Insights Into the Role of Obesity in Heart Failure With Preserved Ejection Fraction: Pathophysiology and Management.

Heart failure (HF) is a significant global health issue, categorized by left ventricular ejection fraction, being either reduced (HFrEF < 0.40) or preserved (HFpEF > 0.50), or in the middle of this range. Although the overall incidence of HF remains stable, HFpEF cases are increasing, representing about 50% of all HF cases. Outcomes for HFpEF are similar to those for HFrEF, leading to substantial health-care resource use. Despite extensive research over the past 2 decades, the prognosis and mortality rates for HFpEF remain high. A key feature of HFpEF is exercise intolerance, characterized by

Does ischemic preconditioning enhance sports performance more than placebo or no intervention? A systematic review with meta-analysis.

BACKGROUND: Ischemic preconditioning (IPC) is purported to have beneficial effects on athletic performance, although findings are inconsistent, with some studies reporting placebo effects. The majority of studies have investigated IPC alongside a placebo condition, but without a control condition that was devoid of experimental manipulation, thereby limiting accurate determination of the IPC effects. Therefore, the aims of this study were to assess the impact of the IPC intervention, compared to both placebo and no intervention, on exercise capacity and athletic performance. METHODS: A system

Study of the N1-(2,3,4-Trimethoxybenzyl)-N2-{2-[(2,3,4-Trimethoxybenzyl)Amine]Ethyl}-1,2-Ethandiamine Compound Effect on the Physical Performance of Animals.

We studied the effect of N1-(2,3,4-trimethoxybenzyl)-N2-{2-[(2,3,4-trimethoxybenzyl)amino]ethyl}-1,2-ethanediamine (compound ALM-802) on the physical performance of mice after acute fatigue. The animals' performance was assessed on a treadmill. The criterion for assessing exercise tolerance was the length of the distance passed when running on a treadmill until complete fatigue. To assess the actoprotective activity of compound ALM-802, we used a method of stepwise increase in load with an initial running speed of 42 cm/sec and its subsequent increase by 5 cm/sec every 5 min. The maximum speed…

Pulmonary function, exercise capacity and dyspnea in patients 7 years after Nuss surgery.

PURPOSE: Pectus excavatum is a frequent thoracic malformation increasingly treated with minimally invasive methods (MIRPE), which are performed for cardio-respiratory problems and in some centers also for esthetic considerations. Theoretically, MIRPE may increase thoracic elastic recoil, work of breathing and cause emphysema. The aim of the present study was to determine whether teenagers who underwent MIRPE may expect normal thoracic cage development, cardio-respiratory function, exercise capacity and asymptomatic functioning. MATERIAL AND METHODS: Fifty five patients (21.1 ± 3.0 years) wh

Clinical symptoms of limited exercise capacity linked to AAI-DDD functionality: An in silico and in vivo approach.

BACKGROUND: Exercise capacity is an important aspect of quality of life in patients undergoing pacemaker implantation. Device algorithms for ventricular pacing avoidance have been developed to avoid unnecessary and potentially harmful effects of right ventricular pacing. However, little data exists on the immediate response of these algorithms to sudden AV block during exercise. METHODS: The ventricular pacing avoidance algorithms of four pacemaker manufacturers were tested in an ex-vivo model. The RSIM-1500-USB Device-Interactive Heart Simulator (Rivertek Medical Systems, Inc.) was used to s

Relationship between level of daily activity and upper-body aerobic capacity in adults with a lower limb amputation.

BACKGROUND: Previous studies show that people with lower limb amputation (LLA) have a sedentary lifestyle, reduced walking capacity, and low cardiorespiratory fitness (VO2peak). There is, however, no knowledge on the relationship between cardiorespiratory fitness and objectively measured level of physical activity in daily life. OBJECTIVES: To investigate the relationship between upper-body VO2peak, physical activity levels, and walking capacity in persons with LLA. STUDY DESIGN: Correlational and descriptive study. METHODS: Fourteen participants with LLA performed an assessment of VO2peak

Diet, Physical Activity, and Daylight Exposure Patterns in Night-Shift Workers and Day Workers.

BACKGROUND: Night-shift work has been reported to have an impact on nutrition, daylight exposure, and physical activity, which might play a role in observed health effects. Because these exposures show diurnal variation, and shift work has been related with disturbances in the circadian rhythm, the timing of assessment of these factors requires careful consideration. Our aim was to describe the changes in patterns of diet, physical activity, and daylight exposure associated with night-shift work. METHODS: We conducted an observational study among female healthcare workers either regularly wor

Beta2-agonists and exercise-induced asthma.

Beta2-agonists taken immediately before exercise provide significant protection against exercise- induced asthma (EIA) in most patients. However, when they are taken daily, there are some negative aspects regarding severity, control, and recovery from EIA. First, there is a significant minority (15-20%) of asthmatics whose EIA is not prevented by beta2-agonists, even when inhaled corticosteroids are used concomitantly. Second, with daily use, there is a decline in duration of the protective effect of long-acting beta2-agonists. Third, if breakthrough EIA occurs, recovery of lung function is sl

Mitochondrial myopathies: diagnosis, exercise intolerance, and treatment options.

Mitochondrial myopathies are caused by genetic mutations that directly influence the functioning of the electron transport chain (ETC). It is estimated that 1 of 8,000 people have pathology inducing mutations affecting mitochondrial function. Diagnosis often requires a multifaceted approach with measurements of serum lactate and pyruvate, urine organic acids, magnetic resonance spectroscopy (MRS), muscle histology and ultrastructure, enzymology, genetic analysis, and exercise testing. The ubiquitous distribution of the mitochondria in the human body explains the multiple organ involvement. Exe

Physical activity and exercise recommendations for patients with valvular heart disease.

There is a paucity of studies looking at the natural history of valvular heart disease (VHD) in exercising individuals, and exercise recommendations are largely based on expert consensus. All individuals with VHD should be encouraged to avoid sedentary behaviour by engaging in at least 150 min of physical activity every week, including strength training. There are generally no exercise restrictions to individuals with mild VHD. Regurgitant lesions are better tolerated compared with stenotic lesions and as such the recommendations are more permissive for moderate-to-severe regurgitant VHD. Indi

Exertional Dyspnoea in Chronic Respiratory Diseases: From Physiology to Clinical Application.

Dyspnoea is a complex, highly personalized and multidimensional sensory experience, and its underlying cause and mechanisms are still being investigated. Exertional dyspnoea is one of the most frequently encountered symptoms of patients with cardiopulmonary diseases, and is a common reason for seeking medical help. As the symptom usually progresses with the underlying disease, it can lead to an avoidance of physical activity, peripheral muscle deconditioning and decreased quality of life. Dyspnoea is closely associated with quality of life, exercise (in)tolerance and prognosis in various condi
