Adolescents at risk for mistreating their children. Part II: A home- and clinic-based prevention program.

OBJECTIVE: To determine if adding an intensive home visitation component to a comprehensive adolescent-oriented maternity program prevents child abuse and neglect. METHODS: We studied 171 participants in a comprehensive, adolescent-oriented maternity program who were deemed to be at high risk for child abuse and neglect. Half were randomly assigned to receive in-home parenting instruction. Major disruptions of primary care-giving by the adolescent mother were classified hierarchically as abuse, neglect, and abandonment. RESULTS: Compliance with home visits varied in relation to the support t

[Spontaneous and experimental models of human juvenile diabetes mellitus].

The author describes his findings pertaining to spontaneous diabetes mellitus in BB rats and the method and results in juvenile alloxan diabetes in neonatal and adolescent Wistar rats of his own inbreeding F8-10. The author presents also the results of attempts to treat juvenile alloxan diabetes in rats by intrafamilial renal-subscapular allotransplants of 2-5 neonatal collagenase nondigested pancreases. Six of eleven BB females developed latent or manifest insulin dependent diabetes mellitus during the third to fourth month of life. An intraperitoneal injection of alloxan to 2-5-day-old rats

Epidemiology of substance use in adolescence: prevalence, trends and policy implications.

This paper reviews the epidemiology of substance use among adolescents. There is a public health imperative in all countries to assess the population rates of tobacco, alcohol and illicit drug use among adolescents. In addition, monitoring trends over time may reflect the net effects of activities and programs carried out to prevent adolescent substance use. School based surveys provide prevalence estimates of substance use, but do not capture street and homeless youth and other high risk adolescents not found in the school environment. Overall, the results of this review suggest that tobacco,

Anemia and coagulation disorders in adolescents.

The transition of childhood to adulthood includes many changes to nearly all parts of the body and that is certainly true of blood and the coagulation system. Some disorders, like iron deficiency anemia, develop as the result of rapid growth. Approximately 10% of American adolescents are anemic and the prevalence is far greater in high-risk populations, such as urban, indigent African-American adolescents, in which 40-50% of young women are anemic. Adolescents at greater-than-average risk for developing iron deficiency anemia, such as athletes involved in lengthy, intense physical activities a

Obstetric outcomes in nulliparous young adolescents.

The objective of this study was to determine whether pregnant young adolescents under 15 years of age developed more obstetric complications than adult women. A retrospective case control study was undertaken. The pregnancies of 121 nulliparous women under age 15 delivered at Department of Obstetrics and Gynecology, Faculty of Medicine, Chulalongkorn University between January 1994 and December 2004 were compared to 121 nulliparous controls age 20-29. Multiple gestations were excluded. Young adolescents were more likely to develop anemia, preterm delivery, preeclampsia and deliver low birth we

Competencies.

This article briefly describes the background and development of the core competencies in medicine and their relevance to various levels of medical education: undergraduate, graduate, and post-graduate. The current status and issues of the competencies for each state of training--medical schools, residencies, and continuing medical education--are reviewed. Strategies for developing and implementing the core competencies into child and adolescent psychiatry training are discussed.

Celiac disease presenting with chilblains in an adolescent girl.

Chilblains, or pernio, are cutaneous lesions that may accompany systemic illnesses including states of malnutrition and autoimmune diseases. We report an adolescent girl in whom chilblains were the chief presenting sign of celiac disease. A gluten-free diet led to weight gain and resolution of the chilblains. We speculate that in this patient, weight loss due to celiac disease contributed to the development of chilblains.

Pollution burden and trajectories of pubertal development: Implications for sleep disturbances and depressive symptoms.

Childhood exposure to pollution has been associated with elevated levels of depressive symptoms during adolescence. Epidemiological studies have related exposure to pollution to altered pubertal timing; however, the effects of pollution exposure on levels of pubertal hormones and their developmental trajectories (i.e., pubertal tempo) are not known. Furthermore, how pollution-related alterations in pubertal development influence trajectories of depressive symptoms during adolescence is not well understood. One factor that has been linked to pollution exposure, pubertal development, and depress

Vaccination: an opportunity to enhance early adolescent preventative services.

