"Different is nice, but it sure isn't easy": differentiating the spectrum of autism from the spectrum of normalcy.

Brian is a 15-year-old boy who has been just changed to your practice because of a change in insurance plans. When taking the social history, his parents note that he has "the hardest time relating to other kids." Sometimes he will be in the middle of a conversation with a friend and then discuss tangential topics. His father reports that Brian "...has always been an easy target. He has always been sort of bigger than other kids, but being bigger than other kids, he has a hard time standing up for himself." He seems to expect to be picked on in any new social situation. When Brian likes someth

Systemic Everolimus Does Not Block the Incubation of Methamphetamine-Craving: A Dose-Response Study.

In rodent models, incubated craving for drugs and natural reinforcers is characterized by a time-dependent increase in cue-elicited operant responding, which posits that common biochemical mechanisms gate incubated craving across different types of reinforcers. Recently, our laboratory identified heightened mTOR activity in the prelimbic (PL) and infralimbic (IL) subregions of the medial prefrontal cortex (mPFC) as being associated with incubated craving for cocaine and sucrose, respectively. Importantly, Everolimus, an FDA-approved mTOR inhibitor, blocked the expression of incubated craving f

Methamphetamine use and HIV prevention: a qualitative study on challenges and opportunities for PrEP utilization.

Methamphetamine use often co-occurs with sexual behaviors that may increase HIV acquisition risk and disrupt adherence to HIV pre-exposure prophylaxis (PrEP). To explore barriers and opportunities for PrEP among people who use methamphetamine, we conducted semi-structured interviews with 28 self-reported HIV-negative adults (ages 18-55) with current moderate-to-high risk methamphetamine use in the New York City metropolitan area. Transcripts were thematically analyzed with double coding and iterative consensus. Participants described sexualized methamphetamine use, polysubstance use, and sexua

Substance use patterns and unprotected sex among street-involved youth in a Canadian setting: a prospective cohort study.

BACKGROUND: Rates of sexually transmitted infections (STI) and unplanned pregnancy are high among youth. While the intersection between drug and alcohol use and unprotected sex is well recognized, few studies have examined the relationship between substance use patterns and unprotected sex among high risk-populations such as street-involved youth. METHODS: Data were derived from the At-Risk Youth Study (ARYS), a prospective cohort of street-involved youth from Vancouver, Canada. Generalized estimating equations (GEE) were used to examine substance use patterns that were independently associat

Effect of the Novel Positive Allosteric Modulator of Metabotropic Glutamate Receptor 2 AZD8529 on Incubation of Methamphetamine Craving After Prolonged Voluntary Abstinence in a Rat Model.

BACKGROUND: Cue-induced methamphetamine craving increases after prolonged forced (experimenter-imposed) abstinence from the drug (incubation of methamphetamine craving). Here, we determined whether this incubation phenomenon would occur under conditions that promote voluntary (self-imposed) abstinence. We also determined the effect of the novel metabotropic glutamate receptor 2 positive allosteric modulator, AZD8529, on incubation of methamphetamine craving after forced or voluntary abstinence. METHODS: We trained rats to self-administer palatable food (6 sessions) and then to self-administer

HIV sex risk behaviors among heterosexual methamphetamine users: literature review from 2000 to present.

Methamphetamine (MA) use is a burgeoning problem worldwide and throughout the United States. Researchers have begun to examine risk behaviors among subgroups of MA users in an attempt to discover strategies to assist MA users in reducing their drug use and preventing transmission of diseases such as HIV and Hepatitis C. Sex risk behaviors have been traditionally difficult to examine and to change through intervention. This is important in light of the fact that MA users report a higher sex drive while on MA. This brief literature review examines recent studies of interest looking at heterosexu

Evaluation of subjective effects of aripiprazole and methamphetamine in methamphetamine-dependent volunteers.

A variety of neuropharmacological strategies are being pursued in the search for an effective treatment for methamphetamine (Meth) addiction. In this study, we investigated the safety and potential efficacy of aripiprazole, an antipsychotic agent acting on both dopamine and serotonin systems. We conducted a double-blind in-patient clinical pharmacology study to assess potential interactions between intravenous (i.v.) Meth (15 mg and 30 mg) and oral aripiprazole (15 mg). In addition, the effects of aripiprazole treatment on abstinence-related craving and cue-induced craving were evaluated. Part

Neurogenic bladder and chronic urinary retention associated with MDMA abuse.

INTRODUCTION: The use of 3,4-methylenedioxymethamphetamine (MDMA, known as "ecstasy"), a synthetic amphetamine and "club drug," has been associated with acute, transient urinary retention. We report a case of neurogenic bladder and chronic urinary retention associated with MDMA abuse. CASE REPORT: A 21-year-old male presented to the emergency department (ED) because he had abdominal pain and difficulty urinating. He had experienced difficulty in initiating urination over the past 1.5 months, with periods of 24 to 36 hours between voids and large volumes of urine. The patient had a chronic pat

"Speed" warps time: methamphetamine's interactive roles in drug abuse, habit formation, and the biological clocks of circadian and interval timing.

