A comparison of elective mutism and emotional disorders in children.

The case notes of 24 children diagnosed as elective mutes were compared with those of 24 matched controls with diagnosed emotional disorders. One-third of the elective mutes (but none of the controls) had experienced delayed development of speech or difficulties in articulation. All the elective mutes lived in two-parent families, but marital discord was noted in half the families in both groups. Compared with the controls, the mute children were more often described as 'anxious', 'depressed' and 'manipulative', and their mothers were characterised as 'over-protective', and tending to 'spoil'

[Clinical features of sex-linked recessive mental retardation with fragile X-chromosome].

The clinical description of 28 boys with recessive sex-linked mental retardation with an X-chromosome fracture is presented. The peculiarities of the relevant somatic and electrophysiological disturbances have been identified which in some cases make it possible to diagnose and in the remaining patients at least to suspect this disease before application of the cytogenetic methods of investigation. The authors also describe the characteristic psychopathological symptomatology manifested in peculiar schizophreniform disorders.

Agenesis of the corpus callosum: report of two pre-school children and review of the literature.

The results of neuropsychological examinations of two patients with agenesis of the corpus callosum showed low intelligence-test performance, impared visuo-motor co-ordination and impaired bimanual co-ordination. The most striking deficits were observed in Case A, particularly in her inability to identify objects verbally when placed in the non-dominant hand, and in her ability to complete a task with the non-dominant hand without vision, when she was unable to complete the same task with vision. This patient is unusual insofar as she manifests deficits noted in acallosal patients, as well as

The effect of reward on listening effort as reflected by the pupil dilation response.

Listening to speech in noise can be effortful but when motivated people seem to be more persevering. Previous research showed effects of monetary reward on autonomic responses like cardiovascular reactivity and pupil dilation while participants processed auditory information. The current study examined the effects of monetary reward on the processing of speech in noise and related listening effort as reflected by the pupil dilation response. Twenty-four participants (median age 21 yrs) performed two speech reception threshold (SRT) tasks, one tracking 50% correct (hard) and one tracking 85% co

Amplification of transcutaneous and percutaneous bone-conduction devices with a test-band in an induced model of conductive hearing loss.

OBJECTIVE: Transcutaneous devices have a disadvantage, the dampening effect by soft tissue between the bone and devices. We investigated hearing outcomes with percutaneous and transcutaneous devices using test-bands in an induced unilateral conductive hearing loss. DESIGN: Comparison of hearing outcomes of two devices in the same individuals. STUDY SAMPLE: The right ear was plugged in 30 subjects and a test-band with devices (Cochlear™ Baha® BP110 Power and Sophono® Alpha-2 MPO™) was applied on the right mastoid tip with the left ear masked. Facilitating communication among speech pathologists treating children with cleft palate.

An interactive web-based system was designed to facilitate communication between nonspecialist speech pathologists who provide therapy for individuals with speech disorders associated with cleft palate or craniofacial anomalies and specialist speech pathologists who provide physiologically based assessments of speech production. The web site includes instructional presentations, streaming video clips of endoscopic examinations, and exchange of information about the nature of therapy as recommended by the specialist and as provided by the nonspecialist. The approach demonstrates use of web-base

Dynamic neural oscillations underpin audiovisual gain from visual lip cues in speech noise.

Speech noise substantially impairs speech perception, whereas congruent lip movements provide effective visual support when auditory input is degraded. Using natural continuous Mandarin sentences, the present study recorded behavioral and electroencephalographic (EEG) data under speech noise conditions with different signal-to-noise ratios (SNR) to examine the dynamic neural mechanisms by which visual lip information facilitates speech perception. The results showed that speech noise significantly reduced speech recognition accuracy and weakened δ band activity and functional connectivity in t

Sound spectrogram analysis in patients receiving Kawahara's surgical voice restoration for advanced carcinoma of the hypopharynx and cervical esophagus.

Loss of voice after total laryngectomy is a major concern that has prompted much effort to develop methods of surgical voice restoration. In 1992, Kawahara described a tracheocolic shunt for voice restoration using a revascularized ileocolic graft. With this method, aspiration through the phonatory shunt is prevented by the ileocolic valve (Bauhin's valve), while vocalization is allowed. The purpose of this study was to use sound spectrogram analysis to evaluate voices surgically restored in that way. Between 2002 and 2005, 10 consecutive patients underwent laryngopharyngoesophagectomy with Ka

Children's performance in complex listening conditions: effects of hearing loss and digital noise reduction.

PURPOSE: To determine the effect of hearing loss (HL) on children's performance for an auditory task under demanding listening conditions and to determine the effect of digital noise reduction (DNR) on that performance. METHOD: Fifty children with normal hearing (NH) and 30 children with HL (8-12 years of age) categorized words in the presence of auditory or visual competitors, or both. Stimuli were presented at 50 dB SPL at a 0-dB signal-to-noise ratio. Children with HL were fitted with behind-the-ear hearing aids that had DNR technology. When DNR was activated, output decreased 4 dB, and si

Longitudinal effects of Alzheimer disease on discourse production.

