Excited delirium deaths in custody: past and present.

First identified in institutionalized psychiatric populations, chronic excited delirium syndrome was not uncommon in the first half of the 20th century. After a temporal pause, excited delirium re-emerged in the 1980s, in an acute form. Generally occurring in victims without organic mental disease, acute excited delirium is associated with stimulant abuse. This exploratory research examines the evolution of excited delirium deaths in custody to determine if medical examiner cases in Maryland reflect the historical and clinical patterns identified in the existing literature. All deaths occurrin

Cardiorespiratory function in stable methadone maintenance treatment (MMT) patients.

Patients in methadone maintenance programmes (MMT) often smoke tobacco and cannabis and many have ongoing illicit drug use. There is therefore potential for these patients to have abnormal cardiorespiratory function; however, few studies address this in stable MMT patients. We assessed resting cardiorespiratory function on 50 stable MMT patients (25 males, 25 females). Forty-six MMT patients were current tobacco smokers, 19 were current cannabis users and none were currently using opioids other than prescribed methadone. We defined abnormalities of respiratory function as those results outside

Motor vehicle crashes and positive toxicology screens in adolescents and young adults.

The purpose of this study was to identify and describe the outcome of motor vehicle crashes for adolescents and young adults with positive toxicology screens. A retrospective design was used to collect data for 134 subjects ranging in age from 15 to 25 at an urban Level II trauma center. Outcomes related to sex, age, injury-severity score, length of stay, and hospital cost were analyzed using multiple regression, and the relationship was significant (p < .0001). Spearman's correlation analysis resulted in a significant relationship between survival outcome, injury severity, discharge, and hosp

The prevalence and effect of alcohol and drug abuse on cohort-matched critically injured patients.

A prospective study was undertaken at a Level I trauma center to evaluate the prevalence of substance use among victims of major trauma, along with the impact on clinical outcome. Five hundred sixteen patients had urine toxicology and blood alcohol screens performed and correlated with pattern and severity of injury, hospital course, and outcome. Three hundred seventy-one patients (71%) screened positive for alcohol or drugs, or both. Fifty-two per cent had positive alcohol screens, and 42 per cent had positive drug screens (cocaine and opiates represented 91% of positive drug screens). Univar

Use of a visual panel detection method for drugs of abuse: clinical and laboratory experience with children and adolescents.

We evaluated the Triage panel for drugs of abuse, a visual method that simultaneously detects seven distinct drug classes in a single aliquot of urine, by use of 1214 urine specimens obtained from children and adolescent patients whose clinical findings warranted a toxicology evaluation. A total of 295 positive results were confirmed by gas chromatography-mass spectrometry. Additional toxicology investigations were not performed on specimens with negative results unless the clinical findings did not correspond with the urine Triage results. The positive predictive value of the test was found t

Prevalence of illicit drugs detected in the urine of women of childbearing age in Alabama public health clinics.

Each year, it is estimated that from 350,000 to 739,000 U.S. infants are exposed in utero to one or more illicit drugs. To estimate the prevalence of and risk factors for illicit drug use by women of childbearing age in Alabama, during 2 months in 1991 the authors collected patient-reported histories, clinical histories, and urine specimens from 6,195 women statewide attending public health maternity clinics, family planning clinics, and a high-risk referral obstetrical clinic. Blind drug screening of urine specimens for marijuana, cocaine, opiates, barbiturates, and amphetamines was performed

Parental pattern, childhood trauma, and impulsivity contribute to the severity of crack cocaine addiction: a cluster analysis.

OBJECTIVE: To determine clusters (groups) of substance use in crack cocaine users through severity scores on the sixth version of the Addiction Severity Index (ASI-6) and compare the groups in relation to risk factors, such as parental style, childhood maltreatment, and impulsivity. METHODS: This cross-sectional study included 531 men with substance use disorder who were admitted to an inpatient addiction treatment unit in southern Brazil. To detect more homogeneousgroups of individuals, the K-means clustering based on ASI-6 scores was used to create groups of individuals with similar severit

Exploring the Metabolism of Flubrotizolam, a Potent Thieno-Triazolo Diazepine, Using Human Hepatocytes and High-Resolution Mass Spectrometry.

BACKGROUND: The abuse of psychoactive substances presents challenges in clinical and forensic toxicology. The emergence of novel and potent drugs that pose significant health risks, in particular towards frequent abusers and users unaware of the ingredients, further complicates the situation. Designer benzodiazepines have become a fast-growing subgroup of these new psychoactive substances (NPSs), and their overdose may potentially turn fatal, especially when combined with other central nervous system depressants. In 2021, flubrotizolam, a potent thieno-triazolo designer benzodiazepine, emerged

Indicators for secondary carcinoma in head and neck cancer patients following curative therapy: A retrospective clinical study.

Patients with head and neck squamous cell carcinoma (HNSCC) are at risk for local and regional relapse, as well as for occurrence of synchronous or metachronous secondary carcinoma. The aim of this retrospective study was to evaluate the frequency of secondary malignancies amongst HNSCC patients and their association with age, sex and TNM classification, as well as smoking and alcohol abuse. Data from 394 patients with HNSCC who were treated at the Department of Otorhinolaryngology-Head and Neck Surgery of the University Hospital Mannheim between 2011 and 2015 were retrieved and analyzed using

Decreased NOX2 expression in the brain of patients with bipolar disorder: association with valproic acid prescription and substance abuse.

Neuroinflammation and increased oxidative stress are believed to contribute to the development of psychiatric diseases. Animal studies have implicated NADPH oxidases (NOX) as relevant sources of reactive oxygen species in the brain. We have analyzed the expression of NOX isoforms in post-mortem brain samples from patients with psychiatric disorders (schizophrenia, bipolar disorder) and non-psychiatric subjects. Two collections from the Stanley Medical Research Institute were studied: the Array Collection (RNA, 35 individuals per group), and a neuropathology consortium collection (paraffin-embe

An accidental poisoning with mitragynine.

