Liver sinusoidal endothelial cells constitute a major route for hemoglobin clearance.

Mild rupture of aged erythrocytes occurs in the spleen, resulting in hemoglobin (Hb) release, whereas pathological hemolysis characterizes several diseases. Hb detoxification is attributed to macrophages, but other routes of Hb clearance remain elusive. Here, we uncover that Hb uptake is chiefly executed by liver sinusoidal endothelial cells (LSECs) via macropinocytosis. Consistently, LSECs display proteomic signatures indicative of heme catabolism, ferritin iron storage, antioxidant defense, and macropinocytic capacity, alongside high iron content and expression of the iron exporter ferroport

Pregnancy With Massive Splenomegaly and Pancytopenia: A Case Report.

Massive splenomegaly complicating pregnancy is a rare clinical entity that poses special difficulties, such as anemia, thrombocytopenia, asc … megaly during pregnancy, including the choice of delivery method. Significant complications include splenic rupture and bleeding, particularly when pancytopenia is present. The need for several transfusions, the potential side effects of transfusion therapy, and factors related to the origin of splenomegaly when assessing maternal-fetal outcomes are discussed in …

Trigeminal Autonomic Cephalalgias Manifested As The Only Initial Symptom Of Ehlers-Danlos Syndrome Type IV.

Ehlers-Danlos syndrome (EDS) type IV is characterized by thin skin with visible veins, easy bruising, characteristic facial features, arterial and digestive complications, as well as rupture of the gravid uterus. It has never been previously reported that trigeminal autonomic cephalalgias (TACs) could manifest as the only initial symptom of EDS type IV. Here, we report a case of a 27-year-old man who presented atypical headache like TACs stimulated by right internal carotid artery dissection. About one month after his discharge, he suffered dissection of the right renal artery and splenic arte

Adult polycystic kidney disease: a disorder of connective tissue?

Adult polycystic kidney disease (APCKD) is one of the most common serious inherited disorders. Many affected patients succumb to the renal and non-renal manifestations of this autosomal dominant disease. The disease is characterized by cyst formation in several organs, most obvious of which is in the renal parenchyma. Other features associated with the disease include hepatic fibrosis, hepatic, pancreatic and splenic cyst formation, Berry aneurysms, colonic diverticulae, hernias and cardiac valvular disorders. Rupture of Berry aneurysm is a sudden and often fatal manifestation in some unsuspec

Hydatidemesis: a bizarre presentation of abdominal hydatidosis.

A 31 year old male presented with high grade fever and abdominal pain of 20 days duration. At the age of 9 he had been operated on for a solitary retroperitoneal hydatid cyst and had been asymptomatic until the age of 21 when he sustained a blunt injury to the abdomen. An exploratory laparotomy for splenic rupture revealed multiple intra-abdominal hydatid cysts, which were removed. The patient remained well until the present episode. An ultrasound examination revealed multiple intra-abdominal hydatid cysts. Seven days after admission, the patient developed hydatidemesis (hydatid cysts and memb…

Rupture of an accessory spleen caused by blunt trauma.

PURPOSE: The accessory spleen is quite a common abdominal anomaly. However, the traumatic accessory spleen rupture is an extremely rare condition requiring surgical intervention, even laparotomy. 9 cases of traumatic accessory spleen were found published between 1962 and 2022. The study aims to evaluate traumatic accessory spleen rupture cases regarding their causes, clinical course, and possible diagnosis without surgery and treatment. METHODS: Desk research method using available online databases. Descriptive methods were employed to analyze the collected data. The results are summarized in

Ruptured primary mucinous cystadenoma of spleen leading to mucinous ascites.

Mucinous cystadenoma are rare benign mucin-producing tumours. Primary mucinous neoplasms most commonly involve ovary, pancreas or appendix. They may rupture to produce mucous ascites/pseudomyxoma peritonei. We are describing a 51-year-old man who presented to us with symptomatic gallstone disease but during preoperative ultrasound was found to have cystic lesions in spleen with free fluid in the abdomen. Splenectomy and drainage of mucin were done. Histopathology was done and a final diagnosis of primary mucinous cystadenoma of spleen with mucinous ascites was made.

