A 50-year-old male, with a medical history of Lynch syndrome and transurethral-resection (TUR) secondary to multifocal bladder tumour (pT1-high grade) with normal subsequent follow-ups, consulted for anal pain, rectal tenesmus and fever for 3 weeks. On examination, he presented perianal oedema and, on digital rectal examination, a right lateral orifice was palpable at 6cm from the anal margin. CT scan revealed a defect-in-continuity in the right rectal wall which communicated with bilateral perirectal collections extending towards the puborectalis-levator ani. On the right side, it extended to

We report a case who was born with extremely low birth weight infant and had experienced abdominal operation for necrotizing enterocolitis, eventually developed ileus due to fatty acid calcium stones after giving human milk fortifier. He had developed necrotizing enterocolitis on day 30 of his age, such that we performed enterectomy and ileostomy. He could not tolerate enteral feeding fully, because intestinal fistula infection was repeated. Although we administered hindmilk, he grew up slowly and he suffered cholestasis as well. We performed end-to-end anastomosis to prevent fistula infection…

BACKGROUND: Children are the primary candidates for intestinal transplant with more than 70% requiring a combined liver-bowel transplant. We report our single-center experience with living donor intestinal transplantation (LDITx) and combined living donor intestinal and liver transplant (CLDILTx) in pediatric patients.
PATIENTS AND METHODS: Between October 2002 and June 2006, 13 living donor intestinal grafts were transplanted in 10 recipients. In five cases CLDILTx was performed. The intestinal grafts consisted of a 150-cm segment of ileum, whereas the liver transplant was completed using st

A 26-year-old man reported diarrhoea, abdominal pain and weight loss (16 kg in 3 weeks). Ultrasonography and computed tomography showed a 5 x 5 cm abnormal rosette-shaped mass with surrounding infiltration in the area of the left colic flexure. Laboratory tests indicated a florid inflammation. Coloscopy and double-contrast radiology of the colon revealed a fistula at the junction between sigmoid and descending colon. At laparotomy a large tumorous mass was found (18 x 15 x 10 cm), which was removed completely by partial resection of small intestine and colon. Histological examination revealed

Operative management of infants with necrotizing enterocolitis and its complications requires a repertoire of surgical techniques, many of which are unique to pediatric surgery. Survival rates for infants with this once deadly illness are improving. Choice of the correct operative procedure and precise technical performance of the technique are mandatory for survival of the infant.

Complications (stenosis, intestinal obstruction, abscess or fistula formation, inflammation, etc.) of colonic diverticulitis are seen in 35% of patients affected. Multiple operative procedures involve less risk than surgery performed in a single session. The mortality in emergency surgery is reported to be up to 40%, while subsequent resection of the diseased colon involves the usual mortality of surgery on the colon, i.e. 1.5-9%. The vital factor in this illness is the earliest possible surgical treatment of the diseased diverticulum, so that the extremely problematical complications can be a

BACKGROUND: A colovesical fistula (CVF) is commonly treated by resection of the intestine containing the fistula or creation of a defunctioning stoma. We herein report a case of successful fistula closure and avoidance of colostomy after placement of a covered colonic self-expanding metallic stent (SEMS) as a palliative treatment for a malignant CVF.
CASE PRESENTATION: A 75-year-old man undergoing infusional 5-fluorouracil and irinotecan chemotherapy plus bevacizumab for recurrent peritoneal dissemination of rectal cancer was admitted to our hospital because of fecaluria with a high-grade fev

Bouveret syndrome, a rare form of gallstone ileus predominantly encountered in elderly female patients, is clinically characterized by the sequelae of duodenal obstruction. Two patients observed within one year are presented and the clinical, diagnostic and therapeutic aspects of the disease are discussed. The surgical treatment of choice is one-stage removal of stone, closure of fistula and cholecystectomy completed by a careful search for residual stones in the small and large intestine.

INTRODUCTION AND IMPORTANCE: Biliary ileus is a rare yet significant cause of mechanical intestinal obstruction, which occurs when a gallstone enters the gastrointestinal tract through a bilioenteric fistula, leading to intestinal blockage. This condition primarily affects elderly patients and is associated with high morbidity and mortality if not diagnosed and treated promptly.
CASE PRESENTATION: We present the case of a 94-year-old female with a history of hypertension and chronic venous insufficiency. The patient was admitted with severe abdominal pain, nausea, and vomiting, with clinical

A 4-year-old girl presented with asymptomatic bowel perforation and transanal protrusion of a ventriculoperitoneal (VP) shunt catheter. She had undergone repair of myelomeningocele at birth and subsequent VP shunting for congenital hydrocephalus 1 month later. Seven months after VP shunting, she underwent revision of the peritoneal catheter. She complained of abdominal pain and nausea at the age of 4 years. She was treated conservatively for 1 month for intestinal obstruction. One month later, her mother noticed the shunt catheter protruding from her anus. Computed tomography (CT) of the abdom

BACKGROUND: Bouveret syndrome is a rare cause of gastric outlet obstruction resulting from the passage of a gallstone into the duodenum or stomach through a biliary-enteric fistula. It is a complication of chronic cholelithiasis and accounts for only 1%-3% of all gallstone-related intestinal obstructions.
CASE SUMMARY: A 49-year-old male presented with nausea, vomiting, and upper abdominal pain. Imaging revealed a large gastric stone and a cholecystoduodenal fistula, confirming Bouveret syndrome. An initial endoscopic attempt to remove the stone was unsuccessful, necessitating surgical interv
