Liver abscess and tracheal fistula induced by transcatheter arterial chemoembolization for hepatocellular carcinoma: A case report.

BACKGROUND: Transarterial chemoembolization (TACE) is a standard treatment for intermediate-stage hepatocellular carcinoma (HCC). The complications of TACE include biliary tract infection, liver dysfunction, tumor lysis syndrome, biloma, partial intestinal obstruction, cerebral lipiodol embolism, etc. There are few reports about tracheal fistula induced by TACE. CASE SUMMARY: A 42-year-old man came to our hospital with cough and expectoration for 1 month after TACE for HCC. Laboratory test results showed abnormalities of albumin, hemoglobin, prothrombin time, C-reactive protein, D-dimer, and

[Recurrent urinary tract infection due to enterovesical fistula secondary to colon diverticular disease: a case report].

INTRODUCTION: Enterovesical fistula are pathological connections between the bladder and pelvic intestinal segments. It consists of a rare complication of neoplastic and inflammatory pelvic disorders, in addition to iatrogenic or traumatic injuries, and correlates with both high morbidity and mortality indexes. CASE REPORT: Male patient, 61 years old, admitted at the hospital clinics featuring abdominal pain and distension, vomiting and fecal retention. Patient's pathological precedents include high blood pressure, diabetes mellitus, vesical dysfunction and recurrent urinary tract infection o

Rapid closure of Crohn's disease fistulas with continuous intravenous cyclosporin A.

Fistulas complicating Crohn's disease often are refractory to medical management. Immunosuppressive therapy is sometimes effective but requires several months before efficacy can be determined. We treated five patients with Crohn's disease complicated by chronic draining fistulas unresponsive to previous surgery, steroids, antibiotics, total parenteral nutrition, 6-mercaptopurine, or azathioprine with continuous infusion cyclosporin A (CyA). The five patients had a total of 12 fistulas (five enterovaginal, three perianal, three enterocutaneous, one enterovesical). CyA was administered initiall

Anastomotic-vaginal fistula after colorectal surgery.

The most feared complication of anterior and low anterior resection is anastomotic dehiscence. Although most leakages remain clinically silent, some may lead to formation of a colovaginal fistula. At the Lahey Clinic Medical Center, the records of nine patients with colovaginal fistula as a complication of colorectal surgery were reviewed to determine clinical characteristics and optimal management. The mean age was 63.7 years (range, 47-72 years). The initial indications for surgery were carcinoma of the rectum (n = 4), diverticular disease (n = 3), and closure of the colostomy after Hartmann

Patients with anorectal malformation and Hirschsprung's disease.

BACKGROUND: Dysganglionosis and aganglionosis have been frequently described in biopsies of the distal bowel in patients with anorectal malformation (ARM). They are interpreted as a developmental disorder of the anorectum. The true association of total colonic aganglionosis and anorectal malformation has not been reported before. The aim of this study was to explore the true association of anorectal malformation and Hirschsprung's disease with or without trisomy 21. MATERIAL & METHODS: A retrospective study of all patients diagnosed with anorectal malformation in our institute from 1986-2008

[Crohn disease--surgical standards].

Surgery for Crohn's disease is restricted to the treatment of complications. Even then, nearly all patients with Crohn's disease must be operated on at least once in lifetime. Surgical concepts base on the right timing for the operation, interventional drainage of abscesses, accurate pre-operative work-up to determine the extent of inflammation, and bowel conserving operation techniques. Respecting these principles, surgery for Crohn's disease can be performed with low complication rates restoring in most cases patients' quality of life.

[Gastrointestinal interventional radiology. Technics and preliminary results].

Gastrointestinal interventional radiology allows the positioning of feeding tubes in difficult situations, as well as the balloon dilatation of stenoses and the transintestinal drainage of fistulas and collections, with some advantages over endoscopic and surgical procedures. In the present series feeding tubes were positioned in 26 patients, both to get over the strictures in the upper gastrointestinal tract and to exclude fistulous tracts or anastomotic leaks from alimentary transit. Balloon dilatation was performed in 10 patients with stenoses of different aetiologies, at different levels o

Crohn's disease.

