Impact of adenotonsillectomy on behavior in children with sleep-disordered breathing.

OBJECTIVES/HYPOTHESIS: Children with sleep-disordered breathing may experience behavioral and learning problems such as inattentiveness and hyperactivity. The aim of this study was to measure the impact of adenotonsillectomy on sleep-related adverse events and behavioral problems in children with sleep-disordered breathing. METHOD: This prospective and interventional study enrolled 40 sleep-disordered breathing children (mean age, 8.4+/-1.6 years) with hypertrophic tonsils and adenoids. All patients completed two polysomnographies, tests of variables of attention (TOVAs), and Child Behavior C

Perioperative use of the Thrombelastograph in patients with inherited bleeding disorders.

There are few reports of the use of Thrombelastgraph (TEG) in patients with isolated or inherited bleeding disorders. Three surgical patients are presented with von Willebrand's disease, factor XII deficiency, and hemophilia A with dysfibrinogenemia in which the abnormalities detected by the TEG were similar to the expected coagulation abnormalities based on preoperative laboratory studies. This initial report suggests that this potential application of the TEG be further investigated.

Nasopharyngeal tonsil's provision of the surface secretions with immunocytes, a property additional to antigen processing.

As we recently found that IgA, IgM, and IgG are produced and secreted by immunocytes present in nasopharyngeal secretions, we tested the hypothesis that B- and T-lymphocytes in the surface secretions are derived from the nasopharyngeal tonsil in an active process. By immunohistochemistry, we found that numerous B- and T-lymphocytes were often accumulated in restricted areas in the epithelium, and some of these cells were demonstrated just beneath the epithelial surface or could be observed protruding into the lumen. A portion of these cells were Ki-67+, indicating clonal expansion and/or immun

[Sleep apneas in children].

There are many causes leading to breathing disorders in children. In the newborn period the immature central regulation of breathing can result in a pattern with apneas and bradycardias most commonly seen in the very premature infant. Therefore, during hospital stay many of these very tiny preterms and some of the very ill term infants do have severe apneas and do need medication and or mechanical support (nasal CPAP, positive pressure ventilation). In the first two to three months of life central dysmaturity can persist in some infants and apneas of infancy can occur further on. Infants with

[Variations in the course of the internal carotid artery: possible risks in so-called standard operations in the area of the pharynx].

BACKGROUND: Many abnormalities in the course of the major vessels of the head and neck have been described. Special care must be taken in case of deviation of the internal carotid artery bulging the pharyngeal wall. PATIENTS: In two cases we demonstrate internal carotid arteries partially running underneath the posterior pharyngeal wall. RESULTS: Those variations are often asymptomatic, only requiring surgical treatment when causing neurologic complaints. On the other hand, they may be very dangerous, if operations in the upper pharynx are carried out. CONCLUSIONS: The surgeon (in special t

Long-term health outcomes of pediatric obstructive sleep apnea: A 10-year multicenter cohort study.

OBJECTIVES: To evaluate the long-term associations between pediatric obstructive sleep apnea (OSA) and multiple domains of child health and to examine potential differences according to surgical treatment. METHODS: We conducted a retrospective cohort to assess children aged 2-18 years with newly diagnosed OSA confirmed by polysomnography (PSG) who were followed for up to 10 years. Three cohorts were constructed: non-surgically treated OSA, surgically treated OSA who underwent adenotonsillectomy, and age-matched healthy controls. Outcomes included growth failure, delayed developmental mileston

Clinical significance of mouth breathing as a marker of severity in pediatric obstructive sleep apnea.

BACKGROUND: To investigate whether clinically assessed mouth-breathing severity is independently associated with polysomnography–defined obstructive sleep apnea severity in children with adenoidal hypertrophy and/or tonsillar hypertrophy, and to evaluate its potential as a non-invasive predictor of moderate-to-severe obstructive sleep apnea. METHODS: We retrospectively analyzed pediatric inpatients admitted for planned adenotonsillectomy at Shanghai Sixth People’s Hospital. Based on a mouth-breathing score (range 1–7), patients were categorized into mild (scores 1–3) or severe (scores 4–7) mou

Management of Sleep-Disordered Breathing in Children With Craniofacial Abnormalities and Skeletal Dysplasia.

