Computed tomography (CT) assessment of the membranous septal anatomy prior to transcatheter aortic valve replacement (TAVR) with the balloon-expandable SAPIEN 3 valve.

OBJECTIVES: The lower limit of the membranous septum (MS) is considered an anatomic landmark for the emergence of the Bundle of His into the left ventricle. Computed tomography (CT) assessment of MS anatomy may provide useful information about the risk of conduction abnormalities following transcatheter aortic valve replacement (TAVR). METHODS AND RESULTS: The study included 102 consecutive patients undergoing TAVR with the Edwards Sapien 3 (S3) valve. Using pre-TAVR CT and post-procedure angiography we evaluated for the presence of calcium in the left ventricular outflow tract (LVOT), calciu…

Conduction System Pacing Improved Cardiac Functions, Myocardial Work and Functional Capacity in Heart Failure with Reduced Ejection Fraction and Right Bundle Branch Block.

Background/Objectives: Conduction system pacing (CSP) is a potential alternative to biventricular pacing (BVP) in heart failure with reduced ejection fraction (HFrEF) and left bundle branch block (LBBB) or non-LBBB. Available data also suggest that unlike BVP, CSP may improve clinical outcome in patients with right bundle branch block (RBBB), although its effects on cardiac mechanics and energetics are ill-defined. Herein, we report on echocardiographic and clinical outcomes of CSP in this patient cohort. Methods: CSP either with His bundle pacing or LBB area pacing was attempted as a primary

Plantar Dermal Deposition of Wild-Type Transthyretin Amyloid (ATTR): A Case Report of a Unique and Challenging Histopathological Manifestation of Cutaneous ATTR Amyloidosis.

A 74-year-old man presented with a cutaneous lesion on the plantar surface of his right foot. His medical history included spinal canal stenosis and bilateral carpal tunnel syndrome. The lesion was clinically suspected to be a fibroma and was subsequently resected. Histopathological examination revealed mild acanthosis and hyperkeratosis, along with eosinophilic changes and thickening of collagen bundles at the base of the specimen. Based on the patient's medical history and the pathological findings, transthyretin amyloid (ATTR) deposition was suspected. Congo red staining confirmed the prese

Cardiac Resynchronization Therapy-Emerging Therapeutic Approaches.

Cardiac resynchronization therapy (CRT) is an important therapeutic tool in the management of patients with heart failure and electrical dyssynchrony. In appropriately selected patients, landmark randomized controlled trials have demonstrated morbidity and mortality benefit beyond standard goal-directed medical therapy. Current guidelines emphasize the greatest clinical efficacy of CRT in patients with symptomatic heart failure, left bundle branch block, and wide QRS duration (> 150 ms). Other relevant considerations include the presence of atrial fibrillation, the presence of AV block, the et

Utility of mapping signals to improve precision of atrioventricular node ablation.

BACKGROUND: Atrioventricular node (AVN) ablation is effective for rate control in atrial fibrillation. This may require multiple radiofrequency applications to achieve complete atrioventricular block (CAB). In this retrospective study, we tested the hypothesis that mapping the AVN utilizing electrograms (EGMs) on both proximal and distal bipoles of the mapping catheter may improve the likelihood of CAB. METHODS: Lesion characteristics and EGM components on the proximal and distal bipoles of the ablation catheter in first-time AVN ablation procedures were analyzed. Outcomes of each lesion, inc

Invasive measurements of atrioventricular conduction parameters prior to and following ventricular septal defect closure with the amplatzer device.

BACKGROUND: Because of anatomical proximity, closure of paramembranous ventricular septal defect (pm-VSD) with an Amplatzer occluder has the potential to affect the atrioventricular (AV) conduction system. PATIENTS AND METHODS: Electrophysiology studies (EPS) were performed prior to and following closure of the pm-VSD in 19 children aged 8.9 +/- 4.5 years. Data from electrocardiograms, Holter monitoring and exercise tests before intervention and at follow up (1.9 +/- 0.8 years) were reviewed. RESULTS: There were no EPS catheter-induced complications. Incidence of total conduction anomalies w

Narrowing the QRS, Expanding the Function: Left Bundle Branch Pacing in Dilated Cardiomyopathy.

