Infectious lesions of the brain stem.

Infectious lesions of the brain stem are rare and include primarily abscess and encephalitis. The most common etiologic agents for abscess formation are Streptococcus sp., Staphylococcus sp., and M. tuberculosis. Encephalitis is associated most often with L. monocytogenes and herpes simplex virus infection. Classical brain stem syndromes are uncommon with brain stem infections, and CSF obstruction can be seen with neurocysticercosis. The diagnosis of these lesions has been greatly aided by CT and MR imaging. Microsurgery and stereotaxis are both appropriate techniques for the treatment of brai…

[Fungus imitators in the newborn brain].

Morphology of the basophilic fibers erroneously considered by some authors as the fungus mycelium found in 4 newborns with periventricular leukomalacia is studied at the light- and electron microscopic level. Differential diagnosis between fungi and these structures which, in the authors opinion, are the damaged non-myelinated axons, including those with mineralization, is presented.

Cavernous malformations of the brain stem.

Once they become symptomatic, cavernous malformations of the brain stem appear to cause progressive morbidity from repetitive hemorrhage, and can even be fatal. Twenty-four patients with long-tract and/or cranial nerve findings from their cavernous malformations of the brain stem were seen for initial evaluation or surgical consultation and thereafter received either surgical or continued conservative treatment. The decision to operate was based on the proximity of the cavernous malformation to the pial surface of the brain stem, the patient's neurological status, and the number of symptomatic…

The incidence of prenatal brain injury.

Recent epidemiological studies have shown that cerebral palsy is only rarely associated with birth asphyxia and that there may be more important causative factors in the prenatal period. For this reason a series of 165 brains of infants who were stillborn or died in the early neonatal period was examined in order to identify the incidence and nature of prenatal brain damage. Seventeen (44%) of the stillborn infants showed evidence of brain damage thought to be related to circulatory disorders. The most frequent abnormality, widespread ischaemic damage to the white matter of the cerebral hemisp…

[Vascular accidents of the brain stem].

Cerebrovascular accidents (CVA's) of the brain stem have the same causes and physiopathological mechanisms as CVA's of the hemispheres, but they are ten times less frequent. They correspond to ischaemic or haemorrhagic lesions in the territories of the vertebral arteries and the basilar artery, including their collateral, circumferential and perforating branches. Clinically, CVA's of the brain stem are characterized by the association of unilateral motor and/or somatosensory symptoms on the side opposite the lesion, and by involvement of the ipsilateral cranial nerves, which helps in evaluatin…

Brain death and the anencephalic newborn.

We will set the stage for our analysis by reviewing selected medical aspects of anencephaly, outlining the history of the use of anencephalics as organ sources, and summarising the results of an important study recently completed at Loma Linda University. We will then employ some of the arguments and justifications underlying the Uniform Determination of Death Act (UDDA) to claim that anencephaly is morally equivalent to brain death, i.e., the reasons for considering brain-dead patients to be dead also apply to anencephalics. Finally, we will critique our proposal and discuss its implications.

Fragments of lipoma ascending to the brain.

A 37-year-old woman underwent microsurgical resection of a "lipodermoid" tumor of the conus medullaris. Preoperative evaluation demonstrated an incidental finding of multiple small hypodense lesions in the cerebral ventricles and basal cisterns on cranial computed tomography. It is hypothesized that these lesions represent rostral subarachnoid embolization of flecks of fat from the conus lesion. Evidence supporting this hypothesis is presented.

Interface between the meningioma and the brain.

Though meningiomas are usually well demarcated from the brain, their interfaces with the brain take various forms. Based on 27 nonoperated supratentorial meningiomas from autopsies, the interfaces were classified into four types, that is, smooth, lobular, finger-like expansion, and invasive. Relationships between the types of interfaces and the so-called capsule of the tumor and subarachnoid vessels are discussed. Additionally, three surgical cases which showed more or less the invasive form are presented with neuroradiological findings.

Heterotopic lingual brain in the newborn.

