Giant-cell tumour of the tendon sheath (GCTTS) is a soft tissue tumour that may invade bone, causing an intrinsic osseous lesion or instability on radiographs. A case with scaphoid instability caused by a histologically-confirmed neighbouring GCTTS has rarely been described in the literature. No definite and radical method is available for the treatment of GCTTS. This report describes an unusual case of a 22-year-old woman who previously experienced a GCTTS in her right elbow, which was removed 10 years earlier. Currently, she presented with an enlarged painless right wrist mass with focal swe

Preiser's disease or aseptic necrosis of the scaphoid is a rare condition whose treatment, whether surgical or non-surgical, is not yet well defined. Its etiology remains unknown, and the treatment options depend on the disease's progression. Two classifications summarize the progress of this condition; the first by Hebert has four stages based on radiographic findings and the other by Kalainov defines two types according to extent of necrosis on MRI. A review of literature and our experience has led us to propose a decision-making algorithm for its therapeutic management, from conservative tr

The medial femoral trochlea flap is a chondrocorticoperiosteal flap. It has recently been described in the context of cartilage lesions. It is mainly used for reconstruction in non-union of the scaphoid's proximal pole and Kienböck disease. The medial femoral trochlea flap may be harvested with a skin island flap to monitor its vascularization. The surgical technique has been extensively described for its primary osteochondral form. However, the corticoperiosteal form has not been thoroughly studied and described. Here, we describe and report the early results of three cases of the composite m

Although based on their long-term clinical evolution there appeared to be no doubt that fractures of the scaphoid modify the mechanical behavior of the carpus, the mechanisms of these modifications have not yet been investigated. This study based on finite element analysis provides insight into the sequence behind the onset of arthritis of the wrist, highlighting the existence of pressure peaks at the nonunion and at the midcarpal interface (scaphoid-capitate and lunate-capitate). This evidence explains the clinical evolution of nonunions of the scaphoid. Our study indirectly demonstrates the

PurposeThis study aimed to evaluate the clinical and radiological outcomes of lunate excision scaphocapitate fusion (LESCF) using a single headless cannulated compression screw in patients with advanced Kienböck disease (Lichtman IIIb-IV). The hypothesis was that this technique would provide significant pain relief, functional improvement, and comparable union rates to multi-screw methods while reducing implant-related morbidity.MethodsA retrospective single-center case series included 24 patients (8 males, 16 females; mean age 37.3 ± 8.5 years) who underwent LESCF between June 2021 and June 2

BACKGROUND: Percutaneous scaphoid fixation through either a volar or dorsal approach has the advantage of minor soft tissue damage compared with ORIF, and faster fracture union compared with conservative treatment. However, this technique demands highly intraoperative reliance on X-ray control, including increased radiation exposure and all associated side effects.
PURPOSE: To test the possibility and efficacy of volar percutaneous scaphoid screw placement under minimalradiation exposure.
METHODS: The sample included 20 hands (seven left, 13 right) from human adult cadavers. For this study,

We present the case of a 37-year-old, multiply injured man who sustained a bilateral perilunar fracture dislocation after a 6 m fall. The fracture dislocations were diagnosed by standard radiographs. The right side was initially treated by closed reduction and external fixation, the left side by closed reduction and splint immobilization. The patient received definite treatment 5 days after the initial injury by open reduction and internal fixation using suture anchors and temporary K-wire fixation which were removed after 6 weeks. The patient achieved almost a full range of motion in both han

PURPOSE: Scaphoid fractures are often complicated by nonunion, but incidence, risk factors, and management have not been well-characterized for pediatric patients.
METHODS: Patients aged <18 years with scaphoid fracture were identified in the 2015 to 2023 PearlDiver M170 database. Patient and fracture characteristics, imaging, and surgeries were extracted. Predictive factors for nonunion were characterized with multivariable analyses. Among patients with nonunion, characteristics of those receiving surgical and nonsurgical management were compared with multivariable analyses.
RESULTS: Of 26,

PURPOSE: Scapholunate ligaments (SLLs) play a well-established role in maintaining carpal alignment and kinematics, and are innervated with sensory mechanoreceptors located within the ligaments. They are involved in the afferent arc of dynamic wrist stability. The aim of this study was to describe the changes in these mechanoreceptor populations in injured SLLs.
METHODS: Injured SLLs were collected from human wrists at the time of SLL reconstruction or limited wrist fusion, where the ligament remnants would otherwise be discarded. These specimens were formalin-fixed and paraffin-embedded for

