Prevalence and determinants of pre-lacteal feeding in South Sudan: a community-based survey.

BACKGROUND: Pre-lacteal feeding (PLF) is a barrier to optimal breastfeeding and increases the risk of diarrhoea and acute respiratory tract … to their infants. The commonest pre-lacteal feeds included glucose solution (54%), water (26%), and infant formula (14%). Having received antenatal breastfeeding counselling decreased the odds of PLF [adjusted odds ratio (AOR) 0.60; 95% CI (0.43, 0.82)]; while discarding of colostrum increased the use of pre-lacteal feeds [AOR 1.57; 95% CI (1.17, 2.11)]. CONCLU …

Relationship between breastfeeding and obesity in childhood.

A cross-sectional study was conducted to investigate the weight status and the relationship of infant-feeding variables, birthweight and birth order with BMI in a group of Iranian children. Five hundred and eleven students of both sexes at the first grade in elementary schools (aged 7 years) were recruited randomly from all 19 educational districts of Tehran. Weights and heights of children and their mothers were measured. Data on breastfeeding (BF), formula-feeding, the timing of introduction of complementary foods (CF), birthweight, and birth order were collected from the mothers. The 2007 W

Delayed onset of lactogenesis among first-time mothers is related to maternal obesity and factors associated with ineffective breastfeeding.

BACKGROUND: Delayed onset of lactogenesis (OL) is most common in primiparas and increases the risk of excess neonatal weight loss, formula supplementation, and early weaning. OBJECTIVE: We examined variables associated with delayed OL among first-time mothers who delivered at term and initiated breastfeeding (n = 431). DESIGN: We conducted in-person interviews during pregnancy and at days 0, 3, and 7 postpartum and extracted obstetric and newborn information from medical records. We defined OL as delayed if it occurred after 72 h and used chi-square analysis to examine its association with p

Vitamin D in pregnancy and lactation: maternal, fetal, and neonatal outcomes from human and animal studies.

During pregnancy and lactation, mothers require significant amounts of calcium to pass on to the developing fetus and suckling neonate, respectively. Given the dependence of adult calcium concentrations and bone metabolism on vitamin D, one might anticipate that vitamin D sufficiency would be even more critical during pregnancy and lactation. However, maternal adaptations during pregnancy and lactation and fetal adaptations provide the necessary calcium relatively independently of vitamin D status. It is the vitamin D-deficient or insufficient neonate who is at risk of problems, including hypo

Feeding schedules in hospitals for newborn infants.

BACKGROUND: Regular breastfeeding times have been thought to help establish routines and promote infant digestion, while frequent breastfeeding has been recommended to enhance breastfeeding and infant growth. OBJECTIVES: The objective of this review was to assess the effects of frequent breastfeeding compared with less frequent breastfeeding in the early days after birth. SEARCH STRATEGY: We searched the Cochrane Pregnancy and Childbirth Group trials register. SELECTION CRITERIA: Randomised and quasi-randomised trials comparing on demand or frequent breastfeeding (two or three hourly) sched

The relationship between infant feeding types, gut microbiome, intestinal inflammation, and neurodevelopment in a neonatal piglet model.

BACKGROUND: The influences of nutrition on the infant's developing gastrointestinal (GI) microbiome, intestinal tract, and brain is unclear. Human milk (HM) is associated with beneficial immune and cognitive development compared with infant formula (IF). This study used a neonatal piglet model to determine the effects of infant feeding exposures (HM vs IF) on the GI microbiome, intestinal inflammation, and brain oligodendrocyte maturation. METHODS: Six pairs of piglets received HM or IF for 28 days. Fecal samples were collected weekly and GI regions (jejunum, ileum, and colon) and brains were…

Bifidobacterium animalis subsp. lactis modulates early-life immune response and gut metabolism.

BACKGROUND: The maturation of the immune system is critical during early life, as it involves the differentiation, maturation, and establishment of immune tolerance of immune cells. This process is influenced not only by genetic factors but also by environmental factors, particularly the symbiotic microbiota. Bifidobacterium animalis subsp. lactis (BB-12), originally found in dairy products, is widely used in infant formula and dietary supplements. However, its role and mechanisms in immune development during early life remain unclear. METHODS: Using GF mice as the experimental model, B. anim…

Proteomics analysis reveals digestion-resistant proteins from colostrum are associated with inflammatory and cytotoxic responses in intestinal epithelial cells.

