BACKGROUND: The goal of this study was to evaluate the performance of haemothorax quantification to predict an abundant transfusion in blunt thoracic trauma patients.
METHODS: This study included all severe trauma patients admitted into our trauma centre from January 2005 to January 2010, who presented a blunt thoracic trauma (thoracic AIS ≥1) and had a CT scan within the first hour following admission. For each haemothorax, thickness of dominant side and the cumulated thicknesses of both sides (Dominant-t, Cumulated-t), as well as lengths (Dominant-l, Cumulated-l) and the calculated volumes

In the landmark group, artery puncture and hematoma occurred in 5.4% of patients, respectively, hemothorax in 4.4%, pneumothorax in 4.9%, brachial plexus injury in 2.9%, phrenic nerve injury in 1.5%, and cardiac tamponade in 0.5%, which were all increased compared with the ultrasound group (p < .05). Catheter misplacements did not differ between groups. …

BACKGROUND: Although it brings satisfactory rib hump correction, concomitant thoracoplasty with surgical correction of scoliosis decreases p … e in 1 second in the N-T group and CSLRR group were better than that in the C-T group. There were 2 hemothorax in the C-T group and 1 pleural effusion in the CSLRR group.
CONCLUSIONS: CSLRR showed significantly better rib hump and thoracic hypokyphosis correction without pulmonary function compromise in the treatment of thoracic adolescent idiopathic scolio …

OBJECTIVE: Our study was undertaken to determine the location of the tortuous intercostal artery in elderly patients by using 3D-CT angiography in order to prevent laceration during thoracentesis.
METHODS: We evaluated the data of 3D-CT angiography of the intercostal artery in consecutive patients who had undergone contrast chest CT scan in our hospital from December 2007 to April 2008. We considered the "percent safe space" (the shortest lower rib-to-intercostal artery distance/the upper rib-to-lower rib distance) to be an index of safety that can be used to prevent laceration of the interco

The authors report the case of a death which occurred during wild boar hunting. The scene details revealed that the victim and the beast were aligned in the visual shooting path and the victim had been struck by overpenetrating bullet shot by his friend on the neighbouring stand. The fatal entrance wound was on the victim's left chest. The authors emphasise the importance of the exhaustive forensic investigation, including autopsy of the beast, in cases like this, in order to distinguish accident from homicide.

Tracheopathia osteoplastica is a benign cartilaginous and osseous metaplasia of the laryngo-tracheobronchial tree diagnosed more commonly in adults over 50 years of age. We report here the case of a 54-year-old man who underwent thyroidectomy for multinodular goiter. Immediately after an uneventful surgery, he developed an acute respiratory failure with radiologic picture of Adult Respiratory Distress Syndrome. Mechanical ventilation was set up again, bronchoscopy with biopsy disclosed a massive tracheobronchial haemorrhage from a tracheopathia osteoplastica. Supportive treatment was successfu

A wide spectrum of complications is associated with invasive hemodynamic monitoring. We report an unusual case of perforation of the left internal mammary artery that occurred during percutaneous insertion of a Swan-Ganz catheter using the subclavian vein approach. To the best of our knowledge, this complication has not been previously reported.

Deiraniya, A. K. (1974).Thorax, 29, 545-552. Cardiac herniation following intrapericardial pneumonectomy. Cardiac herniation is a rare and catastrophic complication of intrapericardial pneumonectomy. Untreated it is invariably fatal. This paper reports three cases of cardiac herniation following intrapericardial pneumonectomy. In two cases massive haemorrhage complicated the cardiac herniation. All three cases were re-explored with two immediate survivals. The diagnosis, aetiology, haemodynamic effects, and management of this complication are discussed, and previously reported cases are review

Thoracic injuries account for a high percentage of combat casualty mortalities, with 80% of preventable deaths resulting from abdominal or thoracic hemorrhage. An effective method for detecting and triaging thoracic injuries is point-of-care ultrasound (POCUS), as it is a cheap and portable noninvasive imaging method. POCUS image interpretation of pneumothorax (PTX) or hemothorax (HTX) injuries requires a skilled radiologist, which will likely not be available in austere situations where injury detection and triage are most critical. With the recent growth in artificial intelligence (AI) for h…

All patients with a diagnosis of hemothorax or pneumothorax (H/PTX) from blunt or penetrating trauma that resulted in a >24 French chest tube placement were included in the study. Patients were excluded if interventional radiology (IR) placed the chest tube, the mechanism was nontraumatic, or …

Thoracic outlet syndrome (TOS) is a rare condition (1-3 per 100,000) caused by neurovascular compression at the thoracic outlet and presents … to the subclavian vessels potentially leading to exsanguination and death, brachial plexus injury, hemothorax, and pneumothorax. In this review, we outline the diagnostic tests and treatment options for TOS to better guide clinicians in recognizing and treating vascular TOS and objectively verifiable forms of neurogenic TOS.

BACKGROUND: Thoracoabdominal trauma presents a diagnostic challenge for the emergency physician. The introduction of bedside ultrasonography (USG) provides a screening tool to detect hemoperitoneum, hemothorax, pneumothorax and pericardial effusion in torso injuries.
AIM: To evaluate the accuracy of extended focused assessment with sonography for trauma (EFAST) for chest and abdominal injuries performed by first responders in a tertiary care hospital of Nepal.
METHODS: This was a prospective study including all trauma patients who obtained either an Injury Severity Score ≥15 or direct trauma…
