Recently we achieved good outcomes in pediatric renal transplantation under basiliximab induction steroid withdrawal regimen including ciclosporin and mycophenolate mofetil (1 and 2 years patient/graft survival rates; 100%/96.6%). Steroid was withdrawn in 3 children and was given alternatively in 18 among 30 children under this regimen. Height standard deviation scores were significantly improved 1 year post-transplantation (-1.7+/-0.26 (mean+/-SE)), compared with those at transplantation (-2.1+/-0.23 (mean+/-SE)). Viral infection is still one of serious complications. Children tend to have sy

Indications, procedures, complications, pharmacokinetics and outcomes of renal transplantation are different in children and in adults. Subjects <18 yrs old, are often included in a unique list as in Italy, benefiting from donors <15 yrs old, and the waiting time is reduced to <12 months in 71% of cases. The risk of thrombosis limits the use of donors <2 yrs and trans-plantation in infants <1 yr. The age at kidney transplantation is <5 yrs in 20-30% of children. In Italy living-related trans-plantation (LRT) is performed in 7% of cases, while in the USA it is more common (57%) and is often pre

Liver cell transplantation is an emerging procedure, consisting of infusing mature adult hepatocytes in the portal system of the recipient. It aims to correct inborn errors of liver metabolism, bridge unstable patients to transplantation, or even allow bridge to recovery in fulminant liver failure. The technique addresses ideally patients with inborn errors of metabolism, unstable but not sick enough for orthotopic transplantation. Best results have so far been obtained in metabolic diseases, such as urea cycle disorders, glycogenosis type I, Crigler Najjar, Refsum disease and factor VII defic

Intestinal transplantation has been established as a treatment option for patients that suffer from intestinal failure with complications from total parenteral nutrition. It is still rapidly evolving and just reached a landmark of 1,000 cases worldwide. Intestinal allografts can be transplanted as isolated, combined with the liver or as a part of a multivisceral allograft. Tacrolimus-based immunosuppression regimens have been used universally with improved outcomes. Clinical outcome in intestinal transplantation has improved significantly over time, impacted by refinement of surgical technique

BACKGROUND: The human corneal endothelium has a limited proliferative capacity in vivo. Until now it has only been possible to replace damaged endothelium by transplantation of a donor cornea. After establishing methods for the isolation and in vitro cultivation of human corneal endothelial cells (HCEC), transplantation of these cells may be an alternative therapeutic option.
MATERIALS AND METHODS: In this review methods for the in vitro cultivation of HCEC and their transplantation onto the Descemet membrane of donor corneas are described.
RESULTS: In vitro proliferation of human adult corn

Studies in experimental models (1953-1956) demonstrated that acquired donor-specific allotolerance in immunologically immature or irradiated animals is strongly associated with donor leukocyte chimerism. Bone marrow transplantation in immune-deficient or cytoablated human recipients was a logical extension (1968). In contrast, clinical (1959) and then experimental organ transplantation was systematically accomplished in the apparent absence of leukocyte chimerism. Consequently, it was assumed for many years that success with organ and bone marrow transplantation involved fundamentally differen

Renal transplantation is the treatment of choice for most patients with end-stage renal disease. The shortage of donor organs, however, remains a major obstacle to successful, early transplantation. This shortage has actually worsened despite an increase in living family-related and unrelated donors. On the other hand, over the last 10 years, allograft and recipient survival have significantly improved. This encouraging outcome reflects many factors, particularly a favorable shift in the balance between the efficacy and toxicity of immunosuppressive regimens. As acute rejection and early graft

Ovarian dysfunction, anovulatory vaginal bleeding, amenorrhea, high prolactin levels, and loss of libido are the causes of infertility in women with chronic renal failure. After renal transplantation, endocrine function generally improves after recovery of renal function. In this study we retrospectively evaluated the prepregnancy and postdelivery renal function, outcome of gestation, as well as maternal and fetal complications for eight pregnancies in eight renal transplant recipients between November 1975 and March 2003 of 1095 among 1425. Eight planned pregnancies occurred at a mean of 3.6

Rejection is the leading cause of corneal graft failure, induced by loss of the so-called eye immune privilege. Prevention of graft rejection with immunosuppressive therapy is then necessary. Topical corticosteroids are currently the gold standard, and steroids are the only treatment for acute rejection episodes. Steroids are nonspecific immunosuppressive agents, and they can induce glaucoma, cataract, infections, and epithelial defects. Cyclosporin has a specific effect, because it inhibits interleukin-2 transcription and, consequently, the specific activation of T lymphocytes. When cyclospor

