Impingement syndrome.

Pain from rotator cuff pathology is a common problem in upper extremity athletes. Impingement of the rotator cuff tendons on the subacromial arch is an important cause. Primary age-related degeneration and instability, however, should not be forgotten as causes for cuff pathology. Conservative treatment is successful in many athletes. Surgical subacromial decompression is reserved for resistant cases. Although often helpful, surgical treatment generally requires a long recovery for throwing athletes and can be associated with residual symptoms.

[Ultrastructure of the deltoid muscle in wrestlers with habitual shoulder dislocation and during rehabilitation after surgical treatment].

Electron microscopical investigation of the musculus deltoideus bioptates has been performed in wrestlers with a habitual shoulder-slip (7 persons). In the same persons contralateral muscle of the healthy arm has been studied (4 persons) and state of the muscle after operative treatment of the shoulder-slip (4 persons) has been analysed. At repeated shoulder-slips signs of the muscle atrophy are noted; this is clear from destructive changes of myofibrils, appearance of a great number of necrotized fibers, outgrowth of the connective tissue. In two cases ectopic formation of the bone in the mus

Geriatric shoulder pain: common causes and their management.

Shoulder pain in the geriatric patient is a common presentation encountered by primary care physicians. Proper evaluation requires an understanding of the pertinent anatomy, a thorough physical examination, and a knowledge of common shoulder disorders that occur in this population. This article provides information needed to evaluate the geriatric patient with shoulder pain. Common causes of shoulder pain--both intrinsic and extrinsic--and their management will be reviewed.

Posterior fracture dislocation of the shoulder with biceps tendon interposition.

Posterior dislocation of the shoulder, a rare injury, results from direct trauma, indirect trauma, or via a seizure or electrical shock. We present a case with a posterior fracture dislocation of the shoulder secondary to a seizure in which interposition of the biceps tendon precluded closed reduction. The fractured lesser tuberosity fragment included the bicipital groove, allowing the biceps tendon to sublux posteriorly preventing closed reduction, thus requiring a subsequent open reduction.

[Recurrence frequency following traumatic shoulder dislocation].

99 cases of primary traumatic shoulder dislocation were evaluated in regard of the influence of the time of immobilization on recurrency. 3 weeks of strict immobilization gave a recurrency rate as low as 6.4% whereas a shorter immobilization resulted in 12% of recurrency within 1--6 years. A survey from 46 other clinics as well as the literature has given no evidence of clear rules of treatment and thus the discussion of treatment remains open.

The Impact of Adding an Eccentric-Exercise Component to the Rehabilitation Program of Patients With Shoulder Impingement: A Critically Appraised Topic.

Shoulder pain is a common musculoskeletal complaint and is often associated with shoulder impingement. The annual incidence of shoulder pain is estimated to be 7% of all injuries, and is the third-most-common type of musculoskeletal pain. Initial treatment of shoulder impingement follows a conservative plan and emphasizes rehabilitation programs as opposed to surgical interventions. Shoulder rehabilitation programs commonly focus on strengthening the muscles of the shoulder complex and, more specifically, the rotator cuff. The rotator cuff is a primary dynamic stabilizer of the glenohumeral jo

Vulnerability of the spinal accessory nerve in the posterior triangle of the neck: a cadaveric study.

Injury to the accessory nerve results in an obvious shoulder droop, loss of shoulder elevation, and pain. Prevention of inadvertent injury to the accessory nerve is critical in neck dissection. No previous study, however, anatomically demonstrates the mechanism of the spinal accessory nerve traction injury. Anatomic determination of the location and course of the spinal accessory nerve may be helpful for a better understanding of the mechanism of the nerve injury. The accessory nerve courses obliquely across the posterior triangle on the surface of the levator scapula muscle and reaches the tr

Ipsilateral Rockwood type V acromioclavicular joint dislocation and midshaft clavicle fracture: A case report.

