Prediction of axillary lymph node status in breast cancer patients by use of prognostic indicators.

BACKGROUND: If axillary lymph node status of breast cancer patients could be accurately predicted from basic clinical information and from characteristics of their primary tumors, then many patients could be spared axillary lymph node dissection. Tumor size alone does not allow the identification of groups with very low or high risk of being axillary node positive. PURPOSE: Our goal was to investigate the possibility of using prognostic indicators to predict axillary node status of patients with primary breast cancer. METHODS: Data from 26,683 patients from the National Breast Cancer Tissue

Psychological expectancy as mediator of vulnerability to alcoholism.

Alcohol expectancy has proven to be a powerful predictor of drinking behavior, including alcoholism, in a wide range of groups. Three recent studies that begin to address expectancy's relation to other alcoholism vulnerability factors are reviewed. Results indicate that: (1) expectancies for reinforcement from alcohol predate teens' first drinking experiences; (2) expectancies predict subsequent drinking onset and problem drinking; (3) high initial expectancies lead to a vicious cycle of progressively more drinking and more positive expectancies during the adolescent years; (4) expectancy medi

Projections of the probability of the "Elfsteden" ice skating tour under future climate scenarios using lake models.

The Elfstedentocht is a culturally significant long distance ice skating tour held in Friesland, the Netherlands. The frequency of the tour has been declining over the last century due to increasingly mild winters. This study investigates the probability of an "Elfsteden" ice skating tour under different future IPCC climate scenarios: SSP1-2.6 (low emissions), SSP2-4.5 (intermediate emissions) and SSP5-8.5 (high emissions). Two different ice growth models are employed to produce projections of ice thickness for the future. The ice growth models are driven by sub-daily meteorological forcing fr

Patient-specific 3D-printed spinal models improve pedicle screw placement accuracy in severe kyphoscoliosis: a comparative study.

Accurate pedicle screw placement in severe kyphoscoliosis remains a major challenge due to distorted and rotated spinal anatomy. This study aims to evaluate whether the use of patient-specific 3-dimensional (3D) printed spinal models improves pedicle screw placement accuracy.  Methods: Fourteen patients with severe kyphoscoliosis (Cobb angle >100°) were included in the study. The study group (n = 7) underwent surgery using patient-specific 3D-printed spine models to assist freehand pedicle screw placement, while the control group (n = 7) underwent surgery using the standard freehand technique

A Retrieval-Augmented Natural Language Interface for Data Description and Meta-Analysis in the Pathogens-in-Foods (PIF) Database.

Food-safety occurrence databases are increasingly important for surveillance, evidence appraisal, and quantitative risk assessment, yet their routine analytical use remains constrained by the need for database literacy and statistical programming. Building on the curated and harmonized Pathogens-in-Foods (PIF) database, we developed and evaluated a retrieval-augmented natural-language interface designed to support grounded querying and reproducible evidence synthesis. The system includes two complementary modes: an Open Chat Mode for exploratory, tool-mediated interrogation of the database and

Population-specific femur models: A step towards improved osteosynthetic biomechanical testing in orthopaedics.

BACKGROUND: Biomechanical testing using synthetic bone surrogates has become a standard method for evaluating osteosynthesis techniques. However, these surrogates often fail to account for population-specific variations in bone anatomy and mechanical properties, leading to limitations in predicting clinical outcomes. This study addresses this gap by developing and validating a population-specific synthetic femur model for older women of European ethnicity, incorporating variations in geometry and mechanics observed in this demographic. METHODS: The femur model was developed using a statistica

Evaluating the predictive performance of malaria antibodies and FCGR3B gene polymorphisms on Plasmodium falciparum infection outcome: a prospective cohort study.

BACKGROUND: Malaria antigen-specific antibodies and polymorphisms in host receptors involved in antibody functionality have been associated with different outcomes of Plasmodium falciparum infections. Thus, to identify key prospective malaria antigens for vaccine development, there is the need to evaluate the associations between malaria antibodies and antibody dependent host factors with more rigorous statistical methods. In this study, different statistical models were used to evaluate the predictive performance of malaria-specific antibodies and host gene polymorphisms on P. falciparum infe

On the modeling, construction, and evaluation of a probabilistic atlas of brain perfusion.

To detect subtle, abnormal perfusion patterns in brain single photon emission computer tomography (SPECT) images, it is necessary to develop quantitative methods in which computer-aided statistical analysis takes advantage of information present in databases of normal subjects. The purpose of this study was to evaluate and examine aspects of the creation and the modeling power of three statistical models for representing brain perfusion as observed in ECD-SPECT. The first model is a local model of voxel-by-voxel mean and variance. The second model is a PCA-based global model that accounts for

Early Life Exposure To Multiple Metals, Nutrition, and Growth in Children - A Scoping Review.