Left bundle branch block (LBBB) is a key driver of electrical and mechanical dyssynchrony in heart failure with reduced ejection fraction (HFrEF). Conventional cardiac resynchronization therapy improves outcomes, yet up to one-third of patients is non-responders, partly due to nonphysiological ventricular activation. Left bundle branch pacing (LBBP) has emerged as a physiological alternative by directly recruiting the His-Purkinje system. We describe the case of a 62-year-old woman with nonischemic dilated cardiomyopathy, severely reduced left ventricular ejection fraction (30%), and complete

Impact of Different Pacing Sites on Pre-Implantation Screening Test of Subcutaneous Implantable Cardioverter Defibrillators.

BACKGROUND: Pre-implantation screening is a crucial step in preventing inappropriate sensing in patients with subcutaneous implantable cardioverter defibrillators (S-ICDs). Paced QRS-T morphology may influence screening outcomes. We aimed to explore the impact of pacing rhythm and different pacing sites on pre-implantation screening of S-ICD. METHODS AND RESULTS: This prospective, single-center study enrolled patients referred for radiofrequency ablation, with both left and right-heart access. A contact-force ablation catheter was used to mimic ventricular pacing at different sites. Automatic

Relationship between polarity of the flutter wave in the surface ECG and endocardial atrial activation sequence in patients with typical counterclockwise and clockwise atrial flutter.

BACKGROUND: The relation between ECG and activation patterns within atria in typical atrial flutter (AFL) patients (pts) has not been defined due to the lack of simultaneous multisite right and left atrial mapping. METHODS: In 13 pts with AFL, a Halo catheter was positioned along tricuspid annulus and multipolar catheters were placed in right atrial appendage, His bundle region, coronary sinus (CS), proximal portion of right pulmonary artery (Bachmann's bundle region, BB) and esophagus (Eso) to record right and left atrial activation simultaneously. RESULTS: In counterclockwise (CCW) AFL (11

Paroxysmal atrioventricular block as an initial presentation of bicuspid aortic valve calcification: a case report.

BACKGROUND: Atrioventricular (AV) conduction disturbances in bicuspid aortic valve (BAV) patients are typically attributed to age-related fibrosis or post-surgical trauma. Direct mechanical impingement of the conduction system by accelerated valvular calcification causing reversible, dynamic AV block is rarely reported. CASE SUMMARY: A 43-year-old male presented with syncope. Electrocardiograms revealed dynamic high-grade AV block and alternating bundle branch block (ABBB). Echocardiography and computed tomography identified a BAV with a calcified spur extending into the membranous interventr

Ventricular tachycardia based on cardiac sarcoidosis with a narrow QRS complex, ablated on the left ventricle free-wall.

A septuagenarian female with cardiac sarcoidosis suffered from drug refractory ventricular tachycardia (VT) requiring multiple implantable cardioverter-defibrillator shocks. The QRS complex during the VT was very similar to that during sinus rhythm although the QRS width during the VT (142 ms) was relatively wider than that during sinus rhythm (107 ms). The VT exit was located on the ventricular septum close to the His-bundle recording region. However, the critical pathway of this VT was detected on the anterior free wall of the left ventricle (LV), and a radiofrequency application at that sit

Atrioventricular Junction Ablation with High-Definition Recording of Atrioventricular Node Potential.

