Molecular analysis of dermatophytes suggests spread of infection among household members.

Dermatophyte infection from the same strains may be an important route for transmission of dermatophytoses within a household. In this study, we used molecular methods to identify dermatophytes in members of dermatophyte-infected households and evaluated variables associated with the spread of infection. Fungal species were identified by polymerase chain reaction (PCR) using primers targeting the internal transcribed spacer (ITS) regions (ITS1 and ITS4). For strain differentiation, fungal DNA was probed with a ribosomal DNA-specific probe (containing ITS1, 5.8S ribosomal DNA, and ITS2) to dete

Toxic dermatolysis, tissue necrosis and impaired wound healing due to sunitinib treatment leading to forefoot amputation.

Innovative treatment strategies in urologic oncology confront the treating physician with a new spectrum of adverse events. With growing understanding of underlying pathomechanisms, we need to identify contraindications against the use of certain antiproliferative drugs. The management of toxicities involves a multidisciplinary approach and thus, the exchange of experience across medical specialties is mandatory. We report a case of fulminant toxic dermatolysis, tissue necrosis and impaired wound healing resulting in the amputation of one forefoot after 6 days of treatment with sunitinib.

Canine symmetrical lupoid onychodystrophy: a retrospective study with particular reference to management.

The records of six dogs in which a diagnosis of symmetrical lupoid onychodystrophy (SLO) had been made were examined retrospectively. The age at onset ranged from six months to eight years. All the dogs had been presented with onycholysis, onychomadesis, onychalgia and onychodystrophy. The diagnosis of SLO was confirmed in all cases by histological examination. Histological features were similar in all cases and included hydropic degeneration of the basal cell layer, pigmentary incontinence and a cell-rich interface dermatitis. Response (defined as good, partial or failure) to various therapie

The biotin requirement of broilers feed maize-palm kernel meal based ration.

6 duplicate floor pens with 20-day-old commercial broiler chicks each, were fed practical-type broiler diet with maize-palm kernel meal, supplemented with graded levels of biotin so that the rations had 40, 80, 120, 160, 200, and 240 micrograms of the vitamin per kg of feed. This treatment was maintained for 6 weeks. Estimation of the body weight, feed intake, blood glucose concentration, and lipid contents of liver, kidney and blood showed that dietary biotin of 120 micrograms/kg feed is a minimum requirement. 160 micrograms/kg feed was required for the prevention of dermatitis, mortality due

Efficacy of combination therapy with efinaconazole 10% solution and 1064 nm Nd:YAG laser for treatment of toenail onychomycosis.

BACKGROUND: Onychomycosis is a common but difficult to treat nail disorder. Treatment strategies thus far have included oral and topical antifungals, surgical treatment and recently lasers have emerged as a therapeutic modality. OBJECTIVE: The objective of this study was to assess whether efinaconazole together with laser would result in greater clinical and mycologic cure and lower rate of relapse compared to efinaconazole alone. METHODS: Thirty subjects were randomized to either self-apply efinaconazole 10% once daily for 48 weeks, or follow the same treatment plan but also receive six tre

Case of cutaneous botryomycosis in an 8-year-old immunocompetent boy with a review of the published work.

Botryomycosis is a rare chronic suppurative granulomatous infection caused by several genera of non-filamentous bacteria. The clinical and histopathological findings are similar to those of mycetoma caused by true fungi or aerobic actinomycetes. Botryomycosis is divided into cutaneous and visceral disease, with the cutaneous form being more common. Histopathology shows granules of etiologic bacteria called "sulfur granules". Botryomycosis occurs more commonly among immunocompromised patients, although some cases have also been reported in immunocompetent patients. We report the case of an 8-ye

Global perspectives for the management of onychomycosis.

Onychomycosis is a fungal nail infection caused by dermatophytes, nondermatophyte molds, and yeasts. This difficult-to-treat chronic infection has a tendency to relapse despite treatment. This paper aims to offer a global perspective on onychomycosis management from expert physicians from around the world. Overall, the majority of experts surveyed used systemic, topical, and combination treatments approved in their countries and monitored patients based on the product insert or government recommendations. Although the basics of treating onychomycosis were similar between countries, slight diff

Efficacy of debridement alone versus debridement combined with topical antifungal nail lacquer for the treatment of pedal onychomycosis: a randomized, controlled trial.

UNLABELLED: Pedal onychomycosis is a common malady caused by dermatophytes, saprophytes, and yeasts. Traditional treatment options for this condition include toenail debridement, and pharmacological therapies that range from the application of topical agents to the oral administration of antifungal medications. In this study, 55 patients (289 toenails) were randomly allocated to either nail debridement (27 [49.09%] patients) or debridement plus application of topical antifungal nail lacquer (28 [50.91%] patients). The primary outcome was mycological cure, and secondary outcomes included foot-r

Impact of shared decision-making in onychomycosis treatment: A cross-sectional study based on the Illuminating Dialogues and Insights in Onychomycosis Management (IDIOM) survey.

Onychomycosis is a fungal infection of the nail characterized by deformity and discoloration. Besides aesthetic concerns, it can impair patients' quality of life and increase the risk of secondary bacterial infections as well as the spread of fungal infections to other body sites and other individuals, making it a potential public health concern. Treatment options include oral and topical medications, the latter often requiring long-term therapy. While oral antifungal agents demonstrate high efficacy against onychomycosis, concerns about side effects and drug-drug interactions may act as barri

Antifungal Drugs for Onychomycosis: Efficacy, Safety, and Mechanisms of Action.

In 1996, oral terbinafine joined itraconazole and fluconazole on the short list of systemic medications that could be used to treat onychomycosis (although fluconazole was not approved for this indication by the US Food and Drug Administration [FDA], it was commonly used for this purpose). In 1999, ciclopirox was the first topical treatment to be FDA approved. The addition of the topical antifungal agents efinaconazole and tavaborole in 2014 expanded the roster of medications available to more effectively manage onychomycosis in a wide range of patients, including those for whom comorbid condi
