[Wuchereria bancrofti: value of the diethylcarbamazine provocation test for the diurnal detection of microfilaria carriers].

Sixty-nine of 83 (83%) patients with circulating microfilariae of nocturnally periodic Wuchereria bancrofti were detected in daytime blood films following the one-time administration of diethylcarbamazine at a dose per patient of 1 +/- .25 mg/kg of body weight. This method of detecting microfilaria carriers in Haiti is recommended as a reasonably sensitive alternative when circumstances make it impossible to conduct a nighttime blood survey.

[Epidemiology of filariasis (onchocerciasis and bancroftosis) in the Tala-Mokolo region (Mandara Mountains of North Cameroon)].

The effect of the bancroftian filariasis on the health of the inhabitants are minimal in the study area, where the prevalence of the disease (nocturnal microfilaremia) is 0.4% only. But skin snips harboring Onchocerca microfilariae are seen in 12.1% of the sample studied. Onchocerciasis skin lesions, low visual acuity and blindness are often seen in the villages where the prevalence is high, among inhabitants more than 40 years of age. Simulium damnosum is seldom seen in the area, except in the large streams and rivers like the mayo Tsanaga and the mayo Djingliya. Larvae and nymphae of this sp

Brugia pahangi: stage-specific antigens on microfilariae detected by serum from infected jirds or by monoclonal antibodies.

Experiments were carried out to determine whether there are stage-specific antigens on microfilariae of Brugia pahangi, using sera from Mongolian jirds infected with B. pahangi and monoclonal antibodies against microfilariae of B. pahangi. These studies showed that microfilariae have both stage-specific and nonspecific antigens. The nonspecific antigens were also present on adult worms and on infective larvae. Among monoclonal antibodies, 6 out of 14 clones produced antibodies against the microfilarial stage-specific antigens, and 8 clones produced antibodies against nonspecific antigens. Thes

Identification of non-cross-reacting antigens of Onchocerca volvulus with lymphatic filariasis serum pools.

Onchocerca volvulus proteins labelled with 125I were immunoprecipitated with onchocerciasis and lymphatic filariasis human serum pools in order to differentiate between cross-reacting and non-cross-reacting antigens. Analysis of the immunoprecipitates by two-dimensional gel electrophoresis revealed that all high molecular weight (Mr) O. volvulus antigens cross-reacted with the lymphatic filariasis serum pools. We observed, however, that at least 8 O. volvulus antigens were specifically immunoprecipitated only by the onchocerciasis serum pools, with Mr ranging from 20,000 up to 43,000 Daltons.

Establishing a filarial clinical research platform in a resource-limited setting: Lessons and experiences from Tanzania.

This paper examines the establishment and lessons learned from the LeDoxy Trial, a multinational, double-blind, randomized, placebo-controlled study that evaluated doxycycline (200 mg/d and 100 mg/d) over six weeks for the treatment of filarial lymphedema in Tanzania's Lindi and Pwani regions. Conducted from 2018 to 2021 across four sites, the trial enrolled 420 participants aged 14-65 years, addressing a critical gap in lymphatic filariasis (LF) research in geographically remote and historically under-researched areas. The trial faced significant challenges, including limited pre-existing res

Asymptomatic lymphatic filariasis in an elderly patient from Bako, Sarawak: A case report and public health implications.

Lymphatic filariasis (LF) remains a significant public health challenge in many tropical regions where the disease is endemic. In Malaysia, LF is found in small pockets across the country. Asymptomatic carriers play a critical role in transmission but are often undetected. This report details an investigation of an asymptomatic filariasis reported by local health authorities involving an 83-year-old female patient residing in the Bako area, Sarawak. Despite being immobile due to a stroke, routine screening identified an infection with Brugia malayi through microscopy and a rapid diagnostic tes

When diabetes meets a neglected tropical disease: chronic lymphatic filariasis due to Wuchereria bancrofti-a case report.

BACKGROUND: Lymphatic filariasis (LF) is a neglected tropical disease in some parts of India and results in chronic lymphedema, hydrocele and acute inflammatory episodes. Even outreach to communities often yields undiagnosed cases of chronic cases needing confirmatory testing and tied-in care. This case illustrates the confluence between chronic infectious and metabolic diseases in rural India, and the requirement of comprehensive primary care. CASE REPORT: A 75-year-old South Asian male farmer with type-2 diabetes mellitus detected at a rural camp in the community from rural Kayar, Yavatmal

The effects of variable spatial aggregation on lymphatic filariasis transmission.

BACKGROUND: Elimination of lymphatic filariasis (LF) is a World Health Organization goal, with several countries at or near prevalence thresholds. Where LF cases remain after mass drug administration, they tend to be spatially clustered, with an overdispersed individual worm burden. Both individual and spatial heterogeneities can cause aggregation of infection; however, few studies have investigated the drivers of heterogeneity and implications for disease elimination. METHODS: We used a spatially explicit lymphatic filariasis model to investigate LF transmission in American Samoa at three sp

Lymphatic filariasis transmission 10 years after stopping mass drug administration in the Gomoa west district of Ghana.

