Decontamination upgrading progresses.

Standards of decontamination of surgical instruments in the NHS in England have come under close scrutiny, and some significant improvements have been achieved. Action plans, developed to ensure the rate of improvement is maintained, are described in this article which is derived from the Department of Health/NHS Estate document 'A review of the decontamination of surgical instruments in the NHS in England'. The Institute of Healthcare Engineering and Estate Management is one of the organisations involved in the programme of work on decontamination.

In vitro evaluation of the antibacterial activity of three different central venous catheters against gram-positive bacteria.

The aim of this study was to evaluate the activity of three different catheters against Staphylococcus aureus ATCC 29213 and the slime-producing Staphylococcus epidermidis ATCC 35984 (RP62A). Three central venous catheters were evaluated: one impregnated with silver sulfadiazine-chlorhexidine, one to which minocycline/rifampin is bonded and a novel one into which silver, platinum and carbon are incorporated. A nonantiseptic catheter was used as the control catheter. One-centimeter trisected pieces of catheter were immersed in phosphate-buffered saline (0.01 mol/l) with 0.25% dextrose and incub

Bacteremia, central catheters, and neonates: when to pull the line.

OBJECTIVES: Physicians who treat neonates who become bacteremic while dependent on central venous catheters face a serious and common dilemma. We sought 1) to evaluate the relationship between central venous catheter removal and outcome in bacteremic neonates, 2) to determine species of bacteria that are associated with an increased risk of infectious complications if the central catheter is not removed promptly, and 3) to provide evidence-based recommendations for central catheter management. METHOD: A retrospective cohort study of all neonates who had central venous access and developed bac

Silicone oil removal from a silicone intraocular lens with perfluorohexyloctane.

Silicone oil can be easily dissolved in vivo from silicone intraocular lenses by perfluorohexyloctane. Using specular microscopy, we examined the corneal endothelium in a patient with silicone lens implantation immediately after cataract surgery and 5 weeks postoperatively. There were no signs of acute or chronic toxicity to the endothelium.

Tuberculosis control through respirator wear: performance of National Institute for Occupational Safety and Health-regulated respirators.

BACKGROUND: In 1995 the National Institute for Occupational Safety and Health issued new rules for personal respirators. All nine new respirator categories are authorized in health care facilities for the prevention of the transmission of tuberculosis (TB). The new N95 respirator category is the most frequently used for this purpose. Data are presented on their efficiency for collecting TB-size bacteria and their potential for reaerosolizing collected bacteria. METHODS: All measurements of bacterial penetration were performed with dynamic aerosol size spectrometers at flow conditions correspo

Changes in clinical features of fungemia in a Japanese University Hospital over a 12-year period.

Forty-five patients with fungemia during 1982-1993 (periods I = 1982, II = 1986-1989, III = 1990-1993) in a Japanese university hospital were reviewed to follow changes in the clinical features of fungemia. The percentage of fungi among microorganisms isolated from blood cultures was almost constant (6-10%) throughout the study period. Fungemia was highly associated with use of intravascular catheters, and some changes in clinical features were observed: 1) Candida albicans, C. parapsilosis and C. glabrata were the main isolates, and the number of fungal species showed a tendency to increase.

Continuing investigation and controversy regarding risk of transmission of infection via dental handpieces.

Current epidemiologic evidence indicates that infectious diseases, specifically blood-borne pathogens such as hepatitis B, hepatitis C and HIV, are not transmitted from patient to patient via dental instruments. However, ongoing laboratory investigations suggest that potential pathogens may be retained within dental handpieces, creating a theoretical risk of cross infection. Controversy regarding certain laboratory study results and the clinical implications of these studies continues. Guidelines and regulations for infection control should be rational, and based on a realistic response to a d

Pseudomonas aeruginosa outbreak associated with a contaminated blood-gas analyser in a neonatal intensive care unit.

Over a 10 month period in a neonatal intensive care unit there was an outbreak of infection caused by Pseudomonas aeruginosa (resistant to ticarcillin, timentin) which involved 24 newborns. There was extensive morbidity and mortality (38%) associated with the infections, which presented as septicaemia (N = 6) (five succumbed and four had coexisting pneumonia), pneumonia (N = 6), meningitis (one, died), conjunctivitis (N = 1), otitis externa (N = 1), conjunctivitis plus otitis externa (N = 1). In addition there were two pseudosepticaemias and six colonized infants, three of whom were treated fo

Contaminated toothbrushes and their disinfection.

Twenty toothbrushes used by healthy subjects were screened for the presence of microorganisms. Microbes were dislodged from the brushes by vortexing, and an average of 4 x 10(3) CFU/mL were recovered from the suspending fluid. Bristles removed from the vortexed brushes still yielded confluent bacterial growth on brain-heart infusion agar medium. Virkon (one per cent), Listerine, Cepacol, Scope, and Plax were tested for their bactericidal effects on microorganisms sedimented from the suspending fluid, on toothbrush bristles and proxabrushes, and on various test species including Candida albican

Nosocomial outbreak of Salmonella typhimurium infection in a nursery intensive care unit (NICU) and paediatric ward.