Crohn's disease is manifested by fissure ulcers, transmural inflammation with lymphocytes, and granulomata affecting any part of the gastrointestinal tract. There seems to be a genetic predisposition, conditioned perhaps by environmental factors and possibly a virus. The disease is characterized by spontaneous fistulae, internal and external, anal lesions, stricture formation, and healing by fibrosis. The presenting symptoms are intestinal and systemic (Table 2). Systemic manifestations, such as arthritis, skin lesions, episcleritis and uveitis, pericholangitis, hydronephrosis, renal stones, a

Insight into the management of non-traumatic perforation of the small intestine.

INTRODUCTION: Management of non-traumatic perforation of the small intestine has always been a consideration for surgeons because of associated enormous morbidity and mortality. There is a paucity of data on the management of non-traumatic perforation of the small intestine. METHODOLOGY: A retrospective study was conducted which involved analysis of 192 patients treated for non-traumatic perforation of small intestine in a tertiary care teaching hospital in North India. The clinical profile and management of the patients were studied. RESULTS: The most common cause of non-traumatic perforati

Bacterial dysentery complicated with Crohn's disease: A case report.

RATIONALE: Crohn's disease is easily confused with bacillary dysentery in clinical manifestations. Crohn's disease is a chronic disease that can involve both intestinal and extraintestinal areas, and severe patients have prolonged and poor prognosis, so early diagnosis and treatment are crucial for patients. This case is a combination of bacillary dysentery and Crohn's disease, which emphasizes the differentiation between bacillary dysentery and Crohn's disease, and expects clinicians to pay attention to and discover the early manifestations of Crohn's disease. PATIENT CONCERNS: A young male

Diagnosis and treatment of multiple postoperative fistulas following resection of a giant abdominal mesenteric fibromatosis: a case report and literature review.

Mesenteric fibromatosis (MF) is a rare, locally aggressive, borderline tumor with no metastatic potential, characterized by a high postoperative recurrence rate. This paper reports a complex case of aggressive MF. Despite receiving targeted therapy that was combined with immunotherapy, the tumor in our case progressed rapidly and symptoms of intestinal obstruction developed. Following a challenging radical resection, delayed and complex multiple complications ensued, including duodenal fistula, vaginal fistula, and ureterorectal fistula. Through personalized management under multidisciplinary

Penile gangrene induced by colovesical fistula: a case report.

INTRODUCTION: Fournier’s gangrene (FG) is a progressive necrotizing soft tissue infection of the perineum, perianal region, and external genitalia caused by one or more pathogenic microorganisms. CASE PRESENTATION: This article presents a case of an 83-year-old male patient with penile FG caused by a colovesical fistula. The patient was admitted with penile swelling and necrosis and had a prior history of a colovesical fistula that had not been adequately treated. Upon diagnosis, comprehensive treatment was administered, including antimicrobial therapy, and anti-shock therapies. Surgical inter

Turnbull-Cutait Pull-Through Procedure Is an Alternative to Permanent Ostomy in Patients With Complex Pelvic Fistulas.

BACKGROUND: A permanent stoma is frequently recommended in the setting of complex or recurrent rectovaginal fistulas because of the high failure rate of reconstructive procedures. The Turnbull-Cutait pull-through procedure is a salvage operation for motivated patients desiring to avoid permanent fecal diversion. OBJECTIVE: To analyze the cure rates of complex rectovaginal fistulas after the Turnbull-Cutait pull-through procedure based on cause. DESIGN: After the institutional review approval board, a retrospective review of women who underwent the procedure (1993-2018) for a rectovaginal fis

Aorto-duodenal fistula secondary to aortic graft replacement.

Secondary aorto-duodenal fistula (SADF) is a rare and serious event occurring in up to 45% of aortic prosthesis infections. The clinical manifestations are variable ranging from isolated signs of graft infection such as fever to massive gastrointestinal bleeding. The diagnosis is based on CT scan and is generally oriented by an inconstant association of indirect signs. Despite a high early severe postoperative morbidity and mortality, especially in presence of a preoperative shock, emergency surgery allows for the diagnosis and treatment of SADF with multidisciplinary management allowing favor
