Screening for mitral valve prolapse: an analysis of benefits and costs in the U.S. Air Force.

Using the techniques of decision analysis, several possible mitral valve prolapse screening strategies for U.S. Air Force aircrew candidates are evaluated. Available clinical and epidemiological data are organized so that the relative merits of various strategies can be assessed by decision makers. The most efficient strategy is to screen pilot candidates with Quetelet's index (weight in kilograms/height in meters squared) and then eliminate those with an index at or below 15 who have an echocardiogram positive for mitral valve prolapse. The next most efficient strategy is to do echocardiogram

Erythrocyte agglutination in microgravity.

This experiment tested a self-contained blood grouping device under microgravity as well as the possibility that gravity could affect the blood grouping reaction. The experiment tested Rh, ABO, and Coombs-sensitized human erythrocytes against their appropriate antisera and diluents. A self-contained device was built which accomplished the agglutination and collected the agglutinates on filter paper. This was flown on the Columbia Space Shuttle on Jan. 12, 1986. The device produced blood agglutinates to several blood groups and the agglutinates were held on filter paper until returned to us by

Assessing applicants to the NASA flight program for their renal stone-forming potential.

Spaceflight could provoke formation of kidney stones, in part by causing hypercalciuria and hyperphosphaturia. Applicants for spaceflight who have metabolic or environmental derangements to begin with might be particularly susceptible to stone formation in space. We, therefore, analyzed 24-h urine samples for stone-forming risk factors in 104 male applicants before their selection into the astronaut-mission specialist corps. The urinary environment was abnormally supersaturated with calcium oxalate in 25.0% of applicants, brushite in 36.5%, and monosodium urate in 66.3%, predisposing these app

Hyperventilation response to cold water immersion: reduction by staged entry.

Staged immersion of humans into cold water was investigated to determine whether the hyperventilation response could be reduced by this behavioral technique. A simple, two-stage procedure involving immersion to the waist for 30 s before full immersion to neck level was compared to non-staged immersion. For the staged immersion, maximum values of respiratory minute volume and respiratory frequency were significantly reduced by 35% and 38%, respectively, from the maxima observed for non-staged immersion. These results indicate that if staged immersion into cold water is possible, it can attenuat

Simulator sickness in U.S. Navy flight simulators.

Flight simulators have become a major factor in pilot training. A general finding from Navy research on simulator design is that equipment features that offer faithful representation improve pilot performance and promote pilot acceptance. To the extent that an aircraft produces motion sickness, its simulator should induce the same result. However, reports of simulator sickness appear to be increasing and a shortcoming in simulation is implied when these effects occur in simulators during maneuvers that do not occasion them in the aircraft. This article presents incidence data from surveys of t

Electrogastrograms during motion sickness in fasted and fed subjects.

Seven human volunteers were subjected to stressful Coriolis stimulation (rotating chair) either during the fasted state or following the ingestion of yogurt (6 oz). Subjects tested after yogurt reached a Malaise-III (M-III) endpoint of motion sickness after significantly (p less than 0.01) fewer head movements than subjects tested in the fasted state. Surface electrogastrogram (EGG) recordings at M-III were similar for both dietary states and consisted of a brief period of tachygastria, followed by a period of low amplitude EGG waves. Ingestion of yogurt enhanced susceptibility to motion sickn

Limitations of postural equilibrium tests for examining simulator sickness.

The psychometric properties of four ataxia tests and their sensitivities to disorientation were examined to assess their potential for measuring balance disturbances frequently reported with simulator sickness. The study was conducted in two parts. In the first, subjects practiced for 10 sessions to examine learning effects and to stabilize performance. In the second, the sensitivities of the four tests were examined by comparing performance before and after exposure to sensory conflict produced using a disorientation training flight simulator. Subjective measures of disorientation, including

Simulator induced syndrome: evidence for long-term aftereffects.

Pilots have experienced various adverse symptoms hours and even days following flight simulator training. This study surveyed pilots undergoing simulator training in an attempt to provide an initial gauge of the extent, severity, and possible risk factors of long-term (greater than 1 day) simulator aftereffects. A three-part voluntary and anonymous questionnaire was used to gather data. We studied pilots of varying experience undergoing advanced flight training which included the use of flight simulators. Some 238 pilots participated in the study, and 196 completed the follow-up survey concern

An evaluation of cognitive-behavioral therapy for training resistance to visually-induced motion sickness.

This investigation examined the techniques for reducing visually-induced motion sickness. On the basis of their responses to a motion sickness history questionnaire, 32 subjects were selected and assigned to 1 of 4 groups such that the groups were matched on the basis of their ability to tolerate visually-induced apparent motion (VM). One group received 10 sessions of desensitization training only (DT); a second group received 10 sessions of cognitive therapy only (CT); a third group received 10 sessions of combined desensitization and cognitive therapy treatment (CG); and a fourth group recei

Motion sickness and gastric myoelectric activity as a function of speed of rotation of a circular vection drum.

