Electrocardiographic features and rhythm disorders in cardiac amyloidosis.

Cardiac amyloidosis (CA) is an infiltrative cardiomyopathy caused by extracellular deposition of amyloid fibrils, mainly derived from transthyretin, either wild-type or hereditary variants, or immunoglobulin light chains misfolding. It is characterized by an increased left ventricular (LV) mass and diastolic dysfunction, which can lead to heart failure with preserved ejection fraction and/or conduction disturbances. The diagnosis is based on invasive pathology demonstration of amyloid deposits, or non-invasive criteria using advanced cardiovascular imaging techniques. Nevertheless, 12-lead ele

Catastrophic Events of Cardiac Sarcoidosis: A Case Report.

Cardiac sarcoidosis (CS) can be silent in most patients with extrapulmonary sarcoidosis. Atrioventricular (AV) block is the most common clinical presentation, but it can also present as fatal ventricular arrhythmias and sudden cardiac death. Endomyocardial biopsy is the gold standard; however, it is not sensitive since CS can involve the myocardium in a patchy distribution. Our case depicts a female who presented with syncope; however, her hospital course was complicated by multiple cardiac arrests. Her initial laboratory tests, including an autoimmune workup, were unremarkable. Cardiac magnet

Iatrogenic ventricular fibrillation in Wolff-Parkinson-White syndrome.

Wolff-Parkinson-White (WPW) syndrome is the most common manifestation of ventricular pre-excitation syndrome and is mostly found in individuals with no structural heart disease. Although the risk of malignant arrhythmias is low, sudden cardiac death (SCD) as the first clinical manifestation of WPW syndrome is well documented, and atrial fibrillation (AF) with a rapid ventricular response is the main mechanism involved. Unfortunately, the signs of pre-excitation and arrhythmias are sometimes under-diagnosed and under-treated. We describe the case of a 31-year-old man who was admitted with an ir

Furthermore, diltiazem reduces conduction in atrioventricular node, which is also useful for heart rate control in patients with atrial fibrillation. In this review, clinical experts highlight studies on diltiazem effectiveness and safety for the treatment of several cardiovascular diseases and make evidence-based …

The clinical spectrum of autoimmune congenital heart block.

Autoimmune congenital heart block (CHB) is an immune-mediated acquired disease that is associated with the placental transference of maternal antibodies specific for Ro and La autoantigens. The disease develops in a fetal heart without anatomical abnormalities that could otherwise explain the block, and which is usually diagnosed in utero, but also at birth or within the neonatal period. Autoantibody-mediated damage of fetal conduction tissues causes inflammation and fibrosis and leads to blockage of signal conduction at the atrioventricular (AV) node. Irreversible complete AV block is the pri

Cardiac resynchronization therapy: expanding clinical indications and refining patient selection.

PURPOSE OF REVIEW: Cardiac resynchronization therapy (CRT) can reduce morbidity and mortality in patients with heart failure. However, a pro … st that patients with NYHA III heart failure and atrial fibrillation may benefit from CRT. However, atrioventricular node ablation should be considered in this population to ensure greater than 92% biventricular pacing. Data from clinical trials do not support the use of CRT in patients with baseline right bundle branch block (RBBB). SUMMARY: Careful selection of CRT …

Misleading ECG appearance of AV block due to concealed AV nodal conduction caused by interpolated ventricular ectopic beats.

Concealed conduction commonly occurs when a retrogradely conducted interpolated ectopic impulse enters the atrioventricular (AV) node; thus, the next sinus beat is not conducted to the ventricle or conducted with a prolonged PR interval because of increased refractoriness of AV conduction system. A 67-year-old man had complaints of exertional fatigue and palpitations at rest. His blood pressure was 110/70 mmHg and heart rate was 78 beats/min. Auscultation revealed a mild systolic murmur at the apex and an irregular rhythm. His electrocardiogram was normal, except for the presence of frequent p

A novel rabbit model of variably compensated complete heart block.