BACKGROUND: Clavicle fractures are among the most common fractures seen in the emergency department. While acromioclavicular (AC) joint injuries are much less common. However, ipsilateral combinations of these injuries are quite rare with only a few cases reported in the literature. CASE SUMMARY: A 29-year-old man who sustained a combination of ipsilateral AC joint dislocation and midshaft clavicle fracture. He underwent open reduction and plate fixation of the clavicle fracture, as well as semi-rigid surgical implants used to restore both the AC ligaments and the coracoclavicular joint. one

Injury rates and injury risk factors among U.S. Army wheel vehicle mechanics.

This study describes injury rates, injury diagnoses, anatomical locations of injuries, limited duty days, and activities associated with injuries in a sample of Army mechanics. Medical records of 518 male and 43 female Army mechanics were screened for injuries during 1 year at a large U.S. Army installation. Weight, height, age, and ethnicity were also extracted from the medical records. Body mass index was calculated as weight/height2. Overall injury rates for men and women were 124 and 156 injuries/100 person-years, respectively, with a rate of 127 injuries/100 person-years for all soldiers

Sports injuries in footballers related to defects of posture and body mechanics.

The objective of this study was to investigate possible relationships between the incidence of sports injury and the existence of body mechanics defects in players of various codes of football. Injuries were recorded prospectively over a 24 month period. Fifteen aspects of body mechanics were evaluated at the start of the study using a specially developed photogrammetric technique known to have high reliability and sensitivity. It was found that subjects who suffered from ankle injuries had lower mean scores for ankle mechanics than the non-injured players. Knee injuries were found to be assoc

Arthroscopic Management of Anterior, Posterior, and Multidirectional Shoulder Instabilities.

Arthroscopic shoulder stabilization offers several potential advantages compared with open surgery, including the opportunity to more accurately evaluate the glenohumeral joint at the time of diagnostic assessment; comprehensively address multiple pathologic lesions that may be identified; and avoid potential complications unique to open stabilization, such as postoperative subscapularis failure. A thorough understanding of normal shoulder anatomy and biomechanics, along with the pathoanatomy responsible for anterior, posterior, and multidirectional shoulder instability patterns, is very impor

NCAA football players are at higher risk of upper extremity injury after first-time concussion.

BACKGROUND: Previous research has demonstrated that concussions increase the risk of subsequent lower extremity musculoskeletal injury in athletes. However, the risk of upper extremity injury in athletes' post-concussion is poorly understood. METHODS: All concussed football players within a National Collegiate Athletic Association (NCAA) Division I conference athletic database were identified between 2017 and 2021. After exclusions, each athlete experiencing their first concussion was then retrospectively reviewed for upper extremity injuries in the year prior to their concussion and in the y

Analysis of ipsilateral and bilateral ratios in male amateur golfers.

The number of injuries that force golfers to quit is also increasing. In particular, the upper body injuries are concerns for amateur golfers. This study was conducted not only to investigate muscular balance, such as ipsilateral and bilateral ratios of the upper body, but to also evaluate the possible problems of muscular joints in amateur golfers. Male golfers (n=10) and a healthy control group (n=10) were recruited for the assessment of muscular function in the upper body, which was measured by an isokinetic dynamometer at 60°/sec. The tested parts were trunk, wrist, forearm, elbow, and sho

Sonographic Examination of The Fetus Vis-à-Vis Shoulder Dystocia: A Vexing Promise.

Since antepartum and intrapartum risk factors are poor at identifying women whose labor is complicated by shoulder dystocia, sonographic examination of the fetus holds promise. Though there are several measurements of biometric parameters to identify the parturient who will have shoulder dystocia, none are currently clinically useful. Three national guidelines confirm that sonographic measurements do not serve as appropriate diagnostic tests to identify women who will have shoulder dystocia with or without concurrent injury. In summary, biometric measurements of the fetus should not be used to

Track and Field Throwing Sports: Injuries and Prevention.

Throwing events in track and field are among the oldest sporting events in recorded history and include shot put, discus, hammer, and javelin. All throwing athletes use the kinetic chain to transfer energy from the lower extremities, through the pelvis, trunk, shoulder, arm, wrist, hand, and finally into the implement to generate maximum force. Throughout this sequential activation, mobilization, and stabilization of body segments, there is opportunity for breakdown in the chain, which can lead to injury. This review describes the biomechanics and kinetic chain components of each event and hig