PURPOSE: This meta-analysis aimed to investigate the effectiveness of conservative and surgical treatments of scaphoid fracture.
METHODS: The literature databases of Pubmed, Cochrane library, and Embase were searched in March 2022. This work extracted the data based on healing time, grip strength, range of wrist motion, nonunion, time before returning to work, and complications (including persistent pain, malunion of the fracture, wound infection, scar sensitivity, hypertrophic scar, and implant-related complications). Stata 14.0 software was used for statistical analysis.
RESULTS: Twelve RC

INTRODUCTION: In posttraumatic wrist arthritis of intermediate severity, two main palliative procedures are used to preserve some mobility in the wrist: proximal row carpectomy (PRC) and scaphoid excision followed by four corner arthrodesis (4CA). Despite satisfactory overall results, the debate continues about which one provides the best results and long-term outcomes, particularly prevention of arthritis progression. Recent comparative studies now provide us with information about mid- and long-term results. The aim of this study was to compare the clinical results, complications, conversion

BACKGROUND: Suspected scaphoid fracture (SF) after a fall on an outstretched hand is a common presentation in the emergency department. Magnetic resonance imaging (MRI) or computed tomography (CT) has been suggested to assist in the diagnosis or exclusion of SF.
PURPOSE: To compare MRI and CT at diagnosing occult SFs.
MATERIAL AND METHODS: We routinely perform CT scans in patients with clinically suspected occult SF, after 7-10 days of injury following two negative radiographs. All eligible patients with a clinically suspected SF, but negative radiographs and a negative CT, underwent an MRI

BACKGROUND: Patients seeking information and advice on treatment of scaphoid fractures unknowingly confront longstanding medical controversies surrounding the management of this condition. However, there are no studies specifically looking into the quality and reliability of online information on scaphoid fractures.
METHODS: We identified 44 unique websites for evaluation using the term "scaphoid fractures". The websites were categorized by type and assessed using the DISCERN score, the Journal of the American Medical Association (JAMA) benchmark criteria and the Health on the net (HON) code.

Scapholunate (SL) ligament injuries generally result from trauma to an extended, supinated wrist, leading to chronic instability that progresses toward osteoarthritis. They can occur together with distal radius fracture. These injuries can be difficult to diagnose, especially early on. Treating chronic SL ligament injuries before the onset of osteoarthritis is still a challenge for surgeons. Until recently, the recommended treatments consisted of open SL ligament reconstruction or repair procedures that reduce pain and improve pinch strength but also lead to wrist stiffness. The introduction o

BACKGROUND: The standard treatment to enhance fracture healing of scaphoid nonunions is surgery. Low-intensity pulsed ultrasound (LIPUS) is gaining in popularity as an alternative treatment to improve fracture healing; however, little is known about success rates of this treatment in scaphoid-delayed unions. The purpose of our study is to define the success rate of LIPUS treatment for delayed union of scaphoid fractures and further analyze whether initial management or fracture type influences success rate.
METHODS: During the period of 2011-2013, in the central orthopedic clinic of our insti

Hierarchical models such as the bivariate and hierarchical summary receiver operating characteristic (HSROC) models are recommended for meta-analysis of test accuracy studies. These models are challenging to fit when there are few studies and/or sparse data (for example zero cells in contingency tables due to studies reporting 100% sensitivity or specificity); the models may not converge, or give unreliable parameter estimates. Using simulation, we investigated the performance of seven hierarchical models incorporating increasing simplifications in scenarios designed to replicate realistic sit

This report describes a 53-year-old man who presented, following a motocross injury, with a Mayfield stage 1 perilunate dislocation through a pre-existing asymptomatic scaphoid non-union with a degenerative midcarpal joint. A salvage reconstruction with scaphoid excision and four corner midcarpal arthrodesis resulted in a satisfactory outcome. Follow-up at 6 years demonstrated no instability and no further progression of the osteoarthritis.

In 1910, Georg Preiser (1876-1913) described five cases of rarifying osteitis. Based on his imaging studies, he diagnosed post-traumatic avascular necrosis (AVN) of the scaphoid without any sign of primary fracture. This was followed by an article in 1911 in which Preiser related his findings to Kienböck's disease and Köhler's disease of the tarsal navicular. Upon searching the literature, we found descriptions and discussions of Preiser's imaging; however, the original images have never been published. We reproduce Preiser's original imaging in this current review. All of these appear to show
