Dynamic versus static reconstruction of the medial patellofemoral ligament for recurrent lateral patellar dislocation.

PURPOSE: To compare clinical and radiological outcomes of static and dynamic medial patellofemoral ligament (MPFL) reconstruction techniques. METHODS: In a retrospective, matched-paired, cohort analysis, 30 patients surgically treated for recurrent lateral patellar dislocation were divided into two groups of 15 patients matched for inclusion and exclusion criteria. The static technique group underwent rigid fixation of the gracilis tendon at the anatomic femoral MPFL insertion and the superomedial border of the patella; the dynamic technique group underwent detachment of the gracilis tendon a

Phylogenetic variation in flexor digitorum brevis: a Nepalese cadaveric study.

Understanding muscle architecture of the foot may assist in the design of surgical procedures such as tendon transfer, biomechanical modeling of the foot, prosthesis design, and analysis of foot function. Flexor digitorum brevis (FDB) is the most superficial intrinsic muscle of the sole of the foot. When compared to the little finger, the little toe has less function and opposition is absent in humans. Hence, the muscles acting on the little toe are undergoing evolutionary changes. The objective of this study was to review the phylogenetic variation occurring in FDB in the Nepalese cadavers. S…

Allograft reconstruction of peroneal tendons: operative technique and clinical outcomes.

BACKGROUND: Irreparable peroneal tendon tears are uncommon and require complex surgical decision making. Intercalary segment allograft recon … nt-reported outcomes. It can be performed without incurring the deleterious effects associated with tendon transfer procedures. We believe that allograft reconstruction is a safe and useful alternative in the treatment of irreparable peroneal tendon ruptures. LEVEL OF EVIDENCE: Level IV, retrospective case series.

Demystifying the Radial Nerve The Management of Radial Nerve Palsy in the Setting of Humeral Shaft Fracture.

The association of radial nerve palsy and humeral shaft fracture is well known. Primary exploration and fracture fixation is recommended for open fractures and vascular injury while expectant management remains the standard of care for closed injuries. In the absence of nerve recovery, exploration and reconstruction is recommended 3 to 5 months following injury. When direct repair or nerve grafting is unlikely to achieve a suitable outcome, nerve and tendon transfers are potential options for the restoration of wrist and finger extension.

Reconstruction of paralyzed biceps brachii muscle by transposition of pedicled latissimus dorsi muscle: report of two cases.

The biceps brachii muscle was reconstructed in two patients with inveterate injuries of the brachial plexus. The latissimus dorsi muscle was transferred on the thoracodorsal neurovascular bundle into the regio brachii anterior and sutured to the long head of the m. biceps brachii. The distal part of the latissimus dorsi muscle was sutured above the tuberositas radii along with the insertion tendon of the biceps brachii muscle. Both patients achieved very satisfactory restoration of flexion in the elbow joint 12 and 16 months after surgery.

Thumb reconstruction--great toe transfer.

Thumb reconstruction has a long and exciting history spanning more than 100 years. Reflecting on the evolution of techniques, the problems encountered and solved, and the advancement of technologies, one is struck by the versatility of surgeons in their varied approaches to the restoration of the thumb. Philosophical differences over the relative advantages and disadvantages of each operation have fueled debate over the choice of technique, but a unifying concept in reconstruction "to replace the lost tissue with like tissue" must be sought. The ideal reconstruction is one that replaces all of

Wrist Extension Reconstruction Using Distal Anterior Interosseous to Extensor Carpi Radialis Brevis Nerve Transfer in Brachial Plexus Injuries.

PURPOSE: Reconstruction of wrist extension in C5-C8 root brachial plexus injuries (BPIs) is challenging, because of limited nerve and tendon donors. The purpose of this study was to report outcomes relating to the pronator quadratus branch of the anterior interosseous nerve (AIN) to extensor carpi radialis brevis (ECRB) nerve transfer from three BP surgeons. METHODS: Ten patients with C5-C8 BPI underwent AIN to ECRB nerve transfer. The mean age of the patients was 30.5 ± 15.9 years. All patients were men. The mean time to nerve surgery following initial injury was 4.5 ± 1.2 months. In all pat

Management of patellar subluxation. A modification of Hauser's technique.

