Methods and pitfalls in treatment of fractures in the digits.

This article describes the indications for and methods of managing phalangeal fractures. The fractures around the finger joints are particularly difficult to treat. The technical details and pitfalls for these cases are discussed in detail. The problems associated with the phalangeal fractures such as articular destruction, nonunion, and postoperative care are also discussed. The authors present their preferred surgical treatment, with review of recent advancement regarding the treatment of phalangeal fractures.

Synovial cysts of the hips in seronegative arthritis.

A 25-year-old woman with recurrent pronounced enlargement of the iliopsoas bursae is described; her hips were evaluated by ultrasonography and MRI. They appeared normal and communications to the iliopsoas bursae were demonstrated. During the preceding 8 years she had intermittent seronegative non-erosive arthritis of the knee and finger joints and her disease was classified as juvenile chronic arthritis.

[A new special form of enchondromatosis].

A case of enchondromatosis with an unusual course was observed. The enchondromas originated in the patellae, subsequently with rapidly spreading metaepiphyseal growth on the knee joints and distally on the medial side of the right tibia. Cold-sensitive exanthema, pigment spots, drumstick fingers, watch glass nails, dentition disorders. Since radical surgery on the right femur 8 years ago there has been no recurrence.

Outcomes of Static External Fixation in Delayed Presentations of Dorsal PIPJ Subluxation.

Background: Neglected dorsal subluxation of the proximal interphalangeal joint (PIPJ) results in significant compromise of hand function. Operations like volar plate arthroplasty, hemihamate arthroplasty and joint fusions have been described to treat them. However, all are open procedures and technically demanding. Dynamic external fixation has been used as a treatment for acute injuries. There is a paucity of literature on the use of static external fixation in the treatment of neglected PIPJ fracture subluxations. We describe the outcomes of the use of static external fixation followed by sp

Use of dynamic distraction external fixation for unstable fracture-dislocations of the proximal interphalangeal joint.

PURPOSE: Unstable fracture-dislocations of the proximal interphalangeal (PIP) joint remain a difficult management problem, often leading to residual pain, stiffness, and recurrent instability. In a military setting, an easily applied, simple to operate, and inexpensive device becomes an attractive option. The purpose of this clinical investigation was to retrospectively review use of dynamic distraction external fixation (DDEF) for unstable fracture-dislocations and pilon injuries of the PIP joint in an active-duty population. METHODS: The fixator is assembled under a local anesthetic from th

Hyperostotic macrodactyly and lipofibromatous hamartoma of the median nerve associated with carpal tunnel syndrome.

A new case with 14-year follow-up of an extremely rare variety of congenital hand macrodactyly is presented. The disease characteristically presents a diffuse proliferation of fibrofatty tissue, but in this special type, osteocartilaginous deposits around the joints can also be found. The case presented included the troublesome feature of a lipofibromatous hamartoma in the median nerve at the wrist and its branches producing carpal tunnel syndrome. The patient obtained benefit from carpal tunnel release and epineurolysis. The hyperostotic development was managed with conservative resection of

Treatment of fracture-dislocation of the proximal interphalangeal joint using extension-block Kirschner wire.

Fourteen cases of fracture-dislocation of the proximal interphalangeal joint (PIPJ) were treated using extension-block Kirschner (K)-wire after closed or open reduction of the dislocation. The postoperative ROM of the PIPJ in the 14 patients averaged 94.4 degrees. A full ROM of the PIPJ was regained in 10 cases. This wire provides an alternative method to the use of extension block splintage in the management of such injuries.

Flexible implant arthroplasty in the proximal interphalangeal joint of the hand.

Between 1966 and 1981, 812 proximal interphalangeal joints were reconstructed by the senior author using the flexible silicone rubber implant resection arthroplasty method. Within this series, a group of 182 patients with 424 proximal interphalangeal joint implants were available for a detailed clinical and roentgenographic review. Increased motion in a more functional arc with good pain relief and a favorable bone response pattern were noted in most cases after surgery. Complications were rare and included implant dislocation or fracture, recurrence of deformity, and infection. The highest co

Biepicondylar fracture dislocation of the elbow joint concomitant with ulnar nerve injury.

