There has been no case of simultaneous fracture of the trapezium and the proximal pole of the scaphoid. We have treated a case with this combination. The fracture of the proximal pole of the scaphoid was only found on computed tomography imaging. We performed accurate reduction and fixation of both fractures with cannulated headless compression screws in a minimally invasive technique. We obtained rigid initial fixation and good clinical and radiographical results.

Management of hand and wrist avascular necrosis with osteochondral fragmentation or focal arthritis can be a challenging problem with a variety procedures described for its treatment. Osteochondral autograft transplantation systems have been utilized in various focal defects of the knee, ankle, elbow, and wrist. The same principle for the treatment of focal defects of the proximal scaphoid, proximal capitate as well as metacarpal head as an alternative treatment is described. The main indication for this treatment is to address focal or partial osteochondral defects where the size of the defec

The authors describe the indications for radiologic explorations in case of traumatic as well as non-traumatic conditions. In case of trauma of the wrist, the radiologic exploration looks for a fracture, a dislocation or a ligament injury. Initially, postero-antérior and lateral views must be completed with an antero-posterior view, a scaphoid incidence and an oblique view. This initial examination can be completed secondarily by specific views for the carpal bones, dynamic X-Rays, CT scan or arthro CT scan. In non-traumatic cases, radiologic explorations look for osteo-articular or soft tissu

The relative length of the ulna compared to the radius, or ulnar variance, appears to be an important element in wrist pathology. The length is determined by age, genetic factors, load (i.e. gymnasts) and gross elbow pathology. The negative ulnar variance or short ulna has been associated with Kienböck's disease, avascular necrosis of the scaphoid and scapholunate dissociations. This could not be confirmed in our observations. On the contrary, a long ulna (positive ulnar variance) is harmful for the ulnar compartment of the wrist as it causes degeneration and perforation of the triangular fibr

Hyperextension of the wrist may induce underestimated intrinsic and extrinsic ligamentous tears. Undetected scaphoid fractures show the importance of accurate primary diagnostic. Evaluation of the anatomic and functional integrity of the ligaments of the wrist remains challenging. It requires great experience, but in any case, radiology remains the basic diagnostic tool used by both the general practitioner and the specialist. Only the use of reproducible imaging techniques will allow for comparison between healthy and pathological side as well as for follow-up. Every one should master and use

Various pathologies of the adult carpus result in clinical scenarios where excision can be considered and even recommended. In the appropriate patient population, isolated carpal excision can alleviate pain and improve mobility. Excisions of the pisiform, trapezium, and trapezoid have abundant literature evidence to support positive long-term functional outcomes. In contrast, isolated excision of the capitate, hamate, and triquetrum has limited support in the literature secondary to compromise of carpal mechanics and lead to recurrent pain. Additionally, isolated scaphoid and lunate excision a

The medicinal treatment of osteonecrosis is described as part of the conservative treatment for atraumatic femoral head necrosis of the hip in adults and for Ahlbäck's disease of the knee joint. For femoral head necrosis, iloprost is used in the early stages (ARCO I and II) as a treatment attempt to eliminate pain and the accompanying bone marrow edema. This also applies to the use of bisphosphonates, whereby preferably small defects (< 30%) are suitable for treatment. Bisphosphonates are successfully used in the knee joint to treat Ahlbäck's disease, although the results are sometimes contrad

Carpal perilunar dislocations are hidden lesions with uncertain outcome and treatment which is not yet standardized. This retrospective study including 39 cases of carpal perilunar dislocations helps clarify these issues. For fresh dislocations, the diagnosis was established in 66%. Surgical treatment was performed in 26 cases. Anatomic results showed nonunion, necrosis and residual instability. Function was always decreased and was worse in open forms or after orthopedic treatment. Scaphoid screwing results were better than those from pinning. Lateral wrist x rays must always be ordered in ca

Six cases of thenar muscle aplasia are reported. Aplasia of the abductor pollicis brevis and opponens pollicis muscles was noted in all cases. The flexor pollicis brevis muscle was inconstantly involved and the abductor pollicis muscle was normal. The aplasia was unilateral in five cases and bilateral in one. Bone dysplasia of the scaphoid, first metacarpal and thumb was associated with muscle aplasia. Arterial dysplasia was also observed in four cases. In all cases electrophysiological examination showed that the thenar branch of the median nerve was not involved. Electromyography showed no s