Renal transplantation is the best treatment option for chronic renal failure, with marked improvement in social activity, work, and family life. In addition to these obvious improvements, it is an extremely cost-effective procedure when successful. Ultrasonography plays a major role in the imaging of these patients, and ultrasound (including color Doppler) is helpful to the transplant physician in detecting graft dysfunction and peritransplant collections, some of which may be drained under ultrasound guidance. It is also helpful in the diagnosis of chronic vascular complications including tra

Osteoporosis, renal osteodystrophy and osteoarthropathy derived from hemodialysis associated amyloidosis are three major bone diseases after the kidney transplantation. Osteoporosis that is induced by steroid administration is an inevitable side effect and develops more frequently in the elderly recipients. After the recovery of renal function due to grafted kidney, secondary hyperparathyroidism usually improves and the serum level of PTH-intact downs to the almost normal range. However, the patients who have tertiary hyperparathyroidism are unable to show the enough lowering of PTH-intact. In

Follicular lymphoma and mantle cell lymphoma are incurable with standard chemotherapy regimens. One approach to improve outcome in patients with these diseases is high-dose therapy and autologous stem cell transplantation. Rituximab, an anti-CD20 monoclonal antibody, is specific for the B-cell surface antigen and can be used in autologous stem cell transplantation to eliminate lymphoma cells before the harvest (in vivo purging) or to prevent regrowth of malignant cells following transplant (post-transplant therapy). Preliminary data from an ongoing multicenter study evaluating the safety and e

Autoimmune hepatitis (AIH) is a good indication for liver transplantation, with 5- and 10-year survival rates approaching 75%. Determining the timing for transplantation remains difficult because progression to end-stage disease may be difficult to predict. After transplantation, the patients are at risk of recurrent AIH. This syndrome is not well-characterized and requires clearer definition. Introduction of corticosteroids is not always associated with arrest of disease. De novo AIH may also develop in the allograft. Although there is usually a good response to the reintroduction of corticos

Greater experience and improved outcomes in liver transplantation have necessarily led to longer waiting lists against a constantly limited donor pool. Split liver transplantation has been conceived as a means to increase the supply of liver grafts. The bipartition of a whole liver provides grafts for two recipients in a complex operation with equally complex manpower and logistical demands. The in situ technique of splitting offers advantages over the ex vivo technique, although after the time-dependent learning curve is overcome, they may theoretically be used interchangeably with acceptable

Transplantation of germ cells leads to restoration of spermatogenesis from spermatogonial stem cells. The original description of germ cell transplantation in 1994 has led to new approaches to explore many basic aspects of spermatogonial physiology. Combining germ cell transplantation with culture and cryopreservation of spermatogonia opens new pathways for genetic engineering and conservation of livestock. In addition, autologous transfer of spermatogonia might be used as an approach for fertility preservation in oncological patients. This review summarises the history of germ cell transplant

Major organ transplant today is to be regarded as an established therapeutic procedure for treating irreversible organ failure. The creation of the necessary technological, immunological and ethical foundations in the 20th century brought about a true breakthrough in transplantation medicine. The presentation outlines various clinical pictures that can lead to chronic organ failure based on examples of heart, liver and kidney diseases. The long-term survival rates of patients/transplants are outlined and a comparison of international statistics drawn. Different problems that can arise in conne…

In March 2000 we performed a bilateral hand transplantation on a 47-year-old policeman who had lost both hands in a bomb accident. Although he was fitted with myoelectric prostheses, he kept investigating the possibility of hand transplantation. The ethical, surgical, medical, legal and human aspects of this special kind of reconstruction was discussed in a symposium in Vienna, and it was decided to go ahead with it if a suitable candidate presented. The operation was successful, the immunosuppressive regimen well-planned and carried out, the post-operative course without any remarkable events

Hepatic transplantation has become the treatment of choice for advanced irreversible liver disease. More than 4,000 hepatic transplantations were performed in the United States in 1997 and more than 11,000 are awaiting transplantation. Graft endurance and overall patient survival has been steadily improving, and between 1992 and 1994, 82% of the patients who received a liver transplant survived for at least a year. Today, liver transplant patients have a five-year survival rate of approximately 75%. The improvement in survival can be attributed to better patient selection and preparation, adva

Hepatolithiasis is frequently encountered in Asia, but is relatively uncommon in Western societies. The improved surgical and stone fragmentation techniques that have evolved over the past decade have reduced the incidence of retained or recurrent stones with a consequent reduction in progressive liver damage and cirrhosis. Nonetheless, disease-related mortality from liver failure, bleeding oesophageal varices and cholangiocarcinoma still exists and a proportion of patients are cirrhotic at their initial presentation. There have been good long-term results following liver transplantation for a
