[Clinical application of artificial condylar process for reconstructing temporomandibular joint].

OBJECTIVE: To assess the feasibility and clinical outcomes of artificial condylar process in reconstruction of the temporomandibular joint. METHODS: Between January 2005 and January 2010, the reconstructions of the temporomandibular joints with artificial condylar process were performed in 10 cases (11 sides, including 7 left sides and 4 right sides). There were 7 males and 3 females with an average age of 50 years (range, 40-68 years). Mandibular condyle defects were caused by mandible tumor in 7 patients with a mean disease duration of 15 months (range, 9-24 months) and by bilateral condyla

Combined fibular osteocutaneous and omental flaps.

BACKGROUND: The free vascularized fibular bone graft, first described in 1975, has become one of the most commonly used bone grafts. It is useful for reconstructing mandibular and traumatic long bone defects and defects following tumor resection. This flap, however, does not have a long pedicle and does not have very much volume. The authors report a solution to these problems through the use of an omental flap as a bridge. METHODS: Over a 3-year period, nine patients were treated by means of a free fibular osteocutaneous flap with an omental flap. These cases required bone reconstruction wit

Swallowing outcomes after microvascular head and neck reconstruction: a prospective review of 191 cases.

BACKGROUND: The use of microvascular free tissue flaps tailored specifically to the ablative surgical defects has allowed precise anatomic reconstructions to be performed and, in turn, has improved patient outcomes. We report here the postoperative swallowing outcomes of patients undergoing microvascular reconstructions for a range of head and neck defects at the Cleveland Clinic. METHODS: The study includes 191 consecutive reconstructions for varied defects. All patients were reconstructed with four specific microvascular flaps based on their surgical defect, and postoperative swallowing out

Unraveling the insect head: Segmentation and pancrustacean homologies.

The segmental composition of the arthropodan head is a long-standing biological question. This is particularly controversial in insects where considerable fusion and modification obfuscate the postembryonic segmentation. Here we propose a reinterpretation of the insect head morphology and segmentation, which also accords with current genetic and embryological data. The new interpretation of a generalized insect head is based on the discovery of intercalary segment remnants (the intercalates), thanks to the use of 3D-reconstruction of a Cretaceous thripidan and to a reassessment of the 'anterio

A Clinically Silent Mandibular Odontogenic Keratocyst in a Geriatric Patient: Diagnostic and Therapeutic Insights.

Odontogenic keratocyst (OKC) is an aggressive, benign, cystic lesion arising from the mandible. A 65-year-old male presented with increasing left jaw swelling for 20 years was diagnosed as OKC with the help of the radiological and pathological correlation. A computed tomography scan revealed the well-defined expansile solid-cystic lesion involving the left mandible with multiple foci of calcification and cortical breach. The patient was managed surgically with segmental mandibulectomy and mandibular reconstruction. Histopathology revealed the fibrous cyst wall lined by keratinized stratified s…

Virtual Surgical Planning and Three-Dimensional Printing to Aid the Anatomical Reduction of an Old Malunited Fracture of the Mandible.

Mandible fracture is a common injury in maxillofacial surgery. It causes not only maxillofacial dysfunction but also facial deformities. Malunited fractures of the mandible have been a vast challenge in clinical treatment due to the misalignment of the broken ends and the occurrence of occlusal disorders. This case report describes using virtual surgical planning and three-dimensional printing to treat a patient with malunited fracture of the mandible. Failing to perform mandibular surgery due to severe brain trauma after the car accident, the patient got malunited healing of mandible. The aut

Orthognathic surgery with two-segment le fort i and sagittal split ramus osteotomies of open bite deformity in an amelogenesis imperfecta patient via virtual planning: A case report.

Amelogenesis imperfecta (AI) is an enamel defect and is often associated with the anterior open bite (AOB) and transverse maxillary deficiency. It is known that in such cases when AI and AOB appeared together, posterior maxillary impaction with or without bilateral mandibular ramus osteotomies is a frequently preferred treatment option. Virtual planning is more reliable rather than the conventional model surgery planning, especially for complicated cases. Usage area of virtual 3D anatomical models reconstructed from Cone Beam Computed Tomography (CBCT) data is expanding day by day for both dia

Mandibular nerve fascicular cross-face for sensitive recovery after mandibulectomy: a new technique.

Trigeminal nerve damage after mandibulectomy is a condition that may occur not infrequently when oncologic resections of the maxillo-facial district are performed. In the last decades microsurgery has made possible effective osteomuscular reconstructions using vascularised free flaps. Nevertheless, this procedure, if assuring satisfactory results, involved the sacrifice of the mandibular nerve. Nowadays, supramicrosurgical innovation has shifted the gold-standard to a higher level, allowing the complete sensitive recovery after mandibulectomy. The cross-face nerve transfer technique is an inno

Giant hemangiopericytoma of mandible: a propos of a case: a variant of the surgical technique for protection of the articular fosse.