As part of a 5-year study of the longitudinal effects of Alzheimer disease (AD) on language, discourse samples from three patients diagnosed with AD and three matched normal controls were analyzed. A picture-description task was used to elicit comparable samples of discourse. Subjects were asked to describe the "Easter Morning" picture by Norman Rockwell. Samples were tape recorded, transcribed, and scored using eight measures of discourse production. These measures successfully quantified the changes in quantity and quality observed in the discourse of AD patients over time. Results revealed

Postoperative and follow-up studies of spastic dysphonia patients treated by recurrent laryngeal nerve section.

Long-term results of surgical treatment of spastic dysphonia by RLN section are evaluated by means of perceptual psychophysical scaling and by the patients themselves. It is found that a significant reduction in spastic dysphonia symptoms occurs as a result of surgery, and that these results are maintained after long-term follow-up in the majority of patients. These experimental observations are in agreement with the subjective assessment of postsurgical communication by the patients involved in the study.

Auditory Distraction and Acclimatization to Hearing Aids.

OBJECTIVE: It is widely recognized by hearing aid users and audiologists that a period of auditory acclimatization and adjustment is needed for new users to become accustomed to their devices. The aim of the present study was to test the idea that auditory acclimatization and adjustment to hearing aids involves a process of learning to "tune out" newly audible but undesirable sounds, which are described by new hearing aid users as annoying and distracting. It was hypothesized that (1) speech recognition thresholds in noise would improve over time for new hearing aid users, (2) distractibility

Deciphering the genetic basis of developmental language disorder in children without intellectual disability, autism or apraxia of speech.

BACKGROUND: Developmental language disorder (DLD) refers to children who present with language difficulties that are not due to a known biomedical condition or associated with autism spectrum disorder (ASD) or intellectual disability (ID). The clinical heterogeneity of language disorders, the frequent presence of comorbidities, and the inconsistent terminology used over the years have impeded both research and clinical practice. Identifying sub-groups of children (i.e. DLD cases without childhood apraxia of speech (CAS)) with language difficulties is essential for elucidating the underlying ge

Impact of stimulus-related factors and hearing impairment on listening effort as indicated by pupil dilation.

Previous research has reported effects of masker type and signal-to-noise ratio (SNR) on listening effort, as indicated by the peak pupil dilation (PPD) relative to baseline during speech recognition. At about 50% correct sentence recognition performance, increasing SNRs generally results in declining PPDs, indicating reduced effort. However, the decline in PPD over SNRs has been observed to be less pronounced for hearing-impaired (HI) compared to normal-hearing (NH) listeners. The presence of a competing talker during speech recognition generally resulted in larger PPDs as compared to the pre

Acoustic-temporal aspects of stop-plosives in the speech of Persian-speaking children with cleft lip and palate.

PURPOSE: The main objective was to examine temporal parameters of stop-plosives in Persian-speaking children with repaired cleft lip and palate (CLP). METHOD: Eleven children with repaired bilateral CLP and 20 typically-developing children participated in the study. Stop-gap duration (SGD) and voice-onset time (VOT) were measured based on digital waveform and spectrographic displays. RESULT: Separate linear mixed model analyses showed significantly longer SGDs for children with CLP for all plosives in word-mid and final positions. Furthermore, children with CLP tend to produce longer VOTs fo

Effect of variations in hearing-aid frequency response on real-life functional performance of children with severe or profound hearing loss.

This study examined the effect of variations in hearing-aid frequency response on real-life functional performance of children with severe to profound hearing loss. A cross-over design was used in a double-blind comparison of the NAL prescription with alternatives that produced either a BOOST or a CUT (6 dB/octave from 0.5 to 2 kHz), relative to the NAL response. The functional performance of 30 children (aged 7 months to 16 years) when wearing hearing aids adjusted to each response over two to four weeks was assessed by using parents' and teachers' observations (PEACH and TEACH scales). Speech perception in tinnitus is related to individual distress level - A neurophysiological study.

Individuals suffering from tinnitus often complain about difficulties understanding speech in noisy environments even in the absence of a peripheral hearing loss. This EEG study aimed to investigate whether aspects of phonetic perception are affected by the experience of tinnitus. We examined a sample of individuals with chronic, subjective tinnitus (n = 30, age range 30-50 yrs.), who underwent behavioural screening (standard tinnitus questionnaires) and comprehensive audiometric testing that covered peripheral and central hearing abilities (pure tone audiometry, suprathreshold audiometry (fre

A knowledge-driven framework for surgical safety check integration using speech recognition and speaker verification.

BACKGROUND: The WHO Surgical Safety Checklist reduces preventable errors, but real operating rooms are characterized by overlapping speech, high ambient noise, similar vocalizations, uncertain speaker identity, and occasional omission of checklist items. ASR-only systems that ignore identity constraints and semantic-temporal structure are therefore prone to misrecognition and incorrect verification. METHOD: We design an integrated verification framework that couples automatic speech recognition and speaker verification (ASR + SV) with a knowledge-driven rule engine derived from the WHO checkl

Cleft Lip and Palate: Overview of Causes, Surgical Repair, and Treatment Approaches: A Narrative Review.

Cleft lip and palate is one of the most common congenital malformations of the skull and face (craniofacial). The deformity is characterized by a loss of integrity of the lip muscles, alveolar bone, and hard and soft palate. The severity of the deformity can range from a small hole in the lip to a large fissure extending into the roof of the mouth and nose. Management and control of cleft patients is a challenge. Intervention in these patients begins very early in life, from the fetal period, and continues into adulthood, and the patient's relatives are always involved in the treatment process
