Scapulothoracic dissociation: An emerging high-energy trauma in medical literature.

Scapulothoracic dissociation (STD) is a devastating consequence of high-energy trauma sustained by the shoulder girdle that can easily result in rapid mortality. Since described by Oreck et al. in 1984, STD has been reported in a handful of journals and individual case series, though is still considered a rare occurrence in the context of shoulder injuries. In this report, we examine the case of a 25-year-old female involved in a high-speed rollover auto accident. Unique to this case was the discovery of a completely transected axillary artery and vein with intracorporeal bleeding and complete…

Spinal accessory nerve palsy: an unusual complication of coronary artery bypass.

The neurologic complications of coronary artery bypass surgery have been well documented, with a reported incidence of 61% in one large study. Most injuries to the peripheral nervous system involve the brachial plexus. We report the first case of a spinal accessory nerve lesion after coronary bypass surgery. The patient presented with progressive right shoulder weakness. Electrodiagnostic studies revealed a partial lesion of the right spinal accessory nerve. Physical therapy, including strengthening, range of motion, and electric stimulation to the right shoulder, was prescribed to assist reco

Postoperative stiffness and rotator cuff tendon healing: a narrative review.

Range of motion restriction or shoulder stiffness is common in rotator cuff tear patients. Shoulder stiffness occurs both preoperatively and postoperatively and contributes to patient dissatisfaction. However, recent studies have suggested that shoulder stiffness may be associated with a greater healing response and lower retear rate. Elevated levels of cytokines such as interleukin (IL)-6, IL-1β, and IL-8 in the joint fluid, synovial tissue, and subacromial bursa of patients with shoulder stiffness suggests an increased healing response following tendon injury. Understanding the mechanisms un

[Macrosomic infants: clinical problems at birth and afterward].

Macrosomic infants (birth weight >4000 g) show increased risk for shoulder dystocia and associated injuries, hypoglycemia and respiratory distress. Higher risk is directly related to neonatal birth weight. High birth weight is also associated with increased risk of developing metabolic syndrome later in life. However the relation between birth weight and later-life metabolic syndrome in not linear, but "U" shaped.

Winged scapula in the emergency department: a case report and review.

Winged scapula is a well-described condition associated with a wide variety of injuries and conditions. It is typically due to dysfunction of the serratus anterior or trapezius muscles, often secondary to long thoracic or accessory nerve palsies. Given the potential for permanent disability, winged scapula is an important diagnosis to consider in any patient presenting with shoulder pain or weakness.

Shoulder Surgery Disasters: How to Manage Complications and Failures After Arthroscopic and Open Shoulder Surgery.

It is important to discuss the optimal treatment options for the common complications after arthroscopic and open shoulder surgery based on both personal experience and evidence available in the literature. Discussion of complications after shoulder stabilization, rotator cuff repair, proximal humerus fracture fixation, coracoclavicular ligament repair, and shoulder arthroplasty can highlight both the optimal treatment pathway and technical tricks along with postoperative outcomes. The orthopaedic surgeon should be informed about the decision-making process for the management of common complic

Rehabilitation and Return to Sport following Operative and Nonoperative Treatment of Anterior Shoulder Instability.

There has been growing interest in the rehabilitation process and timing of returning an athlete to sport following the management options for anterior shoulder instability. The purpose of this article is to review the current rehabilitation and return to sport (RTS) protocols for various nonoperative and operative management strategies following anterior shoulder instability events. When appropriate in the rehabilitation protocol, RTS testing should be criteria based, rather than time based, with a special focus given to psychological readiness in order to promote successful return to athleti

Effect of two types of shoulder prosthesis on the muscle forces using a generic multibody model for different arm motions.

BACKGROUND: This study aims to analyze the effects of a novel dual-bearing shoulder prosthesis and a conventional reverse shoulder prosthesis on the deltoid and rotator cuff muscle forces for four different arm motions. The dual-bearing prosthesis is a glenoid-sparing joint replacement with a moving center of rotation. It has been developed to treat rotator cuff arthropathy, providing an increased post-operative functionality. METHODS: A three-dimensional musculoskeletal OpenSim® model of an upper body, incorporating a natural gleno-humeral joint and a scapula-thoracic joint developed by Blan

Correlation between full-thickness degenerative supraspinatus tear and radiographic parameters including the acromiohumeral centre edge angle and the greater tuberosity angle.

BACKGROUND: Many radiographic parameters associated with the extrinsic cause of supraspinatus tears have been proposed. The aim of this study was to assess the relationship between a full-thickness degenerative supraspinatus tear (FTDST) and the patient's radiographic parameters, including the acromiohumeral centre edge angle (ACEA) and the greater tuberosity angle (GTA). METHODS: A retrospective study was conducted. We included 116 patients who underwent shoulder arthroscopic surgery at our institute. The case group included FTDST patients, whereas the control group also included patients wi

Tender point examination with palpation in different shoulder pathologies: A retrospective study.

BACKGROUND: Shoulder pain is a common symptom for non-traumatic shoulder pathologies and affects 4.7-66.7% of entire population. Even with the latest technological advantages, palpation still is a cost and time efficient tool to evaluate the shoulder pathologies. Our aim was to investigate and compare palpation findings in different shoulder pathologies. METHOD: We used first assessments of impingement syndrome (IS) (n = 205), rotator cuff tear (RCT) (n = 185) and frozen shoulder (FS) (n = 210) patients who received treatment between 2010 and 2019 years. Editorial Commentary: Biomechanical Investigation of Superior Capsule Reconstruction Requires Meticulous Methods.

One of the treatment options for a massive irreparable rotator cuff tear is superior capsule reconstruction (SCR). Biomechanically, SCR depresses and centers the humeral head on the glenoid so that the remaining rotator cuff muscles and the deltoid can provide function. Therefore, in SCR, graft characteristics such as graft type, graft thickness, and graft tension are the critical parameters contributing to the biomechanical effectiveness. SCR was originally developed with fascia lata autografts, but in the United States, the most popular graft used is acellular human dermal allograft. Both ha

Traumatic Instability: Treatment Options and Considerations for Recurrent Posttraumatic Instability.

Recurrent traumatic glenohumeral instability is a complex problem with multiple variables to consider, but patient demographics, activities, as well as clinical and radiographic findings provide significant information to help choose the best treatment option. Although nonoperative treatment is a viable option for primary glenohumeral instability and in-season instability, recurrent instability exhibits anatomic factors which render nonsurgical treatment limited in scope. A proper patient history, clinical examination, and standard and advanced imaging are necessary in the assessment of patien

Magnetic resonance arthrography of the labral-ligamentous complex of the shoulder: an update.

Magnetic resonance (MR) arthrography is increasingly replacing conventional MR imaging for many clinical indications in the shoulder. It is particularly useful in providing additional information about structures which form the labral-ligamentous complex, an important stabilizer of the shoulder. These structures are the fibrocartilaginous glenoid labrum, the glenohumeral ligaments, and the long head of the biceps tendon. The MR arthrographic technique is reviewed, together with anatomical considerations, including normal variants, relating to the labral-ligamentous complex. Lesions such as gle
