Corynebacterium striatum Infection in Incision and Thoracic Cavity with Concurrent CRKP Colonization Following Lung and Bladder Tumor Resection.

Corynebacterium striatum, a commensal of skin and mucosa, is an emerging opportunistic pathogen. We report a rare case of postoperative incision and thoracic infection caused by C. striatum with concurrent pulmonary colonization by carbapenem-resistant Klebsiella pneumoniae (CRKP). Following resection of lung and bladder tumours, the patient developed fever, pleural effusion, and poor wound healing. Sputum cultures suggested CRKP colonization, while pleural fluid cultures identified C. striatum as the true pathogen. Clinical pharmacy consultation was pivotal in resolving this diagnostic dilemm

Macrohormones in endocrinology: prevalence, clinical impact, and diagnostic approaches.

Macrohormones also called macromolecules or macrocomplexes are described as an antigen-antibody complex, consisting of monomeric hormones bound to immunoglobulins. These molecules exhibit minimal or no biological activity. However, their presence may falsely indicate elevation of active hormones levels which can lead to significant diagnostic challenges. Currently, polyethylene glycol precipitation (PEG) is one of the most recognized methods for identifying macrohormones and it has been widely used in diagnosing macroprolactinemia as awareness of its clinical implications is well recognized. Ultrasonographic Features of the Kidney in Patients With Frasier Syndrome and Stage 5 Chronic Kidney Disease: A Case Report and Literature Review.

BACKGROUND: Frasier syndrome (FS) is a rare WT1-related disorder characterized by progressive nephropathy and diffuse renal parenchymal damage on imaging. Ultrasonographic findings of end-stage kidney disease in FS have rarely been documented. METHODS: We present the case of a 12-year-old boy who was diagnosed with FS, harbored a heterozygous c.1447+5G>A mutation in his WT1 gene, and developed CKD Stage 5. Renal ultrasonography and non-contrast CT were performed. A systematic literature review was conducted to contextualize the imaging findings. RESULTS: Non-contrast CT revealed multiple are

From Culture to Sequencing: Evolving Strategies for the Diagnosis of Pediatric Spondylodiscitis.

Pediatric spondylodiscitis is a rare but clinically significant infection affecting the intervertebral disc and adjacent vertebral bodies. Diagnostic delays are common due to its nonspecific presentation and the limited sensitivity of conventional microbiological methods. Early and accurate pathogen identification is essential to guide antimicrobial therapy, minimize unnecessary invasive procedures, and prevent long-term sequelae. Traditional diagnostic tools-including laboratory tests, imaging, blood cultures, biopsy, and histopathological evaluation-remain fundamental but are often insuffici

Dual paradoxical reactions in tuberculous lymphadenitis during and after pregnancy: A case report.

We report the case of a 23-year-old woman with tuberculous lymphadenitis diagnosed prior to pregnancy who developed paradoxical lymph node enlargement during pregnancy and again in the post-partum period. Despite completing a full 9-month course of standard anti-tuberculosis therapy, she experienced recurrent cervical lymphadenopathy and a transient pulmonary opacity following delivery. Extensive investigations, including repeat biopsies and cultures, excluded relapse or drug resistance, supporting the diagnosis of a paradoxical reaction. She was managed conservatively with drainage procedures

Invasive Ureaplasma urealyticum infection mimicking diffuse large B-cell lymphoma relapse: A case report.

Ureaplasma spp. are typically benign colonizers of the urogenital tract but may cause invasive disease in immunocompromised patients. In rare cases, such infections may mimic malignancy and lead to significant diagnostic delay. Diagnosis is challenging due to non-specific clinical manifestations, poor visibility on Gram stain, and limitations of standard culture methods. We report a rare case of invasive Ureaplasma urealyticum infection in a patient in complete remission after rituximab-based chemoimmunotherapy for diffuse large B-cell lymphoma transformed from follicular lymphoma. The infecti

Beyond the breast: Mammographic manifestations of systemic disease.

