Liver transplantation for hepatoblastoma.

From October 1993 to February 2007, 25 liver transplantations were performed for hepatoblastoma. Of these 25, 18 children received cadaveric grafts, and 7 received left lateral segments from living donors. Fifteen patients were at level IV in the pretreatment extent of disease staging system for hepatoblastoma (PRETEXT IV; 11 received cadaveric grafts and 4 underwent living related liver transplantation [LRLT]) and 10 were level III (PRETEXT III; 7 received cadaveric grafts and 3 underwent LRLT). Preoperative chemotherapy was given according to the risk stratification system for children with

[Split-liver transplantation].

Split-liver transplantation is now established as a safe and successful technique that extends the donor pool for patients of all ages and thus reduces waiting-list mortality, although it can not solve the problem of organ shortage alone. Split-liver transplantation additionally represents an alternative to living liver transplantation without a potential risk of harm to the donor. Careful selection of donor and recipient, high technical and surgical skill, and experience are necessary to achieve results comparable to those of whole organ transplantation.

Renal transplantation today.

BACKGROUND: The first successful renal transplant was carried out more than five decades ago between identical twins. At these early days, acute rejection was the limiting factor. DISCUSSION: Due to tremendous progress in immunosuppressive therapy and surgical technique, today, renal transplantation is the gold standard therapy for patients with end-stage renal disease. In fact, in comparison with chronic hemodialysis, renal transplantation offers an increase in quality of life while reducing comorbidities associated with dialysis treatment. RESULTS: Despite numerous beneficial achievements,

[Meniscal allograft transplantation].

Loss of meniscal cartilage leads to degenerative arthritis. Therefore, it is important to save the meniscus whenever possible. But not all damaged menisci can be treated by partial resection or repair. The concept of meniscal transplantation has been suggested and has been become more common in recent years. Meniscal transplantation should be considered for the young patient who has not developed advanced osteoarthritis. Ligament stability and normal knee alignment are further requirements for meniscal transplantation. At this time, meniscal allografts provide the best a priori conditions to p

Updates in heart transplantation.

Heart transplantation (HTx) has been a successful therapy for patients with end-stage heart failure. Since 1987, we have performed 288 HTx. Thirty-six subjects needed mechanical support prior to HTx. We use anti-thymocyte globulin (ATG) as induction therapy and low-dose immunosuppressive agents for maintenance treatment. In June 1996, we performed combined heart and kidney transplantation after bridging for 14 days with an indigenous total artificial heart (TAH). The patient is still well. Our actuarial survival rates at 1, 5, and 10 years are 86%, 76%, and 61%, respectively. One recipient who

Overview of lung transplantation.

Although significant gains have been made in improving lung function and survival in cystic fibrosis (CF), ultimately respiratory failure is the leading cause of mortality in these patients. For CF patients with end stage lung disease, lung transplantation is an option for treatment. The field of lung transplantation has progressed markedly in the last 20 years. Nonetheless it remains a technically complex and challenging procedure, and patients are at risk for numerous short term and long term complications. Potential transplant recipients must be physically and psychologically prepared for t

Vascularized epiphyseal transplant.

In skeletally immature patients, the transfer of vascularized epiphysis along with a variable amount of adjoining diaphysis may provide the potential for growth of such a graft, preventing future limb length discrepancy. This article describes the authors' experience with the vascularized transfer of the proximal fibular epiphysis in the reconstruction of large bone defects including the epiphysis in a series of 27 patients ranging in age from 2 to 11 years. The follow-up, ranging from 2 to 14 years, has been long enough to allow some evaluation of the validity, indications, and limits of this

[Transplant patient monitoring].

Management of transplant recipients consists in a good balance between an optimal graft function and the potential toxic effects of immunosuppressive drugs. Indeed, transplant patient monitoring has to be very meticulous and includes repetitive clinical, biological and morphological tests. Acute or chronic graft dysfunctions should be identified very soon in order to confirm diagnosis by pathologic analyses and to adapt the immunosuppressive treatment. On the other hand, the complications occurring in transplant patients have to be diagnosed promptly to start an optimal treatment without delay

Islet cell transplantation.

People with type 1 diabetes have normal exocrine pancreatic function, making islet cell rather than whole organ transplantation an attractive option. Achieving insulin independence in type 1 diabetes was the perceived goal of islet cell transplantation. The success of the Edmonton group in achieving this in a selected group of type 1 patients has led to renewed optimism that this treatment could eventually replace whole organ pancreas transplantation. However the long-term results of this treatment indicate that insulin independence is lost with time in a significant proportion of patients, al

[Infection in heart transplantation].

Heart transplantation has emerged as the therapy of choice for selected patients with life-threatening, irremediable heart disease. Infectious complications are frequent, but are no longer the leading cause of death. Late infections (particularly viral infections), however, are still a threat to long-term graft survival in these patients. Bacterial infections are the most common, particularly of the respiratory tract, which usually appear in the first month or after one year following transplantation in patients with previous COPD. Catheter-related infection is also common, whereas postoperati

Renal transplantation in children.

OBJECTIVE: Renal transplantation is the preferred method for the treatment of children in end-stage renal disease (ESRD). In this retrospective study, we analyzed the results at our center. PATIENTS AND METHODS: Between November 1993 and June 2006, 86 children (50 boys and 36 girls) received organs from 50 living donors (LDTx) and 36 cadaveric donors (CDTx). Twenty children were <10 years. In addition to ESRD, some patients had one or more other high-risk factors, eg, abnormal lower urinary tract in 36 recipients (42%). The procedure was a preemptive transplantation in 28, and a retransplanta

Coccidioidomycosis in liver transplantation.

Coccidioidomycosis is an endemic fungal infection in the southwestern United States. It causes morbidity and mortality among solid organ transplant recipients who reside in or visit the endemic area or who receive organs from donors infected with the fungus. This paper reviews current literature addressing these infections in liver transplantation programs, including risk factors, clinical manifestations in persons with cirrhosis or who have had a liver transplantation, prophylaxis, treatment, and outcomes.

[Sequelae of organ transplantation].

Transplant medicine has significantly changed the prognosis of diseases leading to terminal organ failure. It has evolved from an experimental procedure to standard therapy for liver, kidney and cardio-vascular diseases. Transplant medicine combines operative organ replacement with the management of severely ill patients before transplantation, as well as life-long follow-up of organ graft recipients. Ten year survival rates of 65% to over 90% have led to a steady increase of transplanted patients seen by general medical care providers which represents a challenge for practicing internists. Ap

Small bowel transplantation.

Until very recently, outcomes from small bowel transplantation (SBTx) lagged behind those in liver, heart, and kidney transplantation because of the magnitude of the immunologic burden; the strong expression of histocompatibility antigens; and the contamination in grafts by bacterial organisms. With novel techniques of immune-induction therapies, such as recipient "preconditioning" with lymphocyte reduction, followed by the more subtle use of immunosuppression-based single-agent tacrolimus, graft and host 1-year survival is now over 90% in the most active US centers, a finding that parallels t

Novel therapies in transplantation.

Immunosuppressant drugs are crucial in order to protect transplanted kidney, heart or liver against the body's natural defence mechanisms. These drugs are used to prevent both acute rejection of the organ as well as chronic deterioration of the graft over longer periods of time. Currently used immunosuppressant drugs are calcineurin inhibitors (cyclosporin and tacrolimus), steroids (prednisolone and methylprednisolone), antimetabolites (azathioprine and mycophenolate mofetil), antiproliferatives (sirolimus) and monoclonal antibodies that are used as an induction therapy (basilximab, daclizumab