BACKGROUND: Although human-milk feeding reduces the risk of necrotizing enterocolitis (NEC) in preterm infants compared with formula feeding, the exact risk-reduction mechanism remains unknown. As NEC occurs at the distal small intestine in which digestion has occurred, we applied proteomics to examine the extent to which colostrum proteins survive simulated infant in vitro-digestion and, thus, have potential to exert biological function. METHODS: Ten preterm colostrum samples were left undigested or in vitro-digested, and lipopolysaccharide (LPS)-binding protein, soluble cluster of different

Application of MALDI-TOF mass spectrometry, and DNA sequencing-based SLST and MLST analysis for the identification of Cronobacter spp. isolated from environmental surveillance samples.

Cronobacter spp. are emerging infectious foodborne bacteria that can cause acute meningitis and necrotizing enterocolitis in neonates and immunocompromised individuals. Although, little is known about its reservoirs or transmission routes, it has been linked to powdered infant formula worldwide. Three Cronobacter spp. (C. sakazakii, C. malonaticus, and C. turicensis) have been described as more virulent, and isolated frequently from infant meningitis cases. The estimated mortality rates are as high as 80% in infants. Thus, surveillance and typing of Cronobacter spp. isolated from food and envi…

Association between type of feeding at discharge from the hospital and nutritional status of very low birth weight preterm infants.

The ideal feeding for premature babies has been the source of extensive debate. The aim of this study was to assess the association between type of feeding at discharge and the nutritional status of very low birth weight infants. This was a retrospective cohort of preterm babies with birth weight ≤1500 g, born between January 2006 and December 2013. The infants were divided into 3 groups according to type of feeding at discharge: exclusive breast milk (group 1), mixed feeding (group 2) and exclusive artificial formula (group 3). Frequencies of each group were calculated, as well as mean Z-scor

[Management of a case of maple syrup urine disease--the use of gluco-insulinotherapy].

We report here the treatment and poor outcome of a case of Maple Syrup Urine Disease with late diagnosis and retrieval (2 and 5 months, respectively). As the proband had quite high levels of plasmatic leucine (1956 micromol/L for a normal upper limit of 77), we started immediately with a gluco-insulin therapy to produce anabolism in the infant. When leucine has fallen to 275.3 micromol/L, we instituted feeding with branched chain amino acid-free protein and high energy from carbohydrates. After reviewing briefly the clinical, biochemical and therapeutic aspects of this disorder, we comment on

Lactose and protein absorption from breast milk and cow's milk preparations and its influence on the intestinal flora. Investigations on two infants with an artificial anus.

Lactose and protein absorption from breast milk and a cow's milk preparation enriched up to 7% of lactose were studied in two infants with an artificial anus applied in the ascending colon region. The concentrations of protein, lactose, glucose and galactose were measured in the fistula stools. In addition, the stools were analysed microbiologically. There were relatively high concentrations of lactose and its decomposition products and low concentrations of protein and aminonitrogen in the fistula stools when breast milk was fed. When the cow's milk formula was applied, only traces of lactose

Allergic proctocolitis: Evaluation and management.

Food protein-induced allergic proctocolitis (FPIAP) is a condition that causes rectal bleeding and mucous in stools, which typically presents in healthy infants within the first month of life and improves by 12 months of age. It can present in both breast- and formula-fed infants, with the most common implicated foods being cow's milk and soy protein. Currently, there are no widely accepted diagnostic criteria, but it is considered a clinical diagnosis, with FPIAP symptoms resolving with removal of culprit food(s) and recurrence with food reintroduction. Although the duration of the eliminatio

Decisions Regarding Newborn Feeding by Latina Mothers.

This study examined the factors that influence Latina mothers' decisions to initiate breastfeeding, formula feeding, or both, and when a given neonatal feeding method is decided, in order to assess the optimum time for healthcare providers to discuss nutrition decisions. A Spanish-speaking health care provider orally administered a 24-question survey to self-identified Latina mothers in the Newborn Nursery at North Carolina Children's Hospital after delivery. A total of 152 Latina mothers, aged 17 to 43 years, were interviewed from September 2014 to September 2015. Three decisive factors resul

Calcium and phosphorus needs of premature infants.

Interpretation of the available studies for the purpose of predicting the recommended Ca and P needs of LBW infants is difficult because of the number of confounding variables that can affect Ca and P metabolism. Clinically, the most important measure of Ca/P "adequacy" must also be determined. Clearly, the predicted recommended intakes for dietary Ca and P would be different if normalization of serum and urine Ca and P levels is chosen rather than achievement of intrauterine retention of mineral or bone mineral content as the index of adequacy of mineral intake. The factorial approach to the
