Variance of vestibular-evoked myogenic potentials.

OBJECTIVES/HYPOTHESIS: Vestibular-evoked myogenic potential (VEMP) has been thought to originate from sacculus. The variance of this potential and the effectiveness of the adjustments of pInII amplitudes using average muscle tonus of ipsilateral sternocleidomastoid muscle were evaluated. In addition, clinical application of VEMP was examined in patients with acoustic tumors (ATs) and vestibular neurolabyrinthitis (VNL). STUDY DESIGN: Prospective evaluation of the VEMP in 18 normal volunteers and 6 patients. METHODS: Variance and left-right difference of each parameter, including pI latency,

[Vestibular lesion in sudden deafness: a prognostic criterium for therapeutic success].

The influence of vestibular dysfunction on severity of initial hearing loss and success of therapy was evaluated in a retrospective study with 142 patients suffering from idiopathic sudden hearing loss. In 48% of these patients combined cochleovestibular disorders were found. In this group mean hearing loss was significantly higher (45 dB versus 32 dB) and hearing recovery after therapy (25% versus 38%) less pronounced. 83% of patients with high grades of vestibular disorders (Vestibular Index: > or = 9) showed a decrease of hearing function between 50 dB and 130 dB compared to 32% in the grou

Neurootological contributions to the diagnostic follow-up after whiplash injuries.

Only in 1992 we had to deal with about 197,731 cases of whiplash injuries due to traffic accidents on the roads of Germany. About 80% recover within a few months. However, about 15%-20% develop the so-called late whiplash injury syndrome with many complaints of the cervico-encaphalic syndrome including headache, vertigo, instability, nausea, tinnitus, hearing loss etc. The orthopedic as well as the radiological findings are mostly unsatisfactory with respect to the neurosensorial complaints. Therefore problems of evidence arise in compensation claims calling for additional neurootological medi

Follow-up of square drawing test in vestibular neuronitis.

The square drawing test (SDT) devised by Sekitani (1975), which is a modification of Fukuda's vertical writing test, was performed to evaluate vestibular dysfunction in 15 patients with vestibular neuronitis followed up for a long period. In particular, two parameters (distance and total length) were analyzed in the present study. Distance was analyzed by dividing the 15 cases into 3 groups. The largest group showed a tendency to shorten at an early stage. Total length was examined by dividing the 15 cases into 3 groups. The largest group showed a tendency towards macrographism at an early sta

Effects of adding transcutaneous vagus nerve stimulation to vestibular rehabilitation on dizziness and postural control in vestibular migraine: a randomized clinical trial.

Vestibular migraine (VM) causes episodic dizziness and imbalance, yet treatment options remain limited. Although management often follows migraine protocols, vestibular and headache symptoms may not coincide. While vestibular rehabilitation (VR) is a noninvasive approach to improve balance, transcutaneous auricular vagus nerve stimulation (taVNS) has shown promise in migraine relief and may influence vestibular function. This parallel-group randomized clinical trial investigated the combined effects of VR and taVNS on postural control and migraine-related symptoms in VM patients. Twenty-eight

Is Vestibular Neuropathy Rather a Neuritis?

Background/objectives Vestibular neuritis (VN) is one of the most common causes of peripheral vestibular neuropathy. The goal of this study is to investigate the possible infectious causes of VN in a large cohort of patients. Material and methods In total, 98 consecutive VN patients were enrolled in this retrospective study over a four-year period (04/2015-04/2019). Diagnosis of VN was made by clinical examination and functional diagnostics. We focused on infectious causes such as neurotropic viruses and Lyme disease (LD) and evaluated infection parameters as well as the concomitant diseases.

Impact of artifacts on VOR gain measures by video-oculography in the acute vestibular syndrome.

OBJECTIVE: The video head impulse test (HIT) measures vestibular function (vestibulo-ocular reflex [VOR] gain - ratio of eye to head movement), and, in principle, could be used to make a distinction between central and peripheral causes of vertigo. However, VOG recordings contain artifacts, so using unfiltered device data might bias the final diagnosis, limiting application in frontline healthcare settings such as the emergency department (ED). We sought to assess whether unfiltered data (containing artifacts) from a video-oculography (VOG) device have an impact on VOR gain measures in acute v

Accelerated Cognitive Decline Associated With Hearing Loss and Bilateral Vestibulopathy: Insights From a Prospective Cross-Sectional Study Using the Repeatable Battery for the Assessment of Neuropsychological Status Adjusted for the Hearing Impaired in the DFNA9 Population.

BACKGROUND: DeaFNess Autosomal dominant 9 (DFNA9) is a hereditary disorder known to affect both hearing and vestibular function in its carriers. Its phenotype is characterized by progressive sensorineural hearing loss (SNHL) and vestibular dysfunction evolving towards bilateral vestibulopathy (BV) by the 3rd to 5th life decade. Recent studies have identified the impact of hearing loss and vestibular dysfunction on cognitive functioning. OBJECTIVE: The main objective of this study was to investigate how the cognitive functioning of carriers of the p.Pro51Ser variant in the COCH gene is affecte

A sustained hyper-g load as a tool to simulate space sickness.

In April 1989 the three European scientist astronauts of the D1 Spacelab Mission were exposed to a 1.5 hours +3G centrifuge run in supine position, resulting in a linear acceleration along the subjects' x-axis. Afterwards, severe motion sickness symptoms were provoked by head movements (Sickness Induced by Centrifugation: SIC). The astronauts mentioned close similarities with what they experienced in space during the D1-Spacelab Mission in 1985, where head movements also provoked motion sickness symptoms (Space Adaptation Syndrome: SAS). Moreover, the astronauts agreed that the rank order of t

Baclofen affects the semicircular canals but not the otoliths in humans.

CONCLUSION: This study showed that GABAB agonist baclofen (10 mg) affects the semicircular canals (SCCs), both centrally and peripherally, but does not influence the otolithic function. OBJECTIVES: The aim of the study was to identify the effects of baclofen on the complete vestibular system, i.e. semicircular canals, saccules and utricles. METHODS: The study had a double-blind, placebo-controlled, repeated measures design and was conducted on healthy male volunteers. With electronystagmography (ENG), the SCC function was evaluated, whereas utricular function was determined by means of unila

Protocol for assessing functional impairments in patients with unilateral and bilateral vestibulopathy: a novel approach to evaluate the impact of vestibular loss in daily life setting.

BACKGROUND: Current vestibular assessments typically focus on isolated reflex pathways, failing to reflect the integrative nature of balance control. Consequently, clinical results often do not align with patient-reported symptoms or functional limitations in daily life. OBJECTIVE: To develop and present a comprehensive multimodal protocol for assessing functional impairments in patients with unilateral vestibulopathy (UV) and bilateral vestibulopathy (BV) using wearable sensors and ecologically valid daily-life tasks. METHODS: We designed a protocol combining nine inertial measurement units

Ocular vestibular evoked myogenic potential to air conducted sound stimulation and video head impulse test in acute vestibular neuritis.

BACKGROUND: Air-conducted (ACS) cervical VEMP (cVEMP) reflect both saccular and inferior vestibular nerve function. Ocular VEMP (oVEMP) to air and bone-conducted vibration (BCV) are critically discussed, whether they reflect predominantly utricular and superior vestibular nerve function. The video head impulse test (vHIT) accurately detects changes in the high frequency range (5-7 Hz) of the vestibular ocular reflex (VOR) in all 3 planes and can be used to assess semicircular canals (SCC) impairment. OBJECTIVES: To evaluate oVEMP and cVEMP in response to 500 Hz ACS stimulation and to compare