Motion sickness symptoms and electrogastrograms (EGGs) were obtained from 60 healthy subjects while they viewed an optokinetic drum rotated at one of four speeds: 15, 30, 60 or 90 degrees.s-1. All subjects experienced vection, illusory self-motion. Motion sickness symptoms increased as drum speed increased up to 60 degrees.s-1; i.e., symptoms decreased at 90 degrees.s-1. Power, spectral intensity, of the EGG at the tachygastria frequencies (4-9 cpm) was calculated at each drum rotation speed since previous studies have shown a close correspondence between development of tachygastrias and motio

Adaptation to vection-induced symptoms of motion sickness.

The visual-vestibular-proprioceptive sensory mismatch of vection provokes motion sickness in approximately 60% of healthy subjects. Approximately 60% of astronauts experience motion sickness in microgravity where vestibular/otolith function is altered. The purpose of this study was to determine the extent to which symptoms of motion sickness and tachygastria, an abnormal 4-9 cpm rhythm of the stomach, decrease or adapt during three repeated exposures to a rotating circular vection drum. Subjects sat in the drum for 45 min: 15 min baseline, 15 min drum rotation at 60 degrees.s-1, and 15 min rec

Performance and well-being under tilting conditions: the effects of visual reference and artificial horizon.

It is generally agreed that the incidence of motion sickness in sailors working below deck is higher than in sailors who have the horizon as a visual reference on the bridge. This study investigated the possible beneficial effect of a projected artificial horizon as a means to prevent seasickness. Twelve subjects were exposed to angular motion in a tilting room under three experimental conditions: a) With the windows covered, allowing no visual reference from the outside world; b) With the windows uncovered, thus allowing a partial view of the environment; and c) With the windows covered and a

Head cooling is desirable but not essential for preventing heat strain in pilots.

Liquid-cooled garments (LCGs) are being considered for reducing heat strain in pilots. While head cooling has been shown to be thermally efficient and subjectively desirable, it is technically difficult to achieve. This laboratory study was carried out to see if head cooling in addition to torso cooling is a necessity. Six male subjects wore a cooling vest and cap under summer flight clothing on three occasions in a climatic chamber set at Tdb = 42 degrees C, Twb = 32 degrees C (RH = 50%), Tg = 52 degrees C at head position, WBGT = 35 degrees C. Cooling conditions were: control (CTRL), no flui

Plasma renin activity and aldosterone: correlations with moderate hypohydration.

Adult male test subjects (n = 16) were assigned to one of three clothing configurations (Army fatigues, fatigues with impermeable chemical protective garments, and fatigues with protective garments plus protective masks) prior to exercise (level treadmill, 1.11 m/s, 50 min/h, 6 h) in a moderate (wet bulb globe temperature, WBGT = 23 degrees C) environment with ad lib water consumption. When protective masks were worn, two through-mask drinking systems were evaluated: the current gravity-fed system for fluid delivery and a new system utilizing a small hydraulic pump (Fist-Flex). Non-ejection cervical spine injuries due to +Gz in high performance aircraft.

The potential for significant neck injuries exists in today's high performance fighter aircraft. The G-loads required to produce injury need not be excessive, nor is experience level necessarily protective. Eight cervical spine injury cases, due to or aggravated by +Gz in F-15 and F-16 aircrew members are reviewed. These include two compression fractures (C5/C7), three left HNP's (C5-6/C6-7), one fracture of the spinous process (C7), one interspinous ligament tear (C6-7), and one myofascial syndrome (C6). Mechanisms of injury and evaluation are discussed. Exercise conditioning may play an impo

A preliminary report on a new anti-G maneuver.

Inspired by Traditional Chinese Medicine and Qigong, we designed a new anti-G maneuver, the Q-G Maneuver, which has proved promising. This maneuver consists of volition mobilization, stepwise tensing of leg and abdominal muscles, and maintenance of a shallow thoracic respiration throughout. It was tested on 24 pilots on the ground and 3 pilots on a centrifuge. All pilots were monitored with heart level blood pressure, oximetry, ear lobe pulse, CO2 concentration in exhaled gas, EEG and ECG; in centrifuge runs, peripheral vision was also monitored. Blood pressure was maintained at 180-240 mm Hg

High-G training for fighter aircrew.

From 8 Jan 85 through 12 Feb 86, 741 USAF fighter aircrew from Tactical Air Command underwent high-G training at the USAF School of Aerospace Medicine, the major objectives of the training being to increase their understanding of G stress and G protection and to raise their G tolerance. The didactics centered on discussion of the G-time tolerance curve and demonstration of an effective anti-G straining maneuver (AGSM). Exposure to G stress on the USAFSAM centrifuge allowed the trainees to determine their G tolerances and to perfect and practice their AGSM. The trainees' mean relaxed and strain

Hormonal status and fluid electrolyte metabolism in motion sickness.

In the first experimental series, 10 healthy male test subjects with a high susceptibility to motion sickness showed a significant increase of ACTH, cortisol, STH, prolactin, ADH, aldosterone concentrations, and plasma renin activity after vestibular tests. The 10 subjects with a moderate susceptibility exhibited a still higher increase of the hormones, except plasma renin. The 8 test subjects with a low susceptibility displayed a considerable increase in ACTH, cortisol, and STH after vestibular stimulation. In the second experimental series, the increase of STH, cortisol, ADH, aldosterone and