The hemangiopericytoma is a tumor of the pericytes. It represents 1% of vascular tumors. It is frequently located in inferior limbs, the pel … s and destruction of the right jaw. A right hemimandibulectomy including the condyle was performed. Mandibular reconstruction was performed with titanium plates with condyle and bone graft of the iliac crest; a piece of silastic with titanium mesh was placed in the temporomandibular articular fossa. Two years after surgery, good aesthetic and functional results were eviden …

Auriculo-condylar syndrome is associated with highly variable ear and mandibular defects in multiple kindreds.

We report on 20 individuals in 4 kindreds with auriculo-condylar syndrome (ACS), an autosomal dominant disorder characterized by congenital auricular clefts, mandibular condyle hypoplasia, temporomandibular joint (TMJ) abnormalities, micrognathia, microstomia, and a round facial appearance with prominent cheeks. Affected individuals have varying degrees of glossoptosis, respiratory distress, masticatory abnormalities, and malocclusion. Data from these families and those previously described suggest that this is a unique disorder with widely variable expression, including lack of obvious extern

Improved mandible function after hemimandibulectomy, condylar head preservation, and vascularized fibular reconstruction.

Temporomandibular joint dysfunction after tumor extirpation of the hemimandible is a frequent sequela after condylar head reconstruction. Condylar head resection is often performed because of oncological and vascular considerations. Recent studies have demonstrated that malignancies of the mandibular ramus and body rarely involve the condylar head, and that the vascularity and supportive structures of the condylar head are sufficient to maintain viability and function. This study demonstrates that temporomandibular joint function is preserved after hemimandibulectomy without resection of the c

Gorham's disease affecting the maxillofacial skeleton.

Twenty-one cases of Gorham's disease in the maxillofacial region are described in the literature. This case is an additional one. The disease has a slight male predominance. All cases involved either the mandible alone or in association with the maxilla or other bones of the head and neck. The most common symptoms were pain, tooth mobility, mandibular pathological fractures, and facial deformity. The mean duration of symptoms prior to diagnosis was 6.4 years. Treatment included surgery, radiotherapy, and various medications, alone or combined. Four patients died of their disease. The diagnosis

Hemifacial microsomia: priorities and sequence of comprehensive otologic management.

Hemifacial microsomia is evidenced by the clinical spectrum of microtia, mandibular deformities, and middle ear malformations. This constellation of defects suggests that the most efficacious treatment will be multidisciplinary management, which includes the otolaryngologist, audiologist, plastic surgeon, and temporal bone radiologist. In the past, microtia--the most clinically apparent malformation--usually has been studied and treated with respect to the area of special interest of the investigator, whether from an otologic, reconstructive, or radiologic perspective. Newer management, howeve

Accuracy of digital templates for guidance of custom-made total temporomandibular joint replacement.

This study evaluated the application of digital templates to guide custom-made total temporomandibular joint (TMJ) replacement. Patients treated with a custom-made total TMJ prosthesis for TMJ osteoarthrosis, ankylosis, or tumours were included prospectively. Before surgery, two types of digital template (articular eminence and condyle neck templates) were designed to guide the bone osteotomy and prosthesis positioning. The reconstructive outcomes were assessed through clinical examinations and accuracy analysis by superimposing the postoperative three-dimensional craniomaxillofacial model ont

Treating Parry-Romberg Syndrome Using Three-Dimensional Scanning and Printing and the Anterolateral Thigh Dermal Adipofascial Flap.

BACKGROUND: As three-dimensional technology becomes more ubiquitous, many plastic surgical applications have emerged. The authors investigate a three-dimensional scanning and printing system for facial soft tissue reconstruction in conjunction with an anterolateral thigh dermal adipofascial flap for the treatment of Parry-Romberg syndrome. METHODS: Seven patients with facial atrophy of the zygomatic, buccal, and mandibular areas were included. Three-dimensional scanning of each patient's face in conjunction was analyzed with computer-aided design (CAD) to quantify areas of facial asymmetry. M

Tongue reconstruction: concepts and practice.

Total or subtotal tongue resection results in the potential for severe speech and swallowing disruption and life-threatening aspiration. This report documents the development of a new design for latissimus dorsi flaps used in tongue reconstruction. In order to create a contractile muscle sling which will raise the neotongue toward the palate for speech and swallowing, the flap is harvested with muscle fibers oriented transverse to its long, skin component axis. The flap is then transferred to the oral and oropharyngeal defect and sutured at the level of the mandibular angle to the remaining mu

Congenital syngnathia: a case report of osseous fusion between the maxilla and mandible.

Congenital fusion of the maxilla and mandible is an extremely rare craniofacial anomaly that can present at birth with trismus and feeding difficulties. We report a case of a neonate with right-sided osseous fusion between the maxilla and mandibular body, initially unrecognized at multiple centers. The patient developed severe hyperbilirubinemia and dehydration due to impaired feeding. Computed tomography with three-dimensional reconstruction confirmed the diagnosis and revealed no involvement of the temporomandibular joint or tooth buds. Ultrasonic osteotomy was used to resect the fusion, res