Mammography is traditionally regarded as a domain confined to the detection of breast malignancies; however, it can also serve as a mirror for diverse systemic, haematological, autoimmune, infectious, and metabolic diseases. Imaging manifestations may reveal distinctive pathognomonic features that directly suggest an underlying systemic disorder or provide subtle indirect clues that prompt timely multidisciplinary evaluation, thereby sparing patients from unnecessary procedures. In this comprehensive review, we present one of the most diverse compilations of extramammary and systemic disorders…

Spontaneous hemorrhage of cavernous malformation in the region of the cochlear nucleus: A case report.

Cerebral cavernous malformations (CMs) are low-flow, thin-walled vascular lesions affecting approximately 0.5%-0.9% of the population; they rarely arise at the skull base, and involvement of the cerebellopontine angle (CPA) is exceptionally uncommon, with only 19-21 cases reported and no prior description of cochlear nucleus involvement. We report a previously healthy 33-year-old woman who presented acutely with right-sided temporal headache, severe vertigo, sensorineural hearing loss, otalgia, and facial weakness. Multisequential MRI, including susceptibility-weighted imaging (SWI), demonstra

Tonsillar Actinomycosis Associated With Cervical Lymphadenopathy: A Rare Benign Condition Mimicking Malignancy.

Tonsillar actinomycosis is an uncommon chronic infection that can clinically and radiologically mimic malignancy. Because of its nonspecific presentation and tumor-like appearance, particularly when associated with cervical lymphadenopathy, it often poses a diagnostic challenge. A 20-year-old patient presented with a four-month history of progressive left tonsillar hypertrophy. Nasofibroscopy showed an irregular tonsillar surface with nasopharyngeal bulging, raising suspicion of a neoplastic lesion. Contrast-enhanced CT revealed asymmetric left tonsillar enlargement with homogeneous thickening

Tumefactive Multiple Sclerosis Mimicking an Intracranial Mass in a 34-Year-Old Man: A Case Report.

Tumefactive multiple sclerosis (TMS) is a rare demyelinating variant of multiple sclerosis that can closely mimic intracranial malignancy or abscess on imaging. This case highlights diagnostic complexity when focal neurological deficits occur alongside a concurrent dermatomal rash. A 34-year-old man with no prior neurological history presented with three days of progressive left-sided weakness and numbness, preceded by ipsilateral neck pain and a pruritic vesicular rash, initially treated as herpes zoster. Brain imaging revealed a 3.5 cm right frontal ring-enhancing lesion with surrounding vas

Combined Hamartoma of the Retina and Retinal Pigment Epithelium: A Literature Review and Case Series.

We present a case series of two young patients referred to our Ophthalmology Department with a progressive decrease in visual acuity of their left eye, where multimodal imaging findings led us to the diagnosis of combined hamartoma of the retina and retinal pigment epithelium (CHR-RPE). Based on a literature review, we discuss the different multimodal imaging strategies, differential diagnosis, and management of CHR-RPE, a rare benign congenital lesion that can mimic malignant intraocular tumors. CHR-RPE has a challenging differential diagnosis with entities such as choroidal melanoma, nevus,

Fractional Flow Reserve from Coronary CT: Evidence, Applications, and Future Directions.

Coronary computed tomography angiography (CCTA) has emerged as the leading noninvasive imaging modality for the assessment of coronary artery disease (CAD), offering high-resolution visualization of the coronary anatomy and plaque characterization. The development of fractional flow reserve derived from CCTA (FFR-CT) has further transformed the diagnostic landscape by enabling the simultaneous evaluation of both anatomical stenosis and lesion-specific ischemia. FFR-CT has demonstrated diagnostic accuracy comparable to invasive FFR. The combined use of CCTA and FFR-CT is now pivotal in a broad

Clinical spectrum of cervical involvement in calcium pyrophosphate deposition disease.

Calcium pyrophosphate deposition (CPPD) disease is a common microcrystalline arthropathy in the elderly, The clinical spectrum includes both acute and chronic inflammatory arthritis, but crystals depositions may also occur without symptoms, with chondrocalcinosis identified incidentally on imaging. Axial involvement is less frequent than peripheral but has been increasingly recognized, particularly in the cervical spine. Its manifestations are heterogeneous and may mimic infectious, inflammatory, neoplastic or degenerative disorders, often leading to misdiagnosis. We report four cases of cervi
