PURPOSE: Understanding how surgical procedures influence anatomic factors associated with patellofemoral instability can help guide surgeons when planning treatments for individual patients. This study sought to understand how patellar tilt is affected in adolescent patients with elevated pre-operative tibial tuberosity to trochlear groove (TT-TG) values undergoing medial patellofemoral ligament reconstruction (MPFLR) with or without an anterior medializing osteotomy (AMZ).
METHODS: Utilizing a prospective database of 274 patellofemoral instability patients who underwent MPFLR ± AMZ by one of

PURPOSE: The aim of this study was to evaluate the correlation between tibial tuberosity-trochlear groove distance (TT-TG) and body height or knee size, and to find height-related pathologic thresholds of increased TT-TG.
METHODS: One-hundred and fifty-three patients with recurrent patellar instability and 151 controls were included. The TT-TG was measured on axial computed tomography (CT) images. Femora width and tibial width were selected to represent knee size. The correlation of TT-TG and gender, body height, femora width, and tibial width was evaluated. The height-related pathologic thre

PURPOSE: Focal, patellar cartilage defects are a challenging problem as most cases have an underlying multifactorial pathogenesis. This systematic review of current literature analysed clinical results after regenerative cartilage repair of the patella with a special focus on the assessment and treatment of existing patellofemoral malalignment.
METHODS: A systematic review was conducted to identify articles reporting clinical results after cartilage regenerative surgeries of the patella using the PubMed and Scopus database. The extracted data included patient-reported outcome measures (PROMS)

Reconstruction of the medial patellofemoral ligament (MPFL) has been increasing as a surgical solution for treatment of recurrent lateral patellofemoral dislocation. Recent attention has been given to fibers extending from the femur to the quadriceps tendon, proximal to the MPFL, termed the medial quadriceps tendon-femoral ligament. This article briefly reviews the proximal medial patellar restraints and surgical procedures for their reconstruction.

PURPOSE: To conduct a systematic review of meta-analyses comparing nonoperative and operative treatment of patellar dislocations to elucidate the cause of the variation and to determine which meta-analysis provides the current best available evidence.
METHODS: A systematic review of the literature to identify meta-analyses was performed. Data were extracted for patient outcomes and recurrent dislocations. Meta-analysis quality was assessed using the Oxman-Guyatt and Quality of Reporting of Meta-analyses systems. The Jadad algorithm was then applied to determine which meta-analysis provided th

UNLABELLED: Trochleoplasty is a surgical procedure, whose goal is to change the abnormal shape of the femoral trochlea in patients with recurrent patellar dislocation. Such surgeries that aim to reshape the articular surface of a bone without damaging it are quite unique in orthopaedic surgery. Although in the beginning, trochleoplasty was reserved for the refractory cases where previous surgery has failed, the last years it became more and more popular because of a better knowledge of anatomy and biomechanics and a greater availability of instruments. The roots of the deepening trochleoplasty

A clinical observation of 49 hips in children who had closed reduction and application of hip spica for developmental dysplasia of hip (DDH) was undertaken in our department. A clinical sign of "Limited Extension at ipsilateral knee" was observed after successful closed reduction of the hip. This sign was absent if the hip was not reduced. This sign was due to hamstring tightness after successful closed reduction. We suggest that the presence of this sign is a reliable indicator of successful closed reduction and also an indicator of the vulnerability of the proximal femoral epiphysis to incre

A 23-year-old morbidly obese woman (BMI: 62,5 kg/m2) came to the ER because of a painful right knee. When walking the knee had collapsed spontaneously. Physical examination of the right leg was not possible due to pain. Examination of the unaffected left leg revealed multi-directional instability. A radiograph showed anterior luxation of the tibia.

We present a case of synovial chondromatosis originating from the quadriceps tendon sheath, which caused a complete quadriceps tendon rupture. The patient was treated using marginal excision. The ruptured quadriceps tendon was repaired. This is the first description of a quadriceps tendon rupture associated with synovial chondromatosis.

The presence of many "abnormal" types of PF joints in asymptomatic cases and the frequent unilateral complaints in morphologically symmetric joints indicate that emphasis must be placed on factors that cause "abnormal" PF joints to decompensate, rendering them symptomatic. These factors include hyperlaxity, puberty, overuse, or athletic activities. It is our conviction that full understanding of PF problems needs dynamic, three-dimensional motion analysis of patellofemoral relationships. At the present time these imaging techniques do not exist, as they require more powerful computer capacitie

We devised a method to measure the Q-angle (the angle between the rectus femoris and the patellar ligament), using computed tomography (CT), in 1993. In this study, I investigated the lateral shift at each point forming the angle as factors affecting the Q-angle. The study group consisted of 83 patients with recurrent patellar dislocation (83 lower limbs) and a control group of 55 healthy people (55 lower limbs). A lateral shift was found in the anterosuperior iliac spine (ASIS) in the dislocation group compared with the control group (P < 0.01). A lateral shift was also found in the anteroinf

This review focuses on the complications of extensor mechanism surgery for symptomatic patellar malalignment and ways to avoid those complications. We combined a review of new and referred patients who had complications resulting from extensor mechanism surgery with a review of selected literature specifically addressing similar complications. By studying the pathologic anatomy at the time of arthrotomy, it was possible in many cases to determine the causes of adverse outcomes with this type of surgery. The complications of extensor mechanism surgery can be avoided by performing a thorough pre

A kinematic magnetic resonance (MR) imaging technique for assessment of malalignment of the patella, involving the acquisition of multiple sequential axial images of the patellofemoral joint during the early increments of passive knee flexion, was used to evaluate 130 patients (235 symptomatic patellofemoral joints) showing clinical evidence of having patellar tracking abnormalities. Twenty-three of the patellofemoral joints had undergone previous surgical procedures for patellar realignment. In addition, 14 (28 patellofemoral joints) asymptomatic control subjects were studied. Normal patellar

A 28-year-old female pedestrians, struck by an automobile, sustained an obturator hip dislocation with concomitant ipsilateral fractures of both the femoral head and femoral neck. This unusual hip injury seems not to have been previously reported. The femoral neck fracture was widely separated from the shaft, and unfortunately, a 6-mm deep indentation fracture involved the anterosuperior portion of the articulating surface of the femoral head. This complex injury was treated with a press-fit bipolar prosthesis.

BACKGROUND: Limited information is available about revision after lateral unicompartmental knee arthroplasty (UKA) due to infrequency of this procedure. The purpose of this study is to examine reason for failure, time to failure, patient characteristics, type of revision and re-revision surgeries after failed mobile- and fixed-bearing lateral UKA.
METHODS: In this single centre study, a total of 43 patients who underwent a revision surgery between 2010 and 2024 after mobile- and fixed-bearing lateral UKA implantation were analyzed. Demographic information, time to revision surgery, reason for