The indirect dopamine (DA) agonist methamphetamine (MAP) is evaluated in terms of its impact on the speed of temporal processing across multiple time scales involving both interval and circadian timing. Behavioral and neuropharmacological aspects of drug abuse, habit formation, neurotoxicity, and the potential links between interval and circadian timing are reviewed. The view that emerges is one in which the full spectrum of MAP-induced effects on timing and time perception is both complex and dynamic in as much as it involves DA-glutamate interactions and gene expression within cortico-striat

A review of the effects of repeated administration of selected phenylethylamines.

Several phenylethylamines are under consideration for international control. The effects of repeated administration of these compounds, including tolerance, physical dependence and central nervous system (CNS) toxicity, are reviewed here. The compounds can be divided into two major chemical groups: those with substituents on the ethylamine portion of the molecule and those with substituents on the phenyl ring. Although the effects of repeated administration have not been directly determined for most of the compounds, certain representative compounds of each chemical group have been examined in

Non-genetic risk indicators for preterm birth: An umbrella review of systematic reviews and meta-analyses.

BACKGROUND: This umbrella review, aimed to summarize systematic evidence on non-genetic risk indicators that are present at the start of pregnancy and their potential association with preterm birth. METHODS: We searched Medline and Embase up to June 30, 2021, for systematic reviews and meta-analyses of observational studies. We included systematic reviews and meta-analyses on observational studies evaluating putative risk indicators that are present at conception. Studies that assessed indicators that arise after conception, and were unlikely to be present before pregnancy were excluded. Pret

Use of methylphenidate and reporting of valvular heart disease: Global pharmacovigilance analysis in children and adults.

INTRODUCTION: Methylphenidate (MPH) is a common treatment of attention-deficit/hyperactivity disorder (ADHD). Concern has been raised regarding its cardiovascular safety, partly in relation with its micromolar affinity for the 5-HT2B receptor, whose activation may result in valvular heart disease (VHD). METHODS: To explore the association between the use of MPH and VHD reporting, we performed a disproportionality analysis within the WHO global safety database (VigiBase) using data, since inception until March 6th 2024, from: (i) the full database and (ii) different age groups (children/adoles

Reviewing the Potential Therapeutic Approaches Targeting the Modulation of Gastrointestinal Microflora in Schizophrenia.

Schizophrenia (SCZ) is a severe brain disorder characterized by an intriguing clinical panel that has begun to gain interest due to its particular phenotype. Having considered the role of gut microflora in psychiatry, the latest discoveries might offer further insight into the underlying mechanisms. Thus, we aimed to offer an updated overview of the therapeutic potential of microorganism-derived supplements alongside dedicated protocols that target the re-establishment of the host's eubiosis. Based on combinations of specific keywords, we performed searches in four databases (PubMed/Medline, I

A Global Survey on Changes in the Supply, Price, and Use of Illicit Drugs and Alcohol, and Related Complications During the 2020 COVID-19 Pandemic.

Background and Aims: COVID-19 has infected more than 77 million people worldwide and impacted the lives of many more, with a particularly devastating impact on vulnerable populations, including people with substance use disorders (SUDs). Quarantines, travel bans, regulatory changes, social distancing, and "lockdown" measures have affected drug and alcohol supply chains and subsequently their availability, price, and use patterns, with possible downstream effects on presentations of SUDs and demand for treatment. Given the lack of multicentric epidemiologic studies, we conducted a rapid global

Trends in stimulant dispensing by age, sex, state of residence, and prescriber specialty - United States, 2014-2019.

BACKGROUND: Stimulant medications are commonly prescribed for the treatment of attention-deficit/hyperactivity disorder; however, they also have high potential for diversion and misuse. We estimated national stimulant dispensing trends from 2014 to 2019 and differences in dispensing by age, sex, state, prescriber specialty, payor type, patient copay, and stimulant type. METHODS: We calculated rates of stimulant dispensing using IQVIA National Prescription Audit (NPA) New to Brand, NPA Regional, and NPA Extended Insights data, which provide dispensing estimates from approximately 49,900 pharma

Analytically Confirmed Intoxication by 4-Fluoromethylphenidate, an Analog of Methylphenidate.

4-Fluoromethylphenidate (4F-MPH) is an halogenated derivative of methylphenidate (MPH), a re-uptake inhibitor for dopamine and norepinephrine used for the treatment of attention deficit hyperactivity disorders. In the last few years, several compounds structurally related to MPH have been marked as new psychoactive substances (NPS) with stimulating and euphoric effects similar to the parent drug, but with more dopaminergic activity. This report represents the first case of an analytically confirmed non-fatal intoxication by 4F-MPH. A 26-year-old female was admitted to the emergency department
