[Phoneme discrimination and dyslexia in school children at grade one to six].

OBJECTIVE: The research on phoneme discrimination in connection with dyslexia has focused mostly on reading ability. The aim of this study was to analyse phoneme discrimination in relation to spelling ability in the context of different school grades. METHOD: The data of 253 children in school grades 1 to 6 who were seen for diagnosis of dyslexia and auditory processing disorder were analyzed retrospectively. Phoneme discrimination was assessed via the Heidelberger Phoneme Discrimination Test at 3 levels: auditory discrimination, repetition of minimal pairs, and analyzing the initial consonan

Midlife use of written Japanese and protection from late life dementia.

BACKGROUND: The cognitive reserve hypothesis would predict that use of written Japanese should confer protection against dementia because of the complexity of its ideograms compared with written English. We sought to test this hypothesis in analyses from a longitudinal study of Japanese-American men. METHODS: Participants were second-generation Japanese-American men (Nisei) on the island of Oahu, Hawaii, who were seen in 1965 and in subsequent examinations to detect dementia beginning in 1991-1993. Use of spoken and written Japanese was self-reported in 1965 (Analyses 1 and 2), and midlife us

External shock waves therapy in dystonia: preliminary results.

BACKGROUND AND PURPOSE: Extracorporeal shock wave therapy (ESWT) has been shown to reduce hypertonia in patients with upper motor neuron syndrome without any side effect. The aim of the present study is to investigate whether ESWT could be useful also in patients with dystonia. METHODS: We evaluated three patients with secondary dystonia and three patients with idiopathic writer's cramp. Placebo treatment was performed in each patient. ESWT was administered during four sessions (once weekly) to the target muscles of hand and forearm using an electromagnetic lithotripter (Modulith SLK--Storz M

Kinesthetic alexia due to left parietal lobe lesions.

To investigate the neuropsychological mechanisms of kinesthetic alexia, we asked 7 patients who showed kinesthetic alexia with preserved visual reading after damage to the left parietal region to perform tasks consisting of kinesthetic written reproduction (writing down the same letter as the kinesthetic stimulus), kinesthetic reading aloud, visual written reproduction (copying letters), and visual reading aloud of hiragana (Japanese phonograms). We compared the performance in these tasks and the lesion sites in each patient. The results suggested that deficits in any one of the following func…

Development of phonological and orthographic skill: a 2-year longitudinal study of dyslexic children.

Twenty-one dyslexic children, ages 9-15, were administered a battery of tests on two occasions separated by 2 years to assess the development of word recognition and spelling. The majority of the subjects were receiving intensive small-group instruction and one-on-one tutoring in reading and writing. Correlational and regression analyses supported the assumption that phonological and orthographic processing are distinct but reciprocally related components of word recognition and spelling. However, phonological skill appeared to capture most of the unique variance in word identification for dys…

Influence of written form on reading comprehension in aphasia.

Reading comprehension of 20 aphasic patients was systematically assessed using equivalent stimuli written in both print and cursive form. A Single-Word-to-Picture Matching task and a Sentence-to-Picture Matching task were employed. Performances of the aphasics were not uniformly facilitated by one written form over the other. Furthermore, the reading comprehension of the majority of individual patients was not influenced by written form. Four of the subjects, however, did appear to display real performance discrepancies when reading cursive versus print, particularly on the Sentence-to-Picture

A Loss for Words: A Case of Aphemia.

Aphemia, also known as pure motor mutism, is a rare disorder characterized by the loss of motor function to produce speech while retaining the ability to understand, read, and write language. Differentiating between aphemia and aphasia can be challenging in many cases, as they both present with similar physical exam findings. The key distinctions lie in the location of the insult in the cerebral parenchyma, the ability to understand language, and the associated dysphagia. We present the case of a 78-year-old male who presented with new-onset aphasia and was found to have aphemia based on physi

Necrotizing soft-tissue infections in obstetric and gynecologic patients.