Atrioventricular (AV) node ablation represents an established therapeutic option in the management of atrial fibrillation (AF) and other atrial tachyarrhythmias, particularly in patients with symptomatic tachycardia who remain unresponsive or intolerant to pharmacological therapy. The procedure is often considered in cases of refractory arrhythmias, antiarrhythmic drugs intolerance, or tachycardiomyopathy, and plays a key role in optimizing outcomes in patients undergoing cardiac resynchronization therapy, where achieving adequate biventricular pacing is otherwise compromised by rapid ventricu

Max brödel: illustrating healed valve ring abscesses.

A 14-year-old adolescent girl presented with severe congestive heart failure, progressive throughout 3 months. A precordial thrill, machinery-like murmur, and right bundle branch block were noted. Death occurred despite digitalis and diuretic therapy and removal of pleural and ascitic fluid. The autopsy revealed 2 multilocular cystic structures in the interventricular septum consistent with being spontaneously drained valve ring abscesses. One of these lesions formed a fistulous communication that penetrated through the interventricular septum between the right aortic sinus of Valsalva and the

Electrocardiographic changes associated with takotsubo cardiomyopathy in a patient with pre-existing left bundle branch block.

An 81-year-old man was referred to our emergency department with severe persistent chest pain. One year before presentation at our department, his 12-lead electrocardiogram (ECG) revealed a normal QRS pattern during the period of normal conduction with intermittent left bundle branch block (LBBB). His ECG immediately after arrival showed deep T-wave inversion in the precordial leads during normal conduction. During LBBB, there was mild ST-segment elevation with poor R-wave progression across the precordial leads. Emergent cardiac catheterization was performed to rule out acute coronary syndrom

Surviving 20 Hours of Critical Avalanche Burial and a Core Temperature of 22.5 °C.

A 24-year-old male snowboarder was buried in an avalanche for 20 h and rescued on the next day at a depth of 2.3 m below the snow surface. A large air pocket was noted in front of his mouth and nose. He was responsive but moved restlessly and uncoordinatedly. The epitympanic temperature was 22.5 °C. He was bradycardic (35/min), and a right bundle branch block with Osborn waves was noted. Rewarming (1 °C/h) was initiated with continuous hemodialysis; core temperature raised to 29.8 °C within 4 h. At 30 °C he became conscious. With rewarming, the heart rate increased to 90 beats per minute and t

Persistent functional atrioventricular block in two patients with prolonged QT intervals: elucidation of the mechanism of block.

This article describes two infants with prolonged QT interval and intermittent second-degree atrioventricular block. An asymptomatic 14-month-old child with persistent 2:1 atrioventricular conduction since birth underwent electrophysiology study including measurements with a contact monophasic action potential catheter. During 2:1 conduction, atrioventricular block occurred distal to the site of the His-bundle recording. Monophasic action potential duration was closely related to prior RR intervals. Single premature atrial or ventricular depolarizations during 1:1 conduction followed by a paus

The Jewish Pathologist Carl Julius Rothberger (1871-1945) and the gradual deprivation of his rights in the Third Reich.

The Jewish pathologist Carl Julius Rothberger (1871-1945) is undoubtedly one of the most important representatives of his field. His studies on atrial fibrillation, the bundle branch block and arrhythmia perpetua in particular secured him a place in medical history. Rothberger also gave the name to an agar used to prove the neutral red reduction of salmonella (Rothberger-Scheffler agar). While Rothberger's name is well known in pathology, his biography and his experiences of stigmatization as a Jewish university lecturer have received little attention. The latter are therefore the focus of thi

[Sub-total hereditary leukonychia, histopathological and electron microscopy study of "milky" nails].

INTRODUCTION: Leuconychia is the most common of ungueal discoloration or dyschromia. The hereditary form, sub-total or total, is very rare. We report the observation of a family in which thirteen members, distributed over four generations, presented a partial or subtotal leukonychia of all nails. On the basis of this observation and histopathological analysis of the propositus nail, we discuss the different hypotheses proposed to account for white nails. OBSERVATION: A 45 year-old man without previous medical history, showed partial or sub-total leukonychia of his twenty nails. Clinical exami