An increasing number of drugs of abuse are sold word wide over the internet. Names like "legal highs", "herbal highs" etc. give the impression that these are safe products, although the risk of fatal reactions might be substantial. Leaves from the plant Mitragyna speciosa, contain active compounds like mitragynine and 7-hydroxymitragynine. It has been reported that the potency of 7-hydroxymitragynine at the μ-opioid receptor is 30 times higher than that of mitragynine and 17 times higher than that of morphine. Case reports regarding poisoning with Kratom are reported, but the toxic or lethal r

Intoxication and settled insanity: a finding of not guilty by reason of insanity.

This article presents a case of first-degree murder for which the defendant was acquitted as not guilty by reason of insanity, based on a defense involving the concept of "settled insanity." The literature on settled insanity is reviewed and discussed in the context of voluntary and involuntary intoxication. Statute and case law from those jurisdictions in which settled insanity is specifically allowed as an acceptable threshold condition for the insanity defense define the concept as a permanent condition resulting from substance abuse, rather than the effects of intoxication, no matter how s

Impaired performance in a test of decision-making by opiate-dependent tobacco smokers.

This study tested whether opiate dependence, tobacco smoking, or their combination accompanied impaired performance on the gambling task (GT), which tests decision-making. GT previously detected impairments in patients with lesions of the ventromedial prefrontal cortex and in substance abusers. Four groups were matched on demographic characteristics and intelligence: methadone-maintained smokers (n = 9) and nonsmokers (n = 9), and control (i.e., not opiate-dependent) smokers (n = 9) and nonsmokers (n = 10). The Wisconsin Card Sorting Task (WCST) was administered to test whether differences in

Detecting synaptic autoantibodies in psychoses: need for more sensitive methods.

PURPOSE OF REVIEW: Schizophrenic psychosis affects near 1% of the population. It typically starts in the first three decades of life, leading most often to chronic disability: antipsychotic treatment is palliative, not curative. The neurobiological abnormalities underlying psychoses are likely to differ across patients, ranging from autosomal dominant genetic disease to substance abuse, but a decreased function of the N-methyl-D-aspartate (NMDA) receptor seems to be a common theme. Emerging evidence suggests that decreased NMDA receptor function may be caused by auto-antibodies against this re

Suicidal behaviour and suicide prevention in later life.

Despite a general decline in late life suicide rates over the last 30 years, older people have the highest rates of suicide in most countries. In contrast, non-fatal suicidal behaviour declines with age and more closely resembles suicide than in younger age groups. There are difficulties in the detection and determination of pathological suicidal ideation in older people. Multiple factors increase suicide risk ranging from distal early and mid-life issues such as child abuse, parental death, substance misuse and traumatic life experiences to proximal precipitants in late life such as social is

Arrest and Prosecution of Animal Sex Abuse (Bestiality) Offenders in the United States, 1975-2015.

Bestiality is an uncomfortable topic about which there is limited scientific literature. Prevalence research has focused on self-reported acts, and no research has established the actual frequency and nature of deliberate sex acts with animals. This quantitative, descriptive study examined 456 arrests for bestiality-related incidents in the United States from 1975 to 2015 to explore patterns of offending, offender characteristics, and how cases were adjudicated. The results suggest that animal sex offending may be linked to other criminal behavior, and involves a spectrum of sexual acts, inclu

Designer drugs: past history and future prospects.

Historically, drugs of abuse have come from two sources: plant products and diverted pharmaceuticals. Today, new, totally synthetic drugs produced by clandestine laboratories have become an increasingly important source of abused substances. Of particular concern are the fentanyls, a family of very potent narcotic analgesics, which first appeared on the streets in California in 1979 under the name "China White". At least 10 different analogs have been identified to date and are thought to be responsible for over 100 overdose deaths. The fentanyls are not used by any particular ethic or age gro

[Use of psychotropic substances by the elderly and driving accidents].

In the context of a study for the Federal Highway Research Institute (Bundesanstalt für Strassenwesen), a database containing almost 40,000 toxicological blood analysis results from drivers with various traffic offences (time frame 1998-2001) from throughout Germany was evaluated. The database contains the results of 25 laboratories in Germany performing toxicological blood analysis of driving offences. Of these laboratories, 23 gave information about their methodology for toxicological analysis by questionnaire. This database and the results from 10,000 toxicological blood analyses of traffic

Potential neonatal toxicity of new psychoactive substances.

Cannabis, cocaine, 3,4-methylenedioxymethamphetamine, and lysergic acid diethylamide are psychoactive substances with a significant increase in consumption during the 21st century due to their popularity in medicinal and recreational use. New psychoactive substances (NPSs) mimic established psychoactive substances. NPSs are known as being natural and safe to consumers; however, they are neither natural nor safe, causing severe adverse reactions, including seizures, nephrotoxicity, and sometimes death. Synthetic cannabinoids, synthetic cathinones, phenethylamines, and piperazines are all exampl

Recreational Drug Overdose-Clinical Value of Toxicological Analysis.

BACKGROUND: Toxicological analysis of patients with acute recreational drug poisoning can improve our understanding of substance use patterns, clinical symptoms, and improve treatment. Patient history alone may be incomplete or misleading. The objective was to assess the differences in patient history and analytical results, to describe the clinical characteristics, implications and hospital management, and to describe the drug use pattern over time. METHODS: A retrospective study including all patients admitted to our toxicology unit with recreational drug toxicity and analytical testing fro