Patellar subluxation is one of the most common causes of internal derangement of the knee in young individuals. Most surgical techniques mandate entrance into the knee joint and require prolonged postoperative immobilization. The result may be excessive postoperative synovitis and occasionally, hemarthrosis. Prolonged postoperative immobilization results in joint stiffness. The authors propose a modification of Hauser's technique which allows tailored correction of the abnormal vector forces applied to the patella, simple adjustment of the tension in the extensor mechanism, creates patellar st

Outcome of latissimus dorsi transfer as a salvage procedure for failed rotator cuff repair with loss of elevation.

Eighteen patients, referred from an outside institution with massive, irreparable rotator cuff tears and loss of elevation, were treated with a latissimus dorsi tendon transfer as a salvage procedure for failed, prior, attempted rotator cuff repair. Clinical outcomes were measured by the American Shoulder and Elbow Surgeon's (ASES) score, pain level, and active range of motion. The average postoperative ASES score was 61, an increase from 43 pre-operatively (P = .05). Active elevation improved to an average of 137 degrees compared to 56 degrees pre-operatively (P < .001). The average post-oper…

[Electromyographic analysis of the leg muscles in the hemiplegic patients with equinovarus deformity of the foot].

A series of EMG study of the leg muscles was carried out with a wire electrode in 86 hemiplegic patients of stroke to visualize the role of each muscle either in the development of equinovarus deformity of the foot or in correcting the deformity through tendon transfer. The muscles examined were anterior tibialis, posterior tibialis, gastrocnemius, soleus, flexor digitorum longus and peroneus brevis of the affected side. Tonic discharge of those muscles was recorded as the patients were elevating the affected leg in supine, sitting or standing posture or were standing on legs. On lifting up th…

Distal nerve transfer for thenar palsy: A cadaveric study.

INTRODUCTION: Severe proximal median nerve palsies often result in irreversible thenar atrophy and thumb abduction function loss. Tendon transfer involves substantial limitations and challenges; but, distal nerve transfer may provide an alternative treatment. Our goal was to validate the anatomical suitability of two distal ulnar nerve branches for thenar muscle reanimation. MATERIALS AND METHODS: We assessed nerve transfer to the recurrent branch of median nerve (RMN) in 16 embalmed cadaveric hands. The ulnar motor branch to the flexor digiti minimi brevis (FDMBn) and the ulnar motor branch …

Biomechanics of the patellofemoral joint and its clinical relevance.

An analysis of the patellofemoral joint based on experimental determinations of pressure distributions on the patellar cartilage and vectorial calculations is presented. The extension torque of all quadriceps muscles is similar. The force transmitted to the patella by the retinacula is small in all knee positions. Because the rigid patella has its own lever arms and tendon insertions at different heights, the forces on patellar and quadriceps tendons are unequal. The small patellar contact areas change according to flexion and are dependent on quadriceps force. Retropatellar pressure is thereb

[Anomalies of muscles and tendons in hand and wrist (author's transl)].

Anomalies of muscles and tendons in hand and wrist are not uncommon. Their signification in practicing handsurgery must be known. They can rarely be foreseen. When found at random during operation, they may be often neglected. But they are sometimes the cause of different syndromes such as disability of some artist's hands, painful wrist by sports of efforts, nerve compression simulating a carpal tunnel syndrome, camptodactylia, etc. They may also necessitate a modification of planning tendon transfers or grafting. On the dorsal aspect one must point out the extensor digitorum brevis; the abse…

Biomechanical comparison of 3 latissimus dorsi transfer sites for reverse total shoulder arthroplasty in the absence of teres minor.

BACKGROUND: Reverse total shoulder arthroplasty is often performed to treat rotator cuff tear arthropathy with irreparable rotator cuff tears. Patients with full-thickness tears involving the posterior cuff and teres minor specifically lose active external rotation of the humerus, limiting activities of daily living. A latissimus dorsi transfer has been described as one potential solution, but few studies have compared different latissimus dorsi transfer sites. This study assesses the biomechanics of 3 latissimus dorsi transfer sites, examining external rotation, deltoid muscle forces, and for

Use of a Medial Femoral Condyle Free Flap for Metacarpal Reconstruction of Type IIIC Congenital Thumb Hypoplasia.

This is a case report of a 14-year-old girl with a Blauth Type IIIc hypoplastic thumb who underwent metacarpal reconstruction with a medial … ected not to undergo the planned second stage of opposition transfer, first webspace deepening, and tendon transfer for the time being, although these procedures may increase her function further. This case presents a novel application of the MFC flap to preserve a five-digit hand in a patient with congenital thumb hypoplasia, with minimal donor site morbidity.