BACKGROUND: In utero and childhood exposure to toxic metals is associated with poor child growth, a predictor of adverse health outcomes. Most existing research focuses on exposure to single metals; the effects of metal mixtures largely remain understudied. Further, few studies consider how diet/nutrients interact with metal mixtures. OBJECTIVE: To synthesize research on the relationship between in utero and childhood metal mixture exposures, nutritional status-metal exposure interactions, and child anthropometric outcomes. METHODS: PubMed and Embase were used to search literature published

Differentiating between appraisal process and product in cognitive theories of posttraumatic stress.

Biased appraisal is central to cognitive theories of posttraumatic stress, but little research has examined the potentially distinct meanings of the term. The ongoing process of appraising social information and the beliefs that emerge as products of that process can be distinguished conceptually. This study sought to examine whether these 2 meanings are empirically distinct as well, and if so, to begin exploring potential relations between these appraisal constructs and posttraumatic stress symptoms. Soldiers (N = 424) preparing for deployment to Iraq or Afghanistan were administered measures

Weight suppression predicts weight change over 5 years in bulimia nervosa.

Recent studies suggest that weight suppression (WS), defined as the discrepancy between current and highest past weight, predicts short-term weight gain in bulimia nervosa (BN) during treatment. The current study was designed to build on this preliminary work by examining the relation between WS and long-term weight change in BN. Treatment-seeking women (N=97) with DSM-IV BN participated in a naturalistic longitudinal follow-up study of eating disorders. At intake, height and weight were measured and highest past weight was assessed. Self-reported weights were collected every 6 months for 5 ye

Radiobiological impact of reduced margins and treatment technique for prostate cancer in terms of tumor control probability (TCP) and normal tissue complication probability (NTCP).

Dose escalation in prostate radiotherapy is limited by normal tissue toxicities. The aim of this study was to assess the impact of margin size on tumor control and side effects for intensity-modulated radiation therapy (IMRT) and 3D conformal radiotherapy (3DCRT) treatment plans with increased dose. Eighteen patients with localized prostate cancer were enrolled. 3DCRT and IMRT plans were compared for a variety of margin sizes. A marker detectable on daily portal images was presupposed for narrow margins. Prescribed dose was 82 Gy within 41 fractions to the prostate clinical target volume (CTV)

The cognitive behavioural model for eating disorders: a direct evaluation in children and adolescents with obesity.

OBJECTIVE: The cognitive behavioural model of bulimia nervosa [Fairburn, C.G., Cooper, Z., & Cooper, P.J. (1986). The clinical features and maintenance of bulimia nervosa. In K.D. Brownell, and J.P. Foreyt (Eds.), Handbook of eating disorders: physiology, psychology and treatment of obesity, anorexia and bulimia (pp. 389-404). New York: Basic Books.] provides the theoretical framework for cognitive behaviour therapy of Bulimia Nervosa. For a long time it was assumed that the model can also be used to understand the mechanism of binge eating among obese individuals. The present study aimed to t

A semiparametric method for addressing underdiagnosis using electronic health record data.

Effective treatment of medical conditions begins with an accurate diagnosis. However, many conditions are often underdiagnosed, either being overlooked or diagnosed after significant delays. Electronic health records (EHRs) contain extensive patient health information, offering an opportunity to probabilistically identify underdiagnosed individuals. The rationale is that both diagnosed and underdiagnosed patients may display similar health profiles in EHR data, distinguishing them from condition-free patients. Thus, EHR data can be leveraged to develop models that assess an individual's risk o

Risk assessment in urgent/emergent coronary artery surgery.

A statistical model has been developed to allow for prediction of individual patient prognosis following urgent/emergent coronary artery bypass grafting (CABG). None of the models previously described for use in coronary artery surgery has been tested with a prospective patient series that confirms the true predictive capacity of the model. Ideally, the predictive ability of such models should be validated with prospective trials. To examine the feasibility of statistical modeling in this clinical context, a computerized model based on the theorem of Bayes was developed to predict operative mo

Antimicrobial susceptibility of Escherichia coli F4, Pasteurella multocida, and Streptococcus suis isolates from a diagnostic veterinary laboratory and recommendations for a surveillance system.

Antimicrobial susceptibility data on Escherichia coli F4, Pasteurella multocida, and Streptococcus suis isolates from Ontario swine (January 1998 to October 2010) were acquired from a comprehensive diagnostic veterinary laboratory in Ontario, Canada. In relation to the possible development of a surveillance system for antimicrobial resistance, data were assessed for ease of management, completeness, consistency, and applicability for temporal and spatial statistical analyses. Limited farm location data precluded spatial analyses and missing demographic data limited their use as predictors with

Protocol for evaluating mechanistic pathways associated with HIV acquisition via nested Least Absolute Shrinkage and Selective Operator analysis.

Statistical analysis to evaluate mechanistic pathways can be limited by non-causal associations as well as co-linearity of high-dimensional data. Here, we present a protocol evaluating statistical associations between multiple exposure variables (sociodemographic and behavioral), immune biomarkers, and HIV acquisition. We describe steps for study setup, combining Least Absolute Shrinkage and Selective Operator with the standard regression approach, and building nested models. This approach can determine to what extent associations between risks for exposure contributes to HIV acquisition with
