Optimizing medial patellofemoral ligament reconstruction: a randomized trial of 3D-printed guide plates for enhanced femoral tunnel accuracy and early functional recovery.

This study aimed to determine whether patient-specific 3D-printed guide plates enhance femoral tunnel placement accuracy, reduce operative time, and promote faster functional and kinematic recovery in medial patellofemoral ligament (MPFL) reconstruction compared to conventional techniques. Prospective, randomized controlled trial. Single academic tertiary care hospital. Sixty patients diagnosed with recurrent patellar dislocation were enrolled and randomly assigned to either a 3D-printed guide plate group (n = 30) or a conventional fluoroscopy-guided group (n = 30). Patients in the interventio

Long-term follow-up study on the Maquet procedure with special reference to the causes of failure.

Thirty-nine patients with symptomatic osteoarthrosis (OA) of the patellofemoral joint were treated with 42 anterior tibial tubercle elevations, also known as the Maquet procedure. The patients all had symptomatic OA of the patellofemoral joint that fell into one of three categories: old patellar fracture, chronic patellar subluxation, or postpatellectomy pain. In the latter group, the OA was manifested by erosions of trochlear articular cartilage from the articulating quadriceps tendon-patellar ligament suture line. All 39 patients had a 1.5-2.5-cm tibial tubercle elevation with medial displac

A Rare Case of Transepiphyseal Distal Femur Fracture Dislocation With Delayed Recovery of a Common Peroneal Nerve Injury.

The aim of this study is to bring attention to a unique case and our approach to treatment in this context. We describe a case of an 11-year-old male who presented to us with an injury to his left knee following trauma with pain, swelling, shortening and deformity for one day. An X-ray revealed a transepiphyseal fracture dislocation of the left distal femur (Salter-Harris type 1 injury) and neurovascular examination was conclusive of foot drop which pointed towards injury to common peroneal nerve (CPN). The patient was taken up for closed reduction with percutaneous pinning under mobile C-arm

The patello-femoral joint--a critical appraisal of its geometric assessment utilizing conventional axial radiography and computed arthro-tomography.

In the quest for treatable causes of anterior knee pain, plain film skyline views of the patello-femoral joint are requested, often in 30 degrees, 60 degrees and 90 degrees of flexion, to assess the functional relationships of the joint. Patello-femoral malalignment predisposes to recurrent subluxation and dislocation, articular cartilage damage and premature degenerative change. The aim of this study is to evaluate critically, by comparative assessment, the information provided by skyline views and axial computed arthro-tomography (CTA). Measurements of the patello-femoral angle (assesses pat

Patellofemoral Pain and Instability in Adolescent Athletes.

Injuries and disorders of the patellofemoral joint in the adolescent athlete can encompass a wide spectrum of symptomatology and pathology. Anterior knee pain is a common presenting symptom in sports medicine clinics, and can have numerous underlying etiologies. This activity-related pain may be the manifestation of enthesopathy, tendinopathy, fat pad impingement, or numerous other conditions, but is more commonly related to more subtle skeletal and muscular imbalances. Treatment is typically nonoperative in nature, and excellent results are reported with physical therapy. Patellofemoral insta

Caring for Wrestlers.

Wrestling is a popular high school and college sport with an injury and illness rate second only to football. It is important that the physician providing medical care for wrestlers be familiar with the unique characteristics of wrestling and the associated common injuries and medical problems. Common orthopedic injuries include shoulder, elbow, and finger dislocation; prepatellar bursitis; knee medial collateral ligament sprains; and cervical strains. Skin infections are the most common cause of missed mat time for wrestlers. Physicians need to be able to identify and treat these infections,

Primary versus secondary distal femoral arthroplasty for treatment of total knee arthroplasty periprosthetic femur fractures.

Current methods of fixing periprosthetic fractures after total knee arthroplasty (TKA) are variable, and include open reduction and internal fixation (ORIF) via plating, retrograde nailing, or revision using standard revision TKA components or a distal femoral arthroplasty (DFA). The purpose of this study is to compare patients who failed plating techniques requiring subsequent revision to DFA to patients who underwent primary DFA. Of the 13 patients (9.2%) who failed primary ORIF, causes included nonunion (53.8%), infection (30.8%), loosening (7.7%), and refracture (7.7%). There were signific

Vascular injury associated with fracture-dislocations of the lower extremity.