Consideration of the individual carpal bone characteristics of the wrist plays a key role in well-functioning biomedical devices and successful surgical procedures. Although geometric differences and individual bone sizes have been analyzed in the literature, detailed morphologic descriptions and correlations covering the entire wrist reported in a clinical context are lacking. This study aimed to perform a comprehensive and automatic analysis of the wrist morphology using the freely available "Open Source Carpal Database" (OSCD). We quantified the size of each of the individual carpal bones a

The forearm skeleton is composed of two bones: the radius and the ulna. This is closely related to manipulative movements. The ulna is part of the elbow joint, whereas the radius and ulna together with the scaphoid and lunate bones, form the wrist joints. Thus, morphofunctional analysis of the adult Sima de los Huesos (SH) forearm bones, provides clues about manipulative activities in one Pleistocene population. From 1976 to the present, over 7000 human fossils have been recovered from the SH site. The radial sample comprised 98 labeled fragments, of which 49 belonged to adult individuals, rep

PURPOSE: The study evaluated the efficacy of SC arthrodesis with lunate preservation for treating patients diagnosed with stage IIIB or IIIC Kienböck's disease, who also exhibit neutral ulnar variance. The study further aimed to explore potential variations in outcomes between patients diagnosed with stage IIIB and IIIC Kienböck's disease.
METHODS: Thirty-two patients diagnosed with stage IIIB (n = 19) and stage IIIC (n = 13) Kienböck's disease underwent SC arthrodesis with distal radius bone grafting stabilised by Herbert compression screws. All participants underwent pre- and post-operative

BACKGROUND: Scaphocapitate syndrome is a rare variety of perilunate instability, described as a trans-scaphoid, trans-capitate fracture, with rotation of the capitate head either 90° or 180°.
METHODS: We present a unique case of scaphocapitate syndrome in which the rotated proximal capitate fragment expelled into the carpal canal.
RESULTS: The capitate head was extricated from the carpal tunnel via the volar approach, and was anatomically aligned and fixed through the dorsal approach using two 2.0 mm headless compression screws. The scaphoid fracture was then also reduced through the dorsal

Treatment of nondisplaced scaphoid waist fractures has evolved from conventional cast immobilization to percutaneous screw insertion. This study assessed clinical and radiologic outcomes of volar percutaneous screw fixation for 15 type B2 scaphoid fractures (according to the Herbert and Fisher classification). All patients were followed for an average of 33 months (range, 6-50 months). All fractures achieved radiographic union at an average of 57 days (range, 35-70 days), requiring no additional procedures. None of the patients showed radiographic signs of osteoarthritis, osteonecrosis of the

PURPOSE: After a distal scaphoid excision, most wrists develop a mild form of carpal instability-nondissociative with dorsal intercalated segment instability. Substantial dysfunctional malalignment is only occasionally seen. We hypothesized that distal scaphoid excision would lead to carpal instability-nondissociative with dorsal intercalated segment instability in cadavers and that the dorsal intercarpal (DIC) ligament plays a role in preventing such complications.
METHODS: We used 10 cadaver upper extremities in this experiment. A customized jig was used to load the wrist with 98 N. Motion

The development of innovative small bone replacements for the human wrist has been partially limited by the lack of a suitable preclinical animal model. This study explores the feasibility of using the Yucatan minipig (YMP) as a preclinical model for small bone replacement. Implants for the radial carpal bone (RCB), homologous to the human scaphoid, were developed for a pilot in vivo animal study. RCB size (volume, bounding box dimensions) was quantified (n = 35), and relationships between animal age, weight, and RCB volume were investigated. Bounding box dimensions were also analyzed relative

BACKGROUND: Despite being an effective analgesic for children with fractures, some clinicians may avoid prescribing ibuprofen due to its potentially harmful effect on bone healing.
OBJECTIVE: To determine if exposure to ibuprofen is associated with an increased risk of bone healing complications in children with fractures.
METHODS: We performed a retrospective study of children aged 6 months to 17 years who presented to the pediatric emergency department (PED) with a fracture of the tibia, femur, humerus, scaphoid, or fifth metatarsus and who followed up with the orthopedic service. We chose