For the clinician, necrotizing soft-tissue infections have remained a daunting opponent since the first writings on the subject over 2000 years ago. Early disease may be incorrectly diagnosed as cellulitis, and this delay in correctly diagnosing and expeditiously proceeding to radical surgical debridement may lead to a high degree of mortality. Although several inciting events and risk factors have been described that allow for the development and progression of this disease, the diagnosis is still made clinically. Only aggressive surgical management in combination with broad-spectrum antibiot…

Historical figure: dr joseph babinski(1857 to 1932).

Each time a patient is examined somewhere, one looks for Babinskis sign. It is recorded in medical files all over the world, but because the history of medicine is only minimally taught, Babinski remains virtually unknown. At times he is thought to be Russian, some write his name with a Y; the reflex he discovered is sometimes abbreviated as Bab or, less frequently, as BBK. The essence of Babinskis success was in creating an original approach to the evaluation of the patient, based on a meticulous clinical exam, looking for those abnormal reflexes or signs which he described, often in the face

Medicine and the Egyptian campaign: the development of the military medical officer during the Napoleonic Wars c. 1798-1801.

This article investigates how French and British army medical officers in Egypt at the turn of the 19th century were affected by campaign experiences. Their encounters with ophthalmia, plague, and other diseases influenced the practice of medicine in later campaigns and fostered the development of the idea amongst military practitioners that military diseases required specialised knowledge. Practitioners' campaign writings are used to demonstrate how British army doctors approached the investigation of the "new" diseases they encountered. In particular, the article focuses on how Dr. James McG…

Three-flap cover for total hand degloving.

Re-surfacing after a total hand degloving injury is one of the most difficult management problems in hand surgery. We present one such case that was managed using three flaps for cover. The radial forearm flap is very thin and pliable, resulting in satisfactory coverage of first web space and the thumb remnant, and facilitates a mobile thumb. The groin-hypogastric flap covered the remnant fingers admirably. The patient had a good first web and could use the hand to lift objects and hold a pen to write legibly within 2 months of the injury.

Does dyslexia develop from learning the alphabet in the wrong hemisphere? A cognitive neuroscience analysis.

A new perspective is described which views developmental dyslexia as the outcome of learning to write the alphabet in the nondominant (right) hemisphere. The letter-level and whole-word subtypes of dyslexia are seen as differing responses adopted to cope with this predicament. Striking similarities between dyslexics and callosotomy patients in the allocation of covert attention to lateralized stimuli provide direction for integrating a diversity of dyslexic research within this framework. This synthesis, together with information from pure alexia, brain activation, and reading research, lends

Developmental dyslexia: the cerebellar deficit hypothesis.

Surprisingly, the problems faced by many dyslexic children are by no means confined to reading and spelling. There appears to be a general impairment in the ability to perform skills automatically, an ability thought to be dependent upon the cerebellum. Specific behavioural and neuroimaging tests reviewed here indicate that dyslexia is indeed associated with cerebellar impairment in about 80% of cases. We propose that disorders of cerebellar development can in fact cause the impairments in reading and writing characteristic of dyslexia, a view consistent with the recently appreciated role of t…

Treatment of biliary colic by water injection in the region of Qimen, Riyue, and Juque points.

Water injection was used in the treatment of 56 cases of biliary colic. The method was based on the writings of Zhang Zhongjing about the role of the Qimen point in treating diseases and clinical practice in this respect, water injection being performed at the most sensitive spot in the region defined by the lines joining the Qimen (Liv 14), Riyue (GB 24), and Juque (Ren 14) points. Pain disappeared in 32 (57.1%) of the 56 cases treated, and the total effective rate was 96.4%, showing a significantly better result compared with that of the control group treated with Western medicine (P < 0.05)…

[Pathogenesis of obsessional states against a psychopathic background].

The paper is concerned with a clinical study of 37 psychopaths with obsessional symptoms: 24 anancasts and 13 psychasthenics. In 14 patients the obsessional ideas were combined with obsessional actions, in 28 cases there was a defect of integrative automatized motions, in 6 cases--tics, in 2--writing spasms and in 1 case--a spastic torticollis without symptoms of an organic lesion of the nervous system. A hyperkinesis of the tic-type, professional dyskinesia and possibly obsessional actions may be explained by a functional dissociation of the neocrotex with subcortex and limbico-reticular comp…