[Atrio-caval shunt in injury to the suprahepatic vena cava. Apropos of a case and review of the literature].

Mortality in cases of trauma-induced lesions of the suprahepatic vena cava is high because it is difficult to expose the lesions, and consequently to control blood loss. Several techniques have been reported for treating this type of lesion. We report a case in which both suprahepatic veins were sutured after installing an atrio-cava shunt via a sternotomy. The limitations of this technique and other surgical possibilities are also discussed.

Takayasu Arteritis Complicated with Vertebral Artery Dissection Aneurysm Treated Endovascularly: Report of One Case.

Takayasu arteritis (TA) is a chronic granulomatous large-vessel vasculitis that primarily affects the aorta and its major branches, such as the subclavian, carotid, and renal arteries. Pathologically, it is characterized by transmural infiltration of lymphocytes and macrophages with granulomatous inflammation, leading to intimal fibrosis and thickening, destruction of medial elastic fibers, and subsequent luminal stenosis or occlusion. Aneurysmal dilation may occur in some cases. We here report a case of TA complicated by a vertebral artery dissecting aneurysm. Our patient was a 47-year-old ma

Vascular Involvement of Immunoglobulin G4-Related Disease: A Case Report.

Immunoglobulin-G4-related disease (IgG4-RD) is a fibro-inflammatory condition that can impact any organs/tissues, including the vascular systems, resulting in aortitis/periaortitis/periarteritis (PAO/PA). The complex nature of this disease and limited understanding have led to potential delays in identifying and managing irreversible organ damage. Herein, we report a 17-year-old female with hyper IgG4 disease, sclerosing mesenteritis, short stature, and insulin resistance who presented with symptoms of fever, epigastric pain, left flank pain, vomiting, dizziness, decreased urine output, and di

Dengue Infections and the Surgical Patient.

Dengue infections are increasing globally and account for significant morbidity and mortality. Severe dengue results in microvascular changes and coagulopathy that may make surgical intervention risky and the overall surgical management challenging. We outline the potential surgical manifestations and complications following dengue infections and describe the clinical, pathogenetic, diagnostic, and treatment aspects of dengue and surgical patients. The main surgical presentations were acute cholecystitis, acute pancreatitis, acute appendicitis, splenic rupture, bowel perforation, gastrointesti…

[Multiple cerebral hydatic cysts of cardiac origin. A case report].

The hydatic cyst is a pathology observed in developing countries. Multiple cerebral localizations with a cardiac origin are exceptional and are sometimes diagnosed only after onset of complications. We present the case of a 22-year-old male student: the diagnosis of multiple cerebral hydatic cysts caused by rupture of a cardiac hydatic cyst was established after an episode of ischemia of the limbs with cerebral hemorrhage induced by heparin. One year later, the clinical situation consisted in intracranial hypertension, hemiplegia and convulsive seizures. We observed no cardiac symptoms. The br

Extended Laparoscopic Central Pancreatectomy with Clamping of the Mesentericoportal Vein and Resection of the Splenic Vessels for a Large Solid Pseudopapillary Tumor.

BACKGROUND: Solid pseudopapillary tumors (SPPTs) are low malignant potential entities found mainly in young females.1,2 Pancreatectomy without tumor rupture is the treatment of choice, and the laparoscopic approach is indicated.3,4 Limited pancreatectomy is possible due to the low risk of malignancy (< 10%) based on the low risk of lymph node invasion or true vascular invasion.1,2 Centrally located large SPPTs can be treated by extended central pancreatectomy with or without vascular resection to avoid pancreatoduodenectomy or distal pancreatectomy. METHODS: A 24-year-old woman was admitted w