Heterotopic brain is a rare entity. Histologically, this lesion resembles mature brain and often contains specialized tissues similar to choroid plexus or glia. Specialized neural stains are necessary to differentiate this rare anomaly from other tumors or conditions found in the head and neck. The differential diagnosis includes squamous cell carcinoma, granular cell tumor, hemangioma, lymphangioma, thyroglossal duct cyst, dermoid cyst, hamartoma, rhabdomyosarcoma, and teratoma. We describe a newborn with heterotopic brain tissue occurring on the dorsum of the tongue. We found only one other …

Prenatal radiation risk to the brain.

The Central Nervous System (CNS) exhibits a high sensitivity to ionizing radiation from conception until after birth. X-irradiation damage o … increased in the maternal circulation following irradiation and reach the fetal CNS through a blood-brain-barrier that is more permiable than normal. The introduction of histamine and/or serotonin into the fetal circulation may result in fetal hypotension, edema, cerebral ischemia, and damage to the developing CNS.

Tuberculous abscess of the brain stem.

Three cases of tuberculous abscess of the brain stem were treated with excision of the abscess supplemented with antitubercular therapy for 12 to 18 months. The lesions were frankly purulent and tubercle bacilli were demonstrated in the pus. A computed tomography scan demonstrated the site and extent of the lesion. Two of three patients developed the abscess during the course of antitubercular therapy for associated tubercular lesions. In spite of modern antitubercular treatment the abscess did not resolve and surgical excision had to be employed as a curative treatment.

[Benign encephalitis of the brain stem].

Seven cases of an encephalitic brainstem syndrome are reported. All had an initial flu-like phase, with a febrile state. Then, all had eye movements disturbances and ataxia. Six had a variable dysfunction of other cranial nerves, five had dysmetria of limbs and three had pyramidal signs. In all, the topographic diagnosis was based on clinical, neurophysiological (electromyography, evoked potentials) and neuroradiological (swelled brainstem on CT scan) findings. Analysis of cerebro-spinal fluid revealed pleocytosis (mean; 165 cells/ml), without profile indicative of multiple sclerosis. Serologi…

Histiocytic sarcoma of the brain in childhood.

CT-scan findings of the brain in two children with true histiocytic sarcoma are described as well as their changes during therapy. Clinical features and differential diagnosis on CT-scan examination are discussed. Follow-up CT-scan during therapy is of value as an indicator of the response to therapy and possibly the long term prognosis.

Massive necrosis of the brain in rabies.

A young man developed virologically proved hydrophobic rabies three months after being scratched on the cheek by a bat in Northern Alberta. He became comatose after 8 days and died 5 weeks after vigorous therapy with immune globulin, interferon and Vidarabine, and excellent maintenance of oxygenation. Electroencephalographic deterioration was gradual. At postmortem examination, the brain showed widespread loss of nerve cells, mild diffuse inflammatory changes and no Negri bodies. The lesions of massive laminar necrosis of the cerebral cortex, total loss of Purkinje cells with preservation of t…

Heterotopic brain tissue in the orbit.

A 3-month-old female infant with Turner's syndrome was noted to have progressive swelling of the left upper lid at the age of 1 month. A computed tomogram demonstrated a well-circumscribed, low-density, homogeneous orbital mass. Initially, the main clinical differential diagnosis included dermoid cyst, hemangioma, and encephalocele. Medial orbitotomy followed by histopathologic examination disclosed a cystic choristomatous malformation containing brain tissue. No choroid plexuses within the mass were demonstrated on microscopic examination. Aspiration of fluid from the cystic mass and subseque…

Diagnosis of brain neuropathology in utero.

The fetal brain can be evaluated very effectively with high-resolution real-time ultrasound equipment. This paper reviews normal fetal brain anatomy with specific emphasis on artifacts of ultrasound scanning of the brain. A review of the developmental abnormalities seen in utero include ventriculomegaly, anencephaly, encephalocele, meningomyelocele, Dandy-Walker syndrome, holoprosencephaly, craniosynostosis, microcephaly, and agenesis of the corpus callosum. The destructive lesions reviewed include hydranencephaly, infection, intracranial hemorrhage, and mass lesion. Identification of these ab…
