We describe a patient with severe pityriasis rubra pilaris in whom extensive extraspinal hyperostoses developed after 13 years of oral retinoid treatment. The most prominent abnormality was a bridging exostosis between the left acetabulum and collum. X-ray examinations of the spine during retinoid therapy showed no abnormalities. During oral retinoid treatment, it is important to ask the patient on a regular basis about any skeletal pains or mobility restriction. Normal spinal x-ray results are no guarantee that a patient is free of hyperostoses. Discontinuation of acitretin therapy resulted i

Chronic nonbacterial osteomyelitis (CNO) is an autoinflammatory bone disease most commonly affecting children and young people. CNO can cause bone pain, hyperostosis and fractures, thereby significantly impacting on patients' wellbeing. The molecular pathophysiology of CNO is characterized by NLRP3 inflammasome activation and a pronounced imbalance between pro- and anti-inflammatory cytokines. In the absence of clinical trials, treatment of CNO remains empiric and is based on personal experience and published case series. This project systematically reviewed the available literature in pediatr…

INTRODUCTION AND IMPORTANCE: Cervical diffuse idiopathic skeletal hyperostosis (DISH) can cause stiffness, reducing quality of life as the result of functional impairment and progressive pain. The standard treatment depends on patient's problem which often involves anterior cervical discectomy and function, but this approach can lead to ASD. Cervical disc arthroplasty (CDA) using a metal-on-metal (MoM) prosthesis may offer an alternative treatment to preserve segmental motion while alleviating symptoms.
CASE PRESENTATION: A 55-year-old man presented with progressive cervical stiffness and pai…

BACKGROUND: SAPHO syndrome is a rare chronic rheumatic disease characterized by five typical manifestations: synovitis, acne, pustulosis, hyperostosis, and osteitis. The follicular occlusion triad (FOT) comprises hidradenitis suppurativa (HS), acne conglobata (AC), and dissecting cellulitis of the scalp (DCS). The associations of FOT with peripheral/axial spondyloarthritis remain poorly understood. This study aimed to delineate the pattern of possible peripheral/axial spondyloarthritis involvement in patients with FOT.
METHODS: All cases of FOT with peripheral/axial spondyloarthritis treated …

INTRODUCTION: Adverse experiences leading to physiological disruptions (stress) in early life produce cascade effects on various biological systems, including the endocrine and metabolic systems, which, in turn, shape the developing skeletal system. To evaluate the effects of stress on adipose and skeletal tissues, we examine the relationship between skeletal indicators of stress (porotic hyperostosis [PH] and cribra orbitalia [CO]), bone mineral density (BMD), vertebral neural canal (VNC) diameters, and adipose tissue distribution in a contemporary pediatric autopsy sample.
METHODS: Data is …

Chronic non-bacterial osteitis (CNO) is an autoinflammatory bone disorder with an evolving classification and limited treatment options. It encompasses conditions like SAPHO (Synovitis, Acne, Pustulosis, Hyperostosis, Osteitis) syndrome and chronic recurrent multifocal osteomyelitis, which share overlapping clinical and radiological features. While nonsteroidal anti-inflammatory drugs (NSAIDs) serve as first-line therapy, refractory cases require second-line options such as tumor necrosis factor (TNF) inhibitors or bisphosphonates. Emerging evidence suggests Janus kinase inhibitors (JAKis), in…

Radiological examination revealed C4-5 split fractures with diffuse idiopathic skeletal hyperostosis and a suspected cervical spinal cord injury. A contrast-enhanced lesion was observed within the fracture cavity of the C5 vertebral body, which was suggestive of a pseudoaneurysm originating from the left vertebral artery. Because of a history of ba …

OBJECTIVE: To analyze clinical and radiographic features that may impact the rate of focal hyperostosis (FH) on computed tomography (CT) for primary and recurrent sinonasal inverted papillomas (IPs) as well as highlight factors that may affect concordance between FH and IP true attachment point (TAP).
METHODS: All IPs resected between 2006 and 2022 were retrospectively reviewed. CTs were read by a neuroradiologist blinded to operative details. IP with malignancy was excluded. Operative reports and long-term follow-up data were evaluated.
RESULTS: Of 92 IPs, 60.1% had FH, 25% had no CT bony c…

Synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) is a relatively rare and often underdiagnosed disorder characterized by chronic inflammation affecting the bones, joints, and skin. While the precise cause of SAPHO syndrome remains elusive, multiple factors such as genetics, immunological dysregulation, and bacterial influences have been implicated in its pathogenesis. One notable aspect of SAPHO syndrome is the wide variability of symptoms experienced by afflicted individuals. A diverse array of osteoarticular manifestations may be observed, with common sites of involvement incl…

Synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome is known as a rare disease characterized by inflammatory lesions on bones and skin. Polymorphism of clinical manifestation and lack of molecular biomarkers have both limited its diagnosis. Our study performed RNA sequencing (RNA-seq) and integrative bioinformatics analysis of long noncoding RNA (lncRNA)-messenger RNA (mRNA) profile in patients with SAPHO syndrome and healthy controls. A total of 4,419 differentially expressed (DE) mRNAs and 2,713 lncRNAs were identified (p < 0.05, fold change > 2) and a coexpression netwo…

OBJECTIVE: SAPHO syndrome (synovitis, acne, pustulosis, hyperostosis, and osteitis) is an autoimmune disease of unknown etiology that seriously affects patients' daily lives. Family-based investigations support genetic contributions toward disease susceptibility. The present study evaluated whether the previously reported autoimmune disease-associated single nucleotide polymorphisms (SNPs) have any genetic overlap with SAPHO syndrome.
METHOD: Genomic DNA was obtained from 71 SAPHO patients and 104 healthy controls. The SNP genotypes of each patient were determined with polymerase chain reacti…

OBJECTIVES: An overexpression of Tgf-β2 leads to calvarial hyperostosis and suture fusion in individuals with craniosynostosis. Inhibition of Tgf-β2 may help rescue fusing sutures and restore normal growth. The present study was designed to test this hypothesis.
DESIGN: Twenty-eight New Zealand White rabbits with delayed-onset coronal synostosis had radiopaque markers placed on either side of the coronal sutures at 10 days of age. The rabbits were randomly assigned to: (1) sham control rabbits (n = 10), (2) rabbits with control IgG (100 μg/suture) delivered in a collagen vehicle (n = 9), and …
