A man with an infected finger: a case report.

INTRODUCTION: Whitlow is an infection of a finger or around the fingernails, generally caused by bacterium. However, in rare cases, it may also be caused by the herpes simplex virus. As herpetic whitlow is not seen often, it may go under-recognised or be mistaken for a different kind of infection of the finger. Delayed recognition and/or treatment puts patients at risk of complications ranging from superinfection to herpetic encephalitis. CASE PRESENTATION: A 23-year-old Caucasian man with no medical history was referred by his primary care physician because of erythema and swelling of the li

Pseudocheckerboard pattern: an interesting histopathological finding in mechanic's hands.

BACKGROUND: Mechanic's hands are a well-known symptom often associated with dermatomyositis and are similar to hand eczema clinically. Histopathology of them usually shows hyperkeratosis, focal parakeratosis, psoriasiform acanthosis, basal vacuolar changes and necrotic keratinocytes in the epidermis and upper dermal mucin deposition. However, there have been few comparative histopathologic evaluations of mechanic's hands and palmoplantar eczema. METHODS: We evaluated the histopathology of mechanic's hands in 6 patients with dermatomyositis who visited our hospital between 2006 and 2014 compar

A pyogenic granuloma treated with topical timolol.

Pyogenic granulomas are benign vascular tumours usually surgically treated by excision or curettage. The only topical therapy reported to date is imiquimod 5% cream but it produces a marked inflammatory response with discomfort generally worse than simple surgical excision or curettage. Here we report the use of timolol 0·5% ophthalmic gel used successfully to treat a teenager with a clinically classical pyogenic granuloma on the finger. There were no reported adverse effects and the lesion completely resolved. It has not recurred after 7 months. Timolol may be an effective and preferable alte

Allergic contact dermatitis to medical equipment in a non-health care worker caring for a disabled spouse.

BACKGROUND: Allergic contact dermatitis (ACD) to rubber accelerators is a common occurrence, especially in health care workers, but the risks in family caregivers are not as well documented. OBJECTIVE: To document a case of a 66-year-old woman who developed recurrent, extensive flares of ACD from contact with rubber accelerators through caregiving. METHOD AND RESULTS: Patch testing revealed a positive reaction to two common rubber accelerators: carba mix (3+) and thiuram mix (3+). The patient described caring for her disabled husband, which involved handling rubberized medical equipment. CO

Validity of self-reported nickel allergy.

BACKGROUND: To estimate the prevalence of nickel allergy, self-reports are sometimes used in epidemiological studies. Self-reports are practical and may facilitate estimation of prevalence provided that the questions are validated. OBJECTIVES: To investigate the validity of self-reported nickel allergy. METHODS: Three hundred and sixty-nine women, aged 30-40 years, from the general population participated in the study. The participants answered a questionnaire before a clinical examination and patch testing. The two questions being validated were 'Are you sensitive/hypersensitive/allergic to

[Acral necrosis in metastatic ovarian carcinoma. A single episode of Moschowitz syndrome during gemcitabine chemotherapy].

For some time now, there have been reports of acral necrosis as a paraneoplasia that may occur in association with a number of different malignant tumours. There have also been a series of reports about acral necrosis associated with chemotherapy with various cytostatics. The treatment of choice if these lesions occur is plasmapheresis. Ultimately, the occurrence of thrombotic microangiopathy (TMA) can only be prevented by close monitoring through regular laboratory controls before each new cycle of chemotherapy. In the differential diagnosis, Raynaud's syndrome should be considered as a premo

[The three-step programme of skin protection. A useful instrument of primary prevention or more effective in secondary prevention?].

For many years the three-step programme of skin protection, consisting of skin protection before work, cleaning and skin care after work, is one of the generally recommended measures to prevent occupational contact dermatitis. While protective creams are supposed to prevent skin damage caused by irritant contact, skin cleansing should mildly remove aggressive substances from the skin, whereas post-exposure skin care is intended to enhance epidermal barrier regeneration. This programme is strongly followed by most of the employees. But in spite of intensive preventive measurements the rate of r

Whitish papules on the palm.

A 21-year-old woman presented with a 1-month history of whitish bubbling of the palms after brief exposure to water. The symptoms arose within 3-5 min after immersion in water, as a white sponge-like appearance, and resolved after a variable drying period. She was otherwise healthy and did not complain of hyperhidrosis. There was no family history of similar lesions. Physical examination revealed a healthy appearance of the palms; however, 5 min after immersion of the hands in warm water, the central part of the left palm became swollen and tiny white papules with dilated puncta appeared. The
