Probiotics in pediatric functional abdominal pain disorders: a systematic review and meta-analysis.

UNLABELLED: Functional abdominal pain disorders (FAPDs), currently referred to as abdominal pain-related disorders of gut-brain interaction (AP-DGBIs), are characterized by recurrent abdominal discomfort in children that cannot be fully explained by an identifiable organic disorder. Probiotics are increasingly used as microbiota-targeted treatments, although their true clinical benefit remains uncertain. The objective of this study is to evaluate the efficacy and safety of probiotic supplementation compared with placebo in children with AP-DGBIs. PubMed, Embase, Web of Science and Scopus were

Design and analysis of randomized clinical trials for onchocerciasis, loiasis and mansonellosis: A systematic review.

BACKGROUND: The design and analysis of randomized clinical trials (RCTs) in filarial diseases such as onchocerciasis, loiasis, and mansonellosis pose unique statistical challenges, including skewed endpoints and limited sample sizes. This systematic review summarizes design and analysis approaches of RCTs conducted in these diseases with a focus on the statistical methodology. METHODS AND FINDINGS: A systematic search was conducted in PubMed and four trial registries to identify RCTs investigating treatments for onchocerciasis, loiasis, and mansonellosis published or registered between 2000 a

Pharmacological and interventional strategies for chronic Chagas cardiomyopathy: a systematic review of clinical outcomes.

BACKGROUND: Chronic Chagas Cardiomyopathy (CCC), the main chronic manifestation ofTrypanosoma cruzi infection, is a major global health burden causing heart failure, arrhythmias, and sudden death. Despite recommendations for conventional heart failure therapies, specific evidence for non-antiparasitic interventions in CCC is limited, creating significant clinical uncertainty. OBJECTIVES: This systematic review aimed to synthesize the available evidence from Randomized Controlled Trials (RCTs) regarding the efficacy and safety of non-antiparasitic pharmacological and device-based therapies in

Thymosin-ɑ1 for people with chronic hepatitis B.

RATIONALE: Chronic hepatitis B is a global public health concern. It is caused by infection with the hepatitis B virus (HBV). The goal of treating chronic HBV infection is to prevent progression to chronic hepatitis, cirrhosis, hepatic decompensation, liver failure, hepatocellular carcinoma, and death. Individual studies have evaluated various immunomodulatory therapies with inconsistent results. Thymosin-ɑ1 is known to have antiviral effects; however, results of randomised clinical trials on the effects of thymosin-α1 as a potential treatment for people with chronic HBV have been inconsistent

Pharmacological therapies for neglected tropical diseases: a systematic review and evidence gap mapping.

BACKGROUND: Neglected tropical diseases (NTDs) comprise 21 endemic infections that affect over one billion people worldwide. This study mapped and identified evidence gaps regarding pharmacological therapies for all WHO-recognized NTDs. METHODS: We systematically searched PubMed for systematic reviews (SRs) and randomized controlled trials (RCTs), and the ClinicalTrials.gov database for ongoing RCTs. Methodological quality was assessed using the AMSTAR-2 for SRs and the Cochrane Risk of Bias tool for RCTs. Data were synthesized into evidence and gap maps, and the distribution of evidence was

Comparative Efficacy and Safety of 15-Valent and 13-Valent Pneumococcal Conjugate Vaccines Alone and in Combination with the 23-Valent Polysaccharide Vaccine: A Systematic Review and Meta-analysis.

In addition to the 23-valent pneumococcal polysaccharide vaccine (PPSV23), the 13-valent and 15-valent pneumococcal conjugate vaccines (PCV13 and PCV15, respectively) are currently utilized as optional vaccines for adults. Although PCV15 provides broader serotype coverage than PCV13, comparative evidence regarding their immunogenicity, safety, and optimal clinical application remains limited. This systematic review and meta-analysis evaluated the immunogenicity and safety of PCV13 and PCV15, both as monotherapy and in combination with PPSV23. We conducted a comprehensive systematic search of P

Application of Causal Inference to Establish Assay Effect in the Absence of a Bridging Study: A Case Study of MenACWY-CRM Conjugate Vaccine Data.

During a vaccine development program, if the assay used to measure immunological endpoints is changed, ideally, a bridging study is performed to establish the relationship between results obtained with the new and previous assay. However, this is not always feasible, and when bridging study data are absent, this can limit the ability to use historical study information to strengthen evidence generated in the clinical program. We present a case study on GSK's quadrivalent meningococcal vaccine (MenACWY-CRM), where the immunogenicity assay was changed over time. A large amount of study data was

Extrapolating from trials to clinic: a predictive model defining the boundaries of benefit for multiple sclerosis therapies in real-world populations based on systematic review.

BACKGROUND: Clinical trials for multiple sclerosis (MS) disease-modifying treatments selectively enroll patients with favorable risk-benefit profiles. However, these therapies are often prescribed more broadly in clinical practice. We aimed to identify which patients are unlikely to benefit and may face substantial harm, and codify this into a data-driven framework for guiding real-world MS treatment decisions. METHODS: Systematic searches of PubMed and ClinicalTrials.gov identified 61 randomized, blinded phase 2b/3 trials with ≥ 100 adults per arm (all pediatric trials were included due to r

Treatment-Related Toxicities of Antibody-Drug Conjugates in Breast Cancer: A Bayesian Network Meta-Analysis.

IntroductionAntibody-drug conjugates (ADCs) are now widely used in breast cancer across multiple disease subtypes. With increasing clinical use, treatment-related toxicities have become an important factor in therapeutic decision-making. However, comparative safety data among ADCs are limited because direct head-to-head trials are lacking.MethodsWe conducted a systematic review and Bayesian network meta-analysis of randomized controlled trials evaluating ADCs in breast cancer, with the literature search updated through March 31, 2025. Treatment-related adverse events (TRAEs) and serious advers

Instrumental Variables in Randomized Trials.

AbstractMany randomized clinical trials fail to play out as intended: some participants assigned to the treatment group remain untreated, while others assigned to the control group cross over and receive treatment. In such settings, intention-to-treat analyses that compare participants by treatment assignment are diluted by noncompliance, while per-protocol analyses that compare participants by treatment received are contaminated by selection bias. Instrumental variables methods can address both problems. We explain the rationale for instrumental variables estimation in clinical trials and ill

Prophylactic drain placement versus non-drainage following gastric cancer surgery: A systematic review and meta-analysis of randomized controlled trials.

Gastric cancer remains a significant global health burden and a leading cause of cancer-related deaths. Surgical resection is the primary curative treatment, but postoperative complications can negatively impact outcomes. Prophylactic drainage (PD) has been widely used to reduce these complications by facilitating early detection and management of fluid collections. This study evaluates the role of PD following gastric cancer surgery through a systematic review and meta-analysis of randomized controlled trials (RCTs). We searched PubMed, Web of Science, Scopus, and Cochrane databases up to Jan

Evaluation of Control Arm Quality in Recent Radiation Oncology Randomized Clinical Trials.

BACKGROUND: For randomized controlled trials (RCTs) to provide the highest levels of evidence for clinical practice, it is ethically imperative for patients assigned to the control arm to receive standard-of-care treatment. Oncologic medical trials investigating new systemic agents have demonstrated a high proportion of RCTs with inadequate control arms. It is unknown whether this finding is prevalent in oncologic trials investigating radiation therapy (RT) for cancer. METHODS: ClinicalTrials.gov was queried for registered clinical trials investigating RT in patients with cancer from 2013 to