Complete heart block (CHB) provides a useful substrate for study of bradycardia-dependent ventricular arrhythmias and cardiac function. Existing CHB animal models are limited by surgical recovery time and reliance on intrinsic escape rhythms. We describe a novel closed-chest rabbit model of CHB involving transcatheter radiofrequency (RF) atrioventricular (AV) node ablation and ventricular rate control with chronic transvenous pacing. Permanent CHB was achieved in 34 of 38 attempts overall. Procedural mortality due to cardiac tamponade (n = 2), airway complications (n = 2), and unknown causes (

[Pacing mode influence on morbidity and mortality].

It is widely believed that physiologic pacing (defined as atrial or atrioventricular synchronous pacing) reduces cardiac morbidity and mortality and is associated with improved quality of life compared to ventricular pacing. In this review we present data from three large prospective trials (PASE, CTOPP, MOST) comparing physiologic pacing to ventricular pacing. The prospective trials report that physiologic pacing is associated with a reduced risk of developing chronic atrial fibrillation over time and improved quality of life in patients with sinus node disease compared to ventricular pacing.

Silent zone on Lorenz plots of the ventricular response before termination of paroxysmal atrial fibrillation--report of a case.

An ambulatory 24 h Holter electrocardiogram was recorded during paroxysmal atrial fibrillation in a 67-year-old man. His paroxysmal atrial f … ard the termination. The silent zone was closely related to the functional refractory period of the atrioventricular node estimated from the Lorenz plots, and the functional refractory period was significantly correlated with the CV and HF power of RR variability. This case suggests that a silent zone on Lorenz plots reflects increased parasympathetic tone. In addition …

Spontaneous occurrence of the induced cardioinhibitory vasovagal reflex.

ECG recording of spontaneous, neurally-mediated syncope is rare. We have observed ten patients who sustained 70 syncopal episodes in whom: (1) ECG monitoring recorded syncope caused by ventricular asystole (AV block, three patients; sinus arrest; seven patients); (2) syncope and the spontaneously observed arrhythmias were reproducible by carotid sinus massage, upright tilt test, or eyeball pressure; and (3) no discernable cause of precipitating factors were detected. Two patients had a history of cardiac disease and four patients had only mild nonclinical ECG or echocardiographic abnormalities

[Nodofascicular fibers (Mahaim) as a part of the reentry circuit in patients with supraventricular tachycardia].

A history of paroxysmal tachycardia with broad QRS complexes resembling left bundle branch block led to electrophysiologic studies in a 20-year-old female. The results were diagnostic of a nodofascicular Mahaim fiber. Delayed retrograde conduction following transient proximal right bundle branch block as well as an infranodal termination mode were noted and are considered to be proof of participation of the nodofascicular fibers in the re-entry loop. Marked differences of atrioventricular conduction were noted on comparing refractory period determinations from the right atrium and from the cor

BRASH Syndrome with Hyperkalemia: An Under-Recognized Clinical Condition.

BRASH syndrome is characterized by bradycardia, renal failure, use of an atrioventricular nodal blocker (AVNB), shock, and hyperkalemia. These symptoms represent an ongoing vicious cycle in a patient with a low glomerular filtration rate taking an AVNB. Decreased clearance of the medication and hyperkalemia associated with renal failure synergize to cause bradycardia and hypoperfusion. This reaction causes renal function to worsen, thereby perpetuating the cycle of BRASH syndrome.

Adenosine-sensitive Wolff-Parkinson-White: Longer time across the atrioventricular groove.

BACKGROUND: Successful ablation sites in Wolff-Parkinson-White syndrome (WPW) are characterized by short atrioventricular (AV) intervals. Approximately 15% of patients with WPW have adenosine-sensitive accessory pathways (APs). We sought to determine if local AV intervals of adenosine-sensitive APs are different from those of adenosine-insensitive APs in patients with WPW. METHODS: Patients ≤21 years with WPW and adenosine-sensitive APs who underwent successful ablation over a 9-year period were included. Patients with WPW and adenosine-insensitive APs were matched by age and weight in a 1:2