INTRODUCTION: Anatomic malformations associated with craniofacial abnormalities (CFAs) and skeletal dysplasias (SDs) contribute to airway obstruction during sleep in children, which may be treated surgically, with positive airway pressure (PAP) therapy, or with dental appliances. This study examines the management of OSA in children with CFAs and/or SDs, including the prevalence of residual OSA postsurgery and PAP therapy compliance. METHODS: This was a retrospective cohort study of children with CFAs and/or SD. Children were included if they underwent polysomnography (PSG) at the Queensland

Long-term Impact of Middle Ear Effusion in Pediatric Tympanostomy Tubes.

OBJECTIVES/HYPOTHESIS: Bilateral myringotomy and tympanostomy tube placement (BMT) is the most common pediatric surgery in the United States. Intraoperative middle ear effusion (MEE) is a risk factor for future BMTs in children with recurrent acute otitis media (RAOM). However, the impact of the type of MEE is unknown. Here, we assess otologic outcomes based on intraoperative MEE type and indication for surgery. STUDY DESIGN: Case series chart review. METHODS: After institutional review board approval, we performed a review of children undergoing BMTs between 2008 and 2009. Included patients

Dynamic tonsillar prolapse masquerading as paradoxical vocal fold movement dysfunction.

INTRODUCTION: Paradoxical vocal fold movement dysfunction (PVFMD) is a disorder in which the vocal folds involuntarily adduct during inspiration resulting in stridor, cough, dysphonia and dyspnea. Diagnosis of PVFMD is difficult given the episodic nature of the disorder and the often-normal laryngeal exam in between episodes. Moreover, additional sources of obstruction have been identified as sources of Periodic Occurrence of Laryngeal Obstruction (POLO). Treatments can vary with site of obstruction. OBJECTIVE: To evaluate pediatric patients presenting to a Vocal Fold Dysfunction Center for e

Surgical treatment for childhood obstructive sleep apnoea: Cold-knife tonsillar dissection versus bipolar radiofrequency thermal ablation.

INTRODUCTION AND OBJECTIVE: Adenotonsillectomy for treatment of childhood obstructive sleep apnoea is effective. The uncomfortable postoperative period and possible complications have significantly increased the use of partial techniques, seeking to improve these aspects while achieving the same results in resolving sleep apnoea. The aim was to present the experience with 2 consecutive groups of patients, comparing total tonsillectomy to bipolar radiofrequency ablation (RFA). METHOD: A group of 96 children that underwent total tonsilloadenoidectomy using cold dissection were compared to anoth…

Association of adenotonsillectomy with asthma outcomes in children: a longitudinal database analysis.

BACKGROUND: Childhood asthma and obstructive sleep apnea (OSA), both disorders of airway inflammation, were associated in recent observational studies. Although childhood OSA is effectively treated by adenotonsillectomy (AT), it remains unclear whether AT also improves childhood asthma. We hypothesized that AT, the first line of therapy for childhood OSA, would be associated with improved asthma outcomes and would reduce the usage of asthma therapies in children. METHODS AND FINDINGS: Using the 2003-2010 MarketScan database, we identified 13,506 children with asthma in the United States who u

Efficacy of sucralfate for the treatment of post-tonsillectomy symptoms.

Pain after tonsillectomy remains the main issue of postoperative morbidity and the search for an effective post-tonsillectomy analgesic is of increasing relevance. The aim of this study was to evaluate the effectiveness of topical sucralfate during an early post-tonsillectomy period. Fifty patients of both genders from 6 to 58 years submitted to tonsillectomy or adenotonsillectomy were randomly included into either a sucralfate treatment group (N = 25) or into a control group (N = 25). Patients of the sucralfate group received topical sucralfate four times a day for 7 days. No topical treatmen
