic changes of reduced carpal height and arthritis of the midcarpal and radiocarpal joints. Costal osteochondral gr

Role of wrist arthroscopy in scapholunate dissociation.

Scapholunate ligament tear is most frequently encountered in the aftermath of trauma in supination and extension of the wrist. It generates chronic instability, leading to osteoarthritis. It may be associated with fracture of the distal epiphysis of the radius or of the scaphoid. These lesions are often difficult to diagnose, especially in early stages. Treatment of chronic scapholunate ligament lesions before onset of osteoarthritis is a challenge for the surgeon. To date, recommendations are for open reconstruction or repair, which can improve pain and grip strength, but very often at the co

Flexor Tendon Ruptures After Distal Scaphoid Excision for Scaphotrapeziotrapezoid Osteoarthritis.

BACKGROUND: Distal scaphoid excision is one treatment option for osteoarthritis of the scaphotrapeziotrapezoid (STT) joint following failure of conservative measures. Potential complications of this procedure include injury to the carpal ligaments, cartilage, and radial artery. METHODS: A single case was identified by the senior author, and the medical record was reviewed for surgical notes, progress notes, and radiographs. RESULTS: A 68-year-old male sustained ruptures of the flexor digitorum superficialis (FDS) and flexor digitorum profundus to the index finger 3 years following a distal s

[Arthroscopic styloidectomy].

The radial styloidectomy is a symptomatic treatment of the radio scaphoidal impingement. Famous many years ago for the treatment of post-traumatic arthritis of the wrist (SLAC, SNAC wrist) this procedure has been progressively abandoned. The miniaturization of the wrist's arthroscopical tools allows stiloidectomy under arthroscopy. This treatment allows the disappearance of the impingement and protects the surrounding soft tissue. After a short historical, anatomical and biomechanical study, the authors expose the technique of the styloidectomy under an arthroscopic procedure.

Magnetic resonance imaging of suspected scaphoid fractures using a low field dedicated extremity MR system.

AIM: To determine whether magnetic resonance imaging (MRI) using a dedicated extremity system could be utilized for the routine assessment of patients with a clinically suspected scaphoid fracture, and to determine the most cost-effective timing of MRI for these patients. MATERIALS AND METHODS: MRI of the wrist using a 0.2T extremity MR system was performed in two groups of patients. Fifty-six patients were examined within 4 days of injury and 53 with persistent symptoms at 10 days to 6 weeks following injury. A scaphoid series of radiographs was negative in both groups. RESULTS: The early M

Effect of four-corner fusion with locking plate without bone graft on functional recovery of the wrist: New treatment guidelines.

We assessed the effect of four-corner intercarpal fusion with locking plate without bone graft on daily activities and pain in patients with stage II and III scapholunate advanced collapse and scaphoid nonunion. Twenty-one patients who underwent four-corner fusion with scaphoidectomy without bone graft were evaluated with the Disabilities of the Arm, Shoulder and Hand (QuickDASH) and Visual Analog Scale (VAS) pain scores before and 16 months after surgery. We also compared postoperative grip strength between the operated and the healthy side. A principal component analysis was used to establis

Bone-ligament-bone Cuenod grafting technique modified by Saffar-Romano for the treatment of chronic scapho-lunate dissociation.

BACKGROUND AND AIM OF THE WORK: The authors show their experience about six patients suffering of chronic scapho-lunate (S-L) dissociation treated with the Cuenod method modified by Saffar-Romano. METHODS: Clinical assessment was performed in all patients and compared before and after surgery at follow-up measuring pain value through the VAS, ROM of the wrist with a goniometer, grip strength by Jamar test, functional outcome with the Italian version of the Disability of Arm, Shoulder and Hand (DASH) score. Radiological assessment consisted in measuring S-L angle and classifying radio-carpic a

The scaphocapitate fracture syndrome: a case report and literature analysis.

Scaphocapitate fracture syndrome is rare, complex injury. We report an unusual presentation of scaphocapitate fracture syndrome, involving fracture of the scaphoid and capitate associated with volar dislocation of the lunate and scaphoid and the proximal fragment of the capitate in a 30-year-old male after a motor vehicle accident. Computed tomography was found to be helpful for achieving the correct diagnosis. Open reduction and internal fixation was performed. The scaphoid fracture was fixed using a headless compression screw, and the volar displaced proximal fragment of the capitate was red