OBJECTIVES: A survey was conducted 10 years after stopping MDA in the Gomoa West District of Ghana to assess the Wuchereria bancrofti prevalence in both human and mosquito populations. METHODS: In seven communities, infection in humans was assessed using the filariasis test strip (FTS). Mosquitoes were collected once a month over six months using pyrethrum spray catches (PSC). The mosquitoes were analyzed for W. bancrofti infections, using dissection followed by poolscreening PCR. RESULTS: FTS results showed that 2/524 (0.38%; 95% CI, 0.0%-0.9%) individuals tested positive for antigen. Disse

Beyond business as usual for lymphatic filariasis mass drug administration in hotspot districts.

BACKGROUND: This study aimed to validate the 'Engage & Treat' (E&T) and 'Test & Treat' (T&T) strategies to treat individuals who seldom or never participate in lymphatic filariasis mass drug administration (LF MDA). METHODS: The study was conducted in a hotspot district of Ghana, with 20 years of MDA. Treatment registers documenting participation were used to identify non-participating individuals. Individuals willingly accepting treatment were given the drugs (E&T). Those who refused treatment were offered testing using the filaria test strip (FTS) and further engaged to receive treatment if

Comparative analysis of skin microbiome of patients with filarial lymphedema and healthy individuals.

BACKGROUND: Lymphatic filariasis, a vector borne parasitic disease is a public health problem in the tropical region. Recurrent skin and soft tissue infections termed adenolymphangitis (ADL) is a major complication of filarial lymphedema. Understanding the changes in skin microbiome associated with this disease may provide novel insights on ADL attacks and lymphedema progression. This study investigates the changes in skin microbial flora in patients affected with filarial lymphedema. METHODS: We employed 16S rRNA gene amplicon-based metagenomic technique to profile the skin microbiome of pat

Potential Anti-Filarial Molecules Against ATP Binding Site of MurE Enzyme: A Molecular Docking and Dynamics Approach to Combat Lymphatic Filariasis.

Lymphatic filariasis (LF) is a mosquito-borne disease caused by parasitic nematodes Brugia malayi, Brugia timori, and Wuchereria bancrofti. The drugs available are effective in several cases, and the absence of vaccination is the crucial factor hindering the elimination of LF. The UDP-N-acetylmuramoyl-L-alanyl-D-glutamate-2,6-diaminopimelate ligase (MurE) plays an important role in the peptidoglycan biosynthesis of Wolbachia endosymbiont B. malayi, which are reported to be a vital drug target for bacterial and endosymbiotic hosts. Thus, we selected the ATP binding cavity of MurE as the potenti

Lymphatic filariasis elimination in the Dominican Republic: History, progress, and remaining steps.

Lymphatic filariasis (LF) is a mosquito-transmitted parasitic disease that is a leading cause of disability globally. The island of Hispaniola, which the Dominican Republic shares with Haiti, accounts for approximately 90% of LF cases in the Americas region. In 1998, the Dominican Ministry of Public Health created the Program to Eliminate Lymphatic Filariasis (PELF) with the goal of eliminating LF transmission by 2020. Baseline mapping revealed 19 (12% of total) endemic municipalities clustered into three geographic foci (Southwest, La Ciénaga and East), with a total at-risk population of 262,

Acidic Calcium-Independent Phospholipase A2 Regulates Eosinophil-Mediated Pathology during Filarial Manifestation of Tropical Pulmonary Eosinophilia.

Eosinophils mediate pathological manifestations during tropical pulmonary eosinophilia (TPE), a potentially fatal complication of lymphatic filariasis, by mechanisms that are incompletely understood. Using two-dimensional gel electrophoresis, mass spectrometry, flow cytometry, and pharmacological and functional studies, we identified acidic calcium-independent phospholipase A2 (aiPLA2) as the master regulator of TPE pathogenesis. FACS-sorted lung eosinophils from TPE mice exhibited aiPLA2-dependent activation characterized by heavy calcium influx, F-actin polymerization, increased degranulatio

The roles of stakeholder experience and organizational learning in declining mass drug administration coverage for lymphatic filariasis in Port-au-Prince, Haiti: A case study.

The World Health Organization (WHO) defines an effective round of mass drug administration (MDA) for lymphatic filariasis (LF) as one that reaches at least 65% of the target population. In its first round of MDA in 2011-2012, the National Program to Eliminate LF in Haiti achieved a 79% epidemiological coverage in urban Port-au-Prince. In 2013, coverage dropped below the WHO threshold and has declined year-over-year to a low of 41% in 2017. We conducted a retrospective qualitative case study to identify key factors behind the decline in coverage in Port-au-Prince and ways to address them. Our f
