Intranasal cross-sectional area and quality of life changes following endoscopic transsphenoidal skull base surgery.

BACKGROUND: Endoscopic transsphenoidal surgical approaches to the skull base may produce alteration of intranasal architecture. Acoustic rhinometry (AR) can provide quantitative data on the nasal airway, which has not been previously applied to skull base surgery. We sought to employ AR to quantify the changes in nasal patency following endoscopic transsphenoidal surgery and identify the relationship between these changes and septal deviation, nasoseptal flap harvest, and sinonasal quality of life (QOL). METHODS: Consecutive patients undergoing endoscopic transsphenoidal resection of a sellar

Lesions involving the jugular foramen: clinical characteristics and surgical management.

CONCLUSION: Lesions involving the jugular foramen (JF) present as various diagnoses. Pulsatile tinnitus is more common in glomus jugulare (GJ) tumors, whereas otalgia and facial nerve paresis are more prevalent in temporal bone malignancies (TBMs). Preoperative facial nerve electroneurography (ENoG) was significantly correlated with postoperative facial nerve function. OBJECTIVE: To describe the diagnosis and surgical management of lesions involving the JF. METHODS: The charts were reviewed for 38 patients who had lesions involving the JF, including 14 patients with TBMs, 11 with GJ tumors,

Lymphoma of the trigeminal nerve--the need for histological diagnosis.

CNS lymphoma involving the trigeminal nerve is a rare condition which presents as a cavernous sinus lesion. It may mimic the radiological appearance of other lesions, and biopsy is essential before considering empirical radiotherapy for lesions in this region. We report the radiological, histopathological and operative findings of a primary non Hodgkin B cell lymphoma involving the trigeminal nerve.

High incidence of optic canal involvement in tuberculum sellae meningiomas: rationale for aggressive skull base approach.

BACKGROUND: Current literature on TSMs underemphasizes the significance of OCI by the tumor. In this study, we aimed to document the incidence of OCI, its management using a SBT, and its significance with relation to the visual outcome. METHODS: Thirty-one patients with TSM were retrospectively analyzed. In 28 patients, SBT consisting of extradural anterior clinoidectomy with falciform ligament and optic nerve sheath opening was performed. Pre- and postoperative visual status was analyzed with respect to OCI. RESULTS: The incidences of OCI and preoperative visual deficit (VD) were 77.4% and

Prognostic significance of transdural invasion of cranial base malignancies in patients undergoing craniofacial resection.

OBJECTIVE: Invasion of the brain and/or dura is a known negative prognostic factor for patients undergoing craniofacial resection for cranial base malignancy. However, an evaluation of factors that may affect prognosis in this patient subgroup has not been undertaken. METHODS: Between 1993 and 2003, 212 patients underwent craniofacial resection for primary malignancy of the cranial base at the University of Texas M.D. Anderson Cancer Center. Twenty-eight patients (eight women, 20 men; median age, 52 yr; age range, 26-76 yr) had evidence of transdural spread (subdural tumor or brain invasion)

[Results of 13 hypoglosso-facial anastomoses].

BACKGROUND AND PURPOSE: Facial nerve paralysis significantly decreases the quality of life in patients after skull base tumors surgery. There are different options for surgical repair of the nerve. One of the most effective methods is the anastomosis of the distal trunk of the facial nerve with the proximal trunk of the hypoglossal nerve or with hypoglossal loop. MATERIAL AND METHODS: Thirteen hypoglosso-facial anastomoses had been performed in our department between 1999 and 2003. In all patients the facial nerve paralysis was due to the surgery in its proximity. In one case the patient suff

Navigation as a tool to visualize bone-covered hidden structures in transfrontal approaches.

A retrospective analysis of 10 patients was performed to evaluate navigation systems in extranasal frontal skull base surgery. When performing a craniotomy following a bicoronal skin incision, the surgeon has to calculate the extent of the frontal sinus to avoid unnecessary damage to the dura or mucoceles later. Due to surgical morbidity including compression of the frontal lobe, many skull base surgeons have refused to use such an approach. Malformation or bone-destruction complicates the identification of the borders and increases the risk of side-effects. Navigation systems can be an altern

Petroclival meningioma. An attempt to define the role of skull base approaches in their surgical management.

BACKGROUND: Petroclival meningiomas represent a difficult surgical challenge. The introduction of modern techniques of skull base surgery has stimulated the interest of the neurosurgical community in the surgical management of these lesions, although very good results have been obtained by experienced neurosurgeons in cases of petroclival meningioma operated using traditional surgical approaches. METHODS: Thirty-one cases of petroclival meningioma have been operated on during a 4-year period using two different philosophies as far as the approach. Group A patients (13 cases) have been operate

Laser Interstitial Thermal Therapy for Posterior Fossa Lesions: An Initial Experience.

BACKGROUND: The application of laser interstitial thermal therapy (LITT) for intracranial lesions in the posterior fossa tumors remains challenging due to the smaller size of this compartment as well as the thickness and angle of the occipital bone. In this study, we reviewed our experience with this treatment modality for posterior fossa lesions. METHODS: We retrospectively reviewed our series of 8 patients with posterior fossa tumors treated with LITT from an Institutional Review Board-approved brain tumor database (2012-2017) of more than 200 cases at our institution. RESULTS: The 8 patie

[Usefulness of CT-angiography for superselective intra-arterial chemotherapy for advanced head and neck cancers].

Eighteen N3 cases, fourteen skullbase invasion cases and twenty-six cases of paranasal sinus cancer with orbital invasion were treated by superselective intra-arterial chemotherapy using CDDP and sodium thiosulfate to preserve the organs and to improve poor prognosis. In these patients, 100-150 mg/m2 of CDDP was administered weekly to each feeding artery of the tumor superselectively at 5 mg/m. CT-arteriography (CTA) was used to diagnosis all feeding arteries of advanced cancers before infusing CDDP. Twenty-three of 26 cases with orbital invasion were treated with preservation of the eyeball.

Endoscopic endonasal surgery and radiotherapy in skull base chordomas: a 20-year retrospective analysis of long-term outcomes and prognostic factors.

BACKGROUND: For skull base chordomas, two key aspects remain controversial: (1) the necessity of adjuvant RT following gross total resection (GTR) and (2) the comparative long-term efficacy of different RT modalities. OBJECTIVE: This study aimed to evaluate the effects of endonasal endoscopic surgery, RT, and other variables on progression-free survival (PFS) and overall survival (OS) in patients with skull base chordomas. METHOD: A retrospective analysis was conducted on 83 patients (2006-2025) treated at the Affiliated Eye Ear Nose and Throat Hospital, Fudan University, to assess the progn