New vaccines are being developed that will be recommended for adolescents Adolescence is a time period when adolescent-parent dynamics change and when adolescents may be confronted with increasing opportunities to engage in risk-taking behaviors such as sexual behavior and substance use. Despite clear recommendations regarding preventative counseling, many adolescents do not receive adequate preventive care. In this manuscript, we provide suggestions as to how a new vaccination schedule can be used to enhance preventative services to both adolescents and their parents.

Neuroimaging in Adolescents: Post-Traumatic Stress Disorder and Risk for Substance Use Disorders.

Trauma in childhood and adolescence has long-term negative consequences in brain development and behavior and increases the risk for psychiatric disorders. Among them, post-traumatic stress disorder (PTSD) during adolescence illustrates the connection between trauma and substance misuse, as adolescents may utilize substances to cope with PTSD. Drug misuse may in turn lead to neuroadaptations in learning processes that facilitate the consolidation of traumatic memories that perpetuate PTSD. This reflects, apart from common genetic and epigenetic modifications, overlapping neurocircuitry engagem

Treatment of physically abused adolescents.

It is clear that a number of factors are emerging as necessary criteria for helping physically abused adolescents: The need to establish specific residential treatment units for treating physically abused adolescents. The importance of educational programming to develop community awareness of the seriousness of this issue. The growing need for an awareness is conventional treatment environments to recognize the physically abused adolescent. The need to develop a sensitive and therapeutic approach for such children.

The effect of parenting behaviours on adolescents' rumination: a systematic review of longitudinal studies.

Rumination is an emotional regulation mechanism strongly associated with the development and maintenance of internalising psychopathology in adolescence and adulthood. Parenting behaviours (PBs) play a pivotal role in the development of rumination in children and adolescents. Nonetheless, the specific PBs that can either protect against or increase the risk of rumination development remain poorly understood. This systematic review aimed to explore the (1) temporal associations between PBs and adolescents' rumination and (2) potential moderators influencing these associations. We conducted a co

Prenatal Tobacco and Alcohol Exposure and Cortical Change Among Youths.

IMPORTANCE: The associations of prenatal alcohol exposure (PAE) and prenatal tobacco exposure (PTE) with adolescent neuroanatomical development are typically evaluated cross-sectionally. It is unclear whether observed effects persist throughout life or reflect different developmental trajectories. OBJECTIVE: To examine whether PAE and PTE are associated with early-adolescent cortical structure and development. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included children aged 9 to 12 years who participated in the Adolescent Brain Cognitive Development (ABCD) Study's first 2 neuroima

COVID-19 vaccine (mRNA BNT162b2) and COVID-19 infection-induced thrombotic thrombocytopenic purpura in adolescents.

The mRNA COVID-19 vaccine and COVID-19 infection caused by the SARS-CoV-2 virus may be immunologic triggers for the development of thrombotic thrombocytopenic purpura (TTP). There is not yet literature that discusses TTP induced by COVID-19 vaccination or infection in pediatric or adolescent patients. We describe three adolescents presenting with TTP (both de novo and relapsed disease) following administration of the Pfizer COVID-19 vaccine or after COVID-19 infection. Our observations demonstrate that the Pfizer-BioNTech mRNA vaccine and COVID-19 infection can act as triggers for the developm

The International society for developmental psychobiology 39th annual meeting symposium: Alcohol and development: beyond fetal alcohol syndrome.

As has been repeatedly demonstrated, alcohol can exert deleterious morphological and physiological effects during early stages in development. The present review examines nonteratological links existing between alcohol and ontogeny. Human and animal studies are taken into consideration for the analysis of fetal, neonatal, infantile, adolescent, and adult responsiveness to the drug. Sensitivity to alcohol's chemosensory and postabsorptive properties, as well as learning and memory processes mediated by such properties, are examined from this developmental perspective. The studies under discussi

Positive affect as a developmental mediator of early adversity and internalizing psychopathology.

BACKGROUND: Early life adversities (ELAs) including experiences such as abuse, neglect, and household dysfunction are strongly linked to psychopathology; yet, the developmental pathways connecting ELA to externalizing and internalizing psychopathology remain unclear. While most research has focused on threat and negative affect, positive emotions may represent a critical but understudied mechanism linking ELA to mental health outcomes. METHODS: Using data from the Adolescent Brain Cognitive Development (ABCD) study, we examined positive affect trajectories across six timepoints spanning child
