Personalized music improves epileptiform activity, REM sleep and quality of life in children with epilepsies.

PURPOSE: There is an indication for EEG entrainment of rhythm, in particular in association with the regulation of frequencies related to epileptic seizures. We present the X-System model, which may predict the neurophysiological effects of music. We suggest that this may "entrain" healthy electrical brain activity more effectively than previously-used, intuitive choices of music. METHODS: We enrolled 19 subjects (5-17 years old) with epilepsy. All children had overnight video/EEG monitoring and polysomnography before, upon starting and three weeks after starting all-night music treatment. A

Exploring the mechanistic link between the oxytocinergic system and mindfulness training in adults with heightened stress: study protocol for a double-blind, randomized, placebo-controlled trial (MOX-MIND).

BACKGROUND: Oxytocin administration is increasingly considered a novel therapeutic support option for alleviating psychological distress in stress-related neuropsychiatric conditions, including anxiety disorders and depression. However, oxytocin as a stand-alone treatment may fail to consistently target the relevant central autonomic circuitry without a complementary supportive therapeutic context that similarly stimulates stress-regulatory states and behaviors. Recent findings suggest that the neurophysiological mechanisms underlying stress regulation induced by mindfulness may rely on an act

EEG microstate analysis in trigeminal neuralgia: identifying potential biomarkers for enhanced diagnostic accuracy.

OBJECTIVE: This study investigated EEG microstate dynamics in trigeminal neuralgia (TN) patients to understand the central nervous system's contribution to this neuropathic pain condition. Despite TN's traditional classification as a peripheral neuropathy, altered brain network organization may play a critical role in pain chronification and treatment resistance, making EEG microstates a valuable tool for capturing these dynamic neural signatures. METHODS: We analyzed resting-state EEG recordings from 14 healthy individuals and 36 TN patients through a systematic analytical pipeline. After pr

Limited Pre-Speech Auditory Modulation in Individuals Who Stutter: Data and Hypotheses.

Purpose We review and interpret our recent series of studies investigating motor-to-auditory influences during speech movement planning in fluent speakers and speakers who stutter. In those studies, we recorded auditory evoked potentials in response to probe tones presented immediately prior to speaking or at the equivalent time in no-speaking control conditions. As a measure of pre-speech auditory modulation (PSAM), we calculated changes in auditory evoked potential amplitude in the speaking conditions relative to the no-speaking conditions. Whereas adults who do not stutter consistently show

Neurophysiological assessment of the motor and sensory spinal pathways in chronic spinal cord injury.

Introduction of transcranial magnetic stimulation (TMS) has provided means to study non-invasively corticospinal functions in humans. The purpose of the present study was to obtain an objective evaluation of spinal cord functions in spinal cord injury (SCI) subjects using TMS, multichannel surface EMG and somatosensory-evoked potentials (SSEP). Multichannel surface EMG recording was performed during reinforcement manoeuvres and during vibratory tonic reflex. Twenty-five post-traumatic clinically incomplete (ambulatory, AMB, and non-ambulatory, nAMB) SCI subjects were studied and compared to a

Hippocampal interictal spikes disrupt theta-band hippocampal-frontal eye field connectivity: fixation skewness as an indicator of transient network instability.

OBJECTIVE: Hippocampal interictal epileptiform discharges (IEDs) could induce transient visual working memory (VWM) impairments, yet the effect of IEDs on VWM network remain unclear. Since VWM depends on dynamic coordination between memory representations in the hippocampus (HP) and visual exploration controlled by the frontal eye field (FEF), we tested whether hippocampal IEDs disrupt this circuit's functional connectivity and whether such disruption is reflected in altered viewing patterns, offering a behavioral readout of transient network dysfunction. METHODS: Seven epilepsy patients with

Neuropsychiatric Phenotype and Treatment Challenges in 47,XYY Syndrome: A Narrative Review with a Case Series of Adolescents.

BACKGROUND: 47,XYY syndrome is a relatively common sex chromosome aneuploidy that remains largely underdiagnosed. While its somatic phenotype is often mild, growing evidence indicates a substantial burden of neurodevelopmental and psychiatric morbidity. However, the characterization of the neuropsychiatric phenotype across development, particularly during adolescence, and the associated treatment challenges remain incomplete. OBJECTIVES: To provide a comprehensive narrative review of the neuropsychiatric phenotype of 47,XYY syndrome and to illustrate clinical complexity and treatment response

Tau pathology in epilepsy: emerging mechanisms and translational opportunities.

The onset of epilepsy in adulthood occurs most commonly after 55 years of age. Given the ageing global population, this disorder represents an increasing burden on healthcare and society. The bidirectional link between epilepsy and dementia is a focus of intense research with underlying tau pathology highlighted as a potential mechanistic link. In this review, we examine the evidence for tau-related neurodegenerative processes in epilepsy beginning with how changes in biochemical and structural properties of the tau protein can lead to abnormal phosphorylation and pathological aggregation. We

Stimulation of spinal afferents from skin activates parabrachial neurons that respond to taste.

The parabrachial (PB) nucleus participates in neural coding for diverse sensory modalities, including nociception and taste. Recent neurophysiological studies identified a subpopulation of PB neurons that responds to both gustatory and oral nociceptive (trigeminal) stimuli. These taste-integrative neurons populate PB subnuclei that receive somatosensory input from bodily skin supplied by spinal afferents. Yet whether spinal sensory input can excite these cells was unexplored. Here, we applied electrophysiological and optogenetic techniques in anesthetized TRPV1; Ai32 mice, which support transd

Apparent Cerebellar Volumetric Stabilization and Emergent Cortical Hyperexcitability During Long-Acting Injectable Aripiprazole Maintenance in Adolescent Bipolar Disorder.

Background and Clinical Significance: This report investigates the complex intersection of macrostructural neuroprotection and cortical hyperexcitability during long-acting atypical antipsychotic maintenance. We present a novel clinical case demonstrating an apparent absence of detectable cerebellar vermis atrophy progression during long-acting injectable (LAI) aripiprazole maintenance, which temporally coincided with the emergence of a potential epileptogenic risk in an adolescent with bipolar disorder (BD) and cannabis use disorder. Beyond motor precision, the vermis modulates emotional-cogn