Vascular damage occurring in conjunction with orthopedic injury of the lower extremity is uncommon. This relative rarity is further complicated by the potentially subtle nature of vascular injury. Doppler signals and even palpable pulses do not exclude vascular damage. Missed or delayed diagnosis with a subsequent avoidable amputation is a too-frequent result. Displaced fractures of the distal femur and proximal tibia and particularly dislocation of the knee are associated with the highest risk of concomitant vascular injury and the poorest collateral circulation to support the distal limb. Evaluation of patello-femoral alignment using MRI.

In evaluating patello-femoral alignment, it is not enough to assess the conformity of the joint two-dimensionally, the direction of the extensor mechanism must be examined three-dimensionally. Using magnetic resonance imaging (MRI), we adopted an ideal extensor mechanism plane intersecting the patellar facet of the femur centrally and perpendicularly. We evaluated the alignment according to the extent to which the patella and/or the tibial tuberosity deviated from the plane. The results suggested that our method is useful for indicating proximal and distal realignment. Furthermore, two-dimensi

[Patellar imbalance and external rotary instability of the knee].

The importance of external hyperrotation and external rotary instability in patellar imbalance is examined. In view of the progressive nature of patellar imbalance and of the predominant part played by the muscular and capsulo-ligamentous elements in the stability of the extensor apparatus, the authors advocate an earlier and more radical surgical approach with a view to improving the patellar medialization forces (transplantation of the intermediate great muscle and retension of the lateral patellar retinaculum) as well as the elements of active internal rotation of the tibia (modified versio

Treating patella fractures with a fixed-angle patella plate-A prospective observational study.

INTRODUCTION: Anterior tension wiring using Kirschner wires (K-wires) is still considered the standard treatment for patella fractures, despite its high complication rate. The objective of this prospective clinical study was to evaluate intra- and perioperative complications as well as the clinical outcome of patients with patella fracture treated with a new developed bilateral, polyaxial, fixed-angle 2.7mm patella plate. PATIENTS AND METHODS: Between 2011 and 2014 all patients with a patella fracture were included in this prospective study and treated with a fixed-angle patella plate. Avulsi

Spondyloepimetaphyseal dysplasia with joint laxity, leptodactylic type: longitudinal observation of radiographic findings in a child heterozygous for a KIF22 mutation.

Spondyloepimetaphyseal dysplasia with joint laxity, leptodactylic type (SEMDJL2), is a rare disorder due to a KIF22 gene mutation and characterized by postnatal short stature, midface hypoplasia and generalized ligamentous laxity. Radiologic hallmark includes severe involvement of the epiphyses and the slender appearance of the metacarpals and phalanges. The aim of the study was to evaluate radiologic findings of SEMDJL2 in a child followed from age 2 years 9 months to 11 years. Using whole-exome sequencing, we identified a single nucleotide de novo p.Pro148Leu mutation in the KIF22 gene. The

Lower limb salvage surgery: modular endoprosthesis in bone tumour treatment.

We retrospectively analysed 90 patients who underwent "en bloc" resection and modular endoprosthesis reconstruction in the lower limbs between 1987-2003. After proximal femur resection, reconstruction was performed with a modular endoprosthesis by Howmedica (KFTR, designed by Kotz) and modular revision endoprosthesis by W. Link or Lima-Lto (Revision system, designed by Wagner). The knee joint was reconstructed with a modular endoprosthesis (Howmedica, KFTR designed by Kotz) after distal femur or proximal tibia resection. Malignant bone tumours were present in 58 patients (64.5%), benign tumour

Triplicate post-traumatic sciatic nerve palsy: evoked potentials in the diagnosis.

Three episodes of sciatic nerve palsy occurred after open reduction and internal fixation of a fracture-dislocation of the left hip and pelvis in a 20-year-old female injured in a motor cycle accident. There were also ipsilateral open fractures of the tibia and fibula and an open knee injury. When the palsy first developed the patient was in a hip spica plaster cast extending from the costal margin to encase the whole left lower limb. At the time of the second and third episodes of palsy she was in a left below-knee cast and it was not possible to fully assess the function of involved muscles

Outcomes After Isolated Medial Patellofemoral Ligament Reconstruction for Recurrent Patellar Instability: Influence of Persistent Postoperative Apprehension and J-Sign.

BACKGROUND: Patients with recurrent instability underwent isolated medial patellofemoral ligament (MPFL) reconstruction regardless of anatomic risk factors within the confines of strict exclusion criteria. Previous publications from this cohort have been limited in the ability to identify risk factors for failure of isolated MPFL reconstruction when recurrent instability was defined as the only mode of failure, likely because of the small patient numbers who experienced this outcome. PURPOSE: To investigate whether persistence of postoperative apprehension and J-sign indicate suboptimal outco
