Injury is our children's greatest health problem, and pediatricians will frequently see finger and hand injuries in the office and emergency room. Many of these will be fingertip crush injuries, which are quite common in toddlers and are often undertreated. Pediatric finger and wrist fractures and sprains generally do well but require proper diagnosis and treatment. Innocent-looking wounds from glass lacerations may disguise extensive damage to underlying nerves, arteries, and tendons. Advances in microsurgery allow replantation of distal amputations even in young children and infants, althoug

Seven patients with third-degree burns of > 70% of their body had autofusions of 19 proximal interphalangeal joints. In 13 of the joints there was fixed deformity of > 60 degrees. In addition to these deformities, each of the patients had deformities due to burns in the axilla and elbows bilaterally. There was only one independently functional hand and in that hand no proximal interphalangeal joint deformity was > 40 degrees. Two patients were totally dependent because of either bilateral involvement or infectious complications of the burn itself. In four patients the hand was used as a helpin

A cadaveric study was performed on extensor tendon separation and excursion after extensor zone I (distal interphalangeal [DIP]) joint and III (proximal interphalangeal [PIP]) level lacerations and treatment. An average 1.5 mm tendon gap with 30 degrees lag following laceration at the DIP joint level and a 1.1 mm average tendon gap with 18 degrees (10 degrees-25 degrees) extension loss at the PIP level was demonstrated. Splinting zone I injuries with the DIP joints in 5 degrees hyperextension and zone III injuries with the PIP joint in 0 degrees extension eliminated tendon separation in all pa

PURPOSE: This study aimed to evaluate the clinical outcomes of surgical treatment for chronic mallet injury with severe extension lag using an anatomic reconstruction of the terminal tendon and lateral band with a palmaris longus (PL) tendon graft.
METHODS: Eleven patients with a mean age of 52 years (range, 24-82 years) who underwent surgical reconstruction for chronic tendon mallet injuries using the PL tendon were included. The harvested PL tendon was either folded longitudinally or divided into two slips. The graft was secured to the distal phalanx using a bone anchor and sutured to the s

STUDY DESIGN: Prospective cohort.
INTRODUCTION AND PURPOSE: This study evaluates the factors influencing treatment outcomes of proximal interphalangeal (PIP) joint collateral ligament injuries when treated with buddy strapping.
METHODS: Sixty-seven patients treated with buddy strapping for a PIP joint injury were enrolled. The finger range of motion (ROM), grip strength, and a Quick Disability of the Arm, Shoulder, and Hand (QuickDASH) score were assessed at 3 and 6 months after the initial injury. The factors that were assessed for their influence on the functional outcomes included age, se

BACKGROUND: Digital myxoid cysts (DMCs) are ganglia of the adjacent distal interphalangeal joint (DIPJ) caused by leakage of fluid from the joint into the surrounding tissues. The connection between the DIPJ and the cyst can be identified by the injection of methylene blue into the DIPJ. However, the injection of methylene blue into the DIPJ is difficult and time-consuming. Based on this understanding of the cause of DMCs, we have used a surgical technique to treat DMCs without the need for skin excision. Herein, we have adapted the technique and demonstrated that precise leakage point identif

PURPOSE OF THE STUDY: The study aims to evaluate the mid-term functional and subjective outcomes of treatment of the mallet finger with a bone fragment using the Ishiguro extension block pinning. The hypothesis was that this technique provides reliable and high-quality outcomes with a low complication rate.
MATERIAL AND METHODS: The study included 54 patients aged 7-17 years who underwent surgery at our department between 2017 and 2022. The inclusion criteria were the diagnosis of the mallet finger with a Doyle type IVa and IVb bone fragment, subluxation of the distal interphalangeal joint, f

We report a case of rheumatoid arthritis in which the earliest symptom was bilateral paratendonitis of the Achilles tendons, followed by paratendonitis of a finger extensor. A series of musculoskeletal ultrasonograms revealed a progression from paratendonitis of the extensor tendon to erosive synovitis of the adjacent metacarpophalangeal joint. High-resolution ultrasonography combined with power Doppler imaging seems to be a useful and convenient tool both for diagnosing paratendonitis early and for closely monitoring the progressing inflammation.

Bowstringing is a rare complication of trigger finger release caused by excessive loss of the proximal pulleys. As the flexor tendon moves away from the center of rotation of the metacarpophalangeal joint, the flexion moment arm is increased. Thus the flexors gain an increased mechanical advantage over the extensors resulting in limited digital extension. We describe a case of severe bowstringing progressing over 9 years resulting in pain, altered digital function, and loss of full extension.

When a mallet finger deformity results from an intra-articular fracture of the distal phalanx comprising more than one third of the articular surface, an accurate reduction of this fracture is necessary to prevent secondary degenerative arthritis. A technique for open reduction is described in which the distal interphalangeal joint is exposed by dividing the extensor tendon and permitting a precise reduction of the fracutre fragment. Elective division of the extensor tendon had not compromised the results.

BACKGROUND: Within hand trauma, the management of unstable, intra-articular fractures of the base of the middle phalanx remains highly controversial. The objective of this research was to determine which operation gives the best outcome in the management of displaced, intra-articular fractures of the base of the middle phalanx causing instability of the proximal interphalangeal joint.
METHODS: A scoping study was performed to determine the evidence available and the terminology used. A Systematic Review was then performed along PRISMA guidelines. This Systematic Review looked specifically for

STUDY DESIGN: Patients with Green's classification grade 2 or 3 A1-pulley trigger digit (TD) were recruited and randomized to receive the proximal interphalangeal joint-blocking orthosis (PIPJ-BO) or metacarpophalangeal joint-blocking orthosis (MCPJ-BO).
INTRODUCTION: TD is a common hand condition that can affect one's performance in activities of daily living. Conservative management of TD involves prescription of orthoses to facilitate recovery. No studies have evaluated the effectiveness of PIPJ-BO, optimal orthosis wear regime, and other factors affecting orthotic effectiveness.

Arthritis after plant injury is often apparently aseptic. We report two cases due to Pantoea agglomerans. In one case, the bacterium was isolated only from the pediatric blood culture media, BACTEC Peds Plus, monitored in BACTEC 9240, and not from the other media inoculated with the joint fluid. This procedure could help improve the diagnosis of septic arthritis.

PURPOSE OF THE STUDY: Recent anterior dislocations of the proximal interphalangeal (PIP) joint are uncommon injuries. Chronic and irreducible dislocations have been most often reported. This study aims to precise the anatomic and radiologic aspects of these lesions about three cases, and to compare these cases and their treatment with thirteen cases published in the literature between 1966 and 1994.
MATERIAL AND METHODS: Three patients presented an anterior dislocation of the PIP joint after direct or twisting traumatism. One had an avulsion of the central slip of the extensor tendon and rema

The authors recommend corrective procedures for the absence of intrinsic muscle function after irreversible injury to the ulnar nerve. The modified Zancolli capsulodesis was utilized in four cases with ulnar and in one case with combined ulnar and median nerve injury. The modification consists of a temporary fixation of the metacarpophalangeal joint with a Kirschner wire and the use of a transosseous absorbable suture. Two personal cases are reported.