In this article, we present a case of humeral biepicondylar fracture dislocation concomitant with ulnar nerve injury in a seventeen year-old male patient. Physical examination of our patient in the emergency room revealed a painful, edematous and deformed-looking left elbow joint. Hypoesthesia of the little finger was also diagnosed on the left hand. Radiological assessment ended up with a posterior fracture dislocation of the elbow joint accompanied by intra-articular loose bodies. Open reduction-Internal fixation of the fracture dislocation and ulnar nerve exploration were performed under ge

Heterotopic ossification of the extensor tendons in the hand associated with traumatic spinal cord injury.

Heterotopic ossification (HO) occurs in spinal cord injury (SCI), most frequently in the large joints such as hips, shoulders, knees, and elbows. It always occurs below the level of neurologic lesion. In the upper extremities, HO associated with SCI usually involves the flexor side of the involved joint. HO has only been reported once to involve the hands and rarely develops parallel to the long bones. We present a 44-year-old male with C5 traumatic SCI who developed HO involving the extensor tendons of one hand. The HO was discovered four months after the SCI and involved the extensor sheaths

Villonodular synovitis: pigmented and nonpigmented variations.

During the past 20 years, 34 cases of pigmented villonodular synovitis have been seen at Jackson Memorial Hospital, University of Miami Medical Center. There were 18 cases in the knee, eight in the hip, one in the elbow, two in the finger, three in the ankle, one in the wrist, and one in the sternoclavicular joint. In reviewing the pathology slides, it was difficult to differentiate fibrous histiocytoma from villonodular synovitis. The cause is considered to be inflammatory. The treatment is subtotal synovial resection with joint replacement as necessary. Recurrences after synovectomy are rare

Intramedullary Insetting of Silicone Implant for Lateral Stability in Distal Interphalangeal Joint Arthroplasty.

Silicone implant arthroplasty is an alternative surgical intervention for painful and deformed osteoarthritis of the distal interphalangeal (DIP) joints. DIP joint stability is essential for hand function; however, it carries a potential risk of postoperative joint instability. To address this concern, an intramedullary implant insetting method was used to maintain joint stability by minimum resection of the head of the middle phalanx and preserving the collateral ligament. In the new method, the length of the bone excision was limited to maintain the lateral cortical bone with the insertion o

Surgical treatment of proximal interphalangeal joint stress fracture in a climber.

Proximal interphalangeal joint stress fractures commonly occur as epiphyseal line injuries in teenage climbers; however, fractures with closed epiphyseal lines are rare. Poor outcomes are more common if treatment is delayed. We report the case of a 16-year-old climber who underwent delayed surgical treatment for a fracture with a closed epiphyseal line, with a favorable outcome. There was 20 ° extension lag at the proximal interphalangeal joint and a fracture on the dorsal aspect of the base of the metaphyseal bone, with osteosclerosis at the fracture site. The fracture site was refreshed and

Hemi-Hamate Autograft Arthroplasty for the Proximal Interphalangeal Joint Revisited: A New Surgical Approach.

The hemi-hamate arthroplasty for proximal interphalangeal joint (PIPJ) dorsal fracture dislocations relies on complete dislocation of the joint using the 'shotgun' approach which provides excellent exposure but damages the delicate intrinsic joint stabilisers. We present a new approach to the PIPJ when performing the hemi-hamate arthroplasty. The volar surgical approach involves freeing up the whole tendon sheath-periosteal unit as a single layer, and retracting this to one side. The articular surface of the joint can then be accessed with a little distraction and hyperextension. The method do

Dorsal dimelia in patau syndrome: a case report.

We present a case of a child with Patau syndrome that exhibits features consistent with congenital palmar nail syndrome. The literature is reviewed and evidence presented to demonstrate that this is a defect in the dorso-ventral patterning of the limb and thus a form of dorsal dimelia. In order to differentiate this from other instances of ectopic nail tissue we suggest congenital palmar nail syndrome should be more specifically defined as duplicated nails, absent flexion creases, non-glabrous skin on the palmar surface, reduced movement at the interphalangeal joints and hypoplastic terminal p

Trigger finger: the effect of partial release of the first annular pulley on triggering.

A prospective, randomized study was performed on 19 patients, in whom the proximal, middle, or distal third of the first annular pulley was divided to determine if a specific portion of the first annular pulley was responsible for the clinical triggering associated with restrictive flexor tenosynovitis. In all 19 patients, a partial resection of the first annular pulley resulted in continued clinical triggering with active digital flexion. At this point, a standard complete first annular pulley release was performed, with resolution of clinical triggering of the involved digit in all patients.