A nosocomial outbreak of multidrug resistant Salmonella typhimurium in a Nursery Intensive Care Unit (NICU) and Paediatric ward is reported. Eight (16.6%) out of a total of 48 babies taken ill during this outbreak expired. Clinical manifestations included diarrhoea and fever. The organism was isolated from stool samples/rectal swabs of all the 48 cases and blood of 6 cases. An investigation was undertaken to trace the source of infection. The organism was isolated in pure cultures from three suction machines of NICU. The epidemic was immediately controlled with identification of the source but

Septic arthritis after arthroscopy: the contributing roles of intraarticular steroids and environmental factors.

During a 9-month period at a small surgical center, seven cases of postarthroscopic septic arthritis occurred after 352 arthroscopic procedures, for an infection rate of 2.0%. Electrocardiographic cables contaminated with Pseudomonas aeruginosa from an unsterile cleaning solution probably led to two shoulder infections. Five other infections (one definite Staphylococcus aureus and three definite and one possible coagulase-negative staphylococci) were attributed to inadequate arthroscope disinfection. In the initial phase of the outbreak, use of intraoperative intraarticular corticosteroids cor

The effects of electric current on bacteria colonising intravenous catheters.

The effect of a direct electric current (10 microA) on the growth of Staphylococcus aureus, Staphylococcus epidermidis, Escherichia coli, Klebsiella pneumoniae and Proteus mirabilis was investigated. When the ends of negatively-charged intravascular catheters were placed in nutrient agar seeded with bacteria, circular zones of inhibition of bacterial growth were observed around the catheters. The zones ranged from 6 to 16 mm in diameter according to the organism under test. Zones of inhibition were not produced around positively-charged catheters. Bacteria colonising the surfaces of catheters

Surgical treatment of aortic root abscess.

A significant number of patients with infective aortic valve endocarditis develop aortic annular abscess. Although antibiotics may occasionally sterilize an aortic root abscess, most patients require surgical intervention. A review of our experience with 21 consecutive patients surgically treated for aortic root abscess disclosed that 13 patients had prosthetic valve and eight had native aortic valve endocarditis. The predominant microorganism was Staphylococcus aureus, particularly in those patients with native aortic valve endocarditis. The abscess was limited to the aortic annulus in 10 pat

[A case of preoperative esophageal cancer developing lobar pneumonia due to MRSA infection of the catheter].

The 65-year-old male patient of esophageal cancer before operation suffered from lobar pneumonia. Bacteriological examination identified MRSA in his sputum and catheter tip. None of the cefem type antibiotics were effective, but CLDM and MINO were so much effective against that. We studied 47 cases of esophageal cancer which received operation last year. In many bacteria which were identified in the sputum and pus, MRSA occupied 30% as well as Pseud. aeruginosa. In the preoperative period enteral nutrition (ED) may be acceptable, if possible, as nutritional support alternative to TPN.

Comparison of Blood Culture Contamination Rates Between Initial Central Venous Puncture and Catheter Hub Sampling.

OBJECTIVES: Blood cultures are essential for guiding antimicrobial therapy, but contamination remains a major challenge, particularly when samples are drawn from central venous catheters (CVCs). Blood cultures obtained from CVCs, even when newly inserted, show higher contamination rates than those from peripheral venipuncture, likely due to translocation of skin bacteria during skin dilation and catheter insertion. In critically ill patients with suspected infection or difficult peripheral venous access, CVC insertion often coincides with the indication for blood culture sampling. We hypothesi

Ensuring Equal Implantation Conditions on Both Sides During Breast Augmentation: Effective Decontamination of Implantation Funnels.

BACKGROUND: The role of mitigation strategies to decrease contamination during breast implant surgery has advanced over the years, and the utility of implantation funnels has been recently validated by several studies. However, the additional cost of using separate funnels for each breast has led surgeons to use the same funnel on both sides, which increases the risk of microbial contamination and complications such as capsular contracture in the second breast. METHODS: This prospective study included ten consecutive patients undergoing primary breast augmentation. The sterility of the funnel

Recommendations for Deep Brain Stimulation Device Management During a Pandemic.

Most medical centers are postponing elective procedures and deferring non-urgent clinic visits to conserve hospital resources and prevent spread of COVID-19. The pandemic crisis presents some unique challenges for patients currently being treated with deep brain stimulation (DBS). Movement disorder (Parkinson's disease, essential tremor, dystonia), neuropsychiatric disorder (obsessive compulsive disorder, Tourette syndrome, depression), and epilepsy patients can develop varying degrees of symptom worsening from interruption of therapy due to neurostimulator battery reaching end of life, device

Is there value in the routine practice of discarding the incision scalpel from the surgical field to prevent deep wound contamination with Cutibacterium acnes?

BACKGROUND: Cutibacterium acnes is found in skin flora of the shoulder and is the most common microbe identified in periprosthetic shoulder infections. The purpose of this study is to determine if there is C acnes present on the incision scalpel in patients undergoing shoulder arthroplasty despite extensive skin preparation techniques to prevent wound contamination. METHODS: The authors collected a consecutive case series of patients meeting inclusion criteria. Patients were included if they underwent either primary or revision shoulder arthroplasty at the tertiary care hospital with the seni
