Exertional sickling: questions and controversy.

Sickle cell trait (SCT) occurs in about 8% of African-Americans and is often described to be of little clinical consequence. Over time, a number of risks have emerged, and among these are rare but catastrophic episodes of sudden death in athletes and other individuals associated with physical activities which is often described as exercise collapse associated with sickle trait (ECAST). Despite an epidemiologic link between SCT and sudden death as well as numerous case reports in both medical literature and lay press, no clear understanding of the key pathophysiologic events has been identified…

Impaired cytoadherence of Plasmodium falciparum-infected erythrocytes containing sickle hemoglobin.

Sickle trait, the heterozygous state of normal hemoglobin A (HbA) and sickle hemoglobin S (HbS), confers protection against malaria in Africa. AS children infected with Plasmodium falciparum are less likely than AA children to suffer the symptoms or severe manifestations of malaria, and they often carry lower parasite densities than AA children. The mechanisms by which sickle trait might confer such malaria protection remain unclear. We have compared the cytoadherence properties of parasitized AS and AA erythrocytes, because it is by these properties that parasitized erythrocytes can sequester

The spectrum of haemoglobin variants in two scheduled tribes of Sundargarh district in north-western Orissa, India.

BACKGROUND: Tribal communities in India constitute a major part of the population and are vulnerable to many erythrocytic hereditary and haematological disorders such as haemoglobinopathies. Genetic studies so far undertaken on tribal groups are scanty, patchy and incomplete. No field-based systematic studies of hereditary haemolytic disorders in Orissa are available. Further, the extent of haemoglobin variants among the tribals in the state is not known. The present study was carried out in the Bhuyan and Kharia tribes of Sundargarh district in Orissa. AIM: This study aims to find the preval

Racial differences in ristocetin-induced platelet aggregation.

Several investigators have reported defective ristocetin-induced platelet aggregation (RIPA) in individuals whose red blood cells contain sickle haemoglobin, but the race of control subjects in these studies was not stated. Therefore, maximal amplitude of RIPA was examined in 75 normal whites and blacks, of 16 of whom had sickle trait defined by haemoglobin electrophoresis and sickle prep. Final ristocetin concentrations in platelet rich plasma were 1.1, 1.2 and 1.5 mg/ml. Mean aggregation at 1.1 mg/ml was significantly less in blacks (mean 31%) than in whites (mean 72%) (P less than 0.001). 6

Heat-Related Illnesses.

Heat-related illnesses encompass a spectrum of disorders resulting from exposure to high temperatures and the body's inability to efficiently dissipate heat. These syndromes range from mild illnesses, such as heat edema, heat rash, and heat cramps, to more concerning manifestations such as exercise-associated collapse, heat exhaustion, and exertional rhabdomyolysis. The most severe and potentially life-threatening heat-related illnesses include exercise collapse associated with sickle cell trait, severe exercise-associated hyponatremia, and heat stroke. Common risk factors of heat-related illn…

Sickle Cell Education: A Survey of Antenatal Healthcare Givers.

To explore the educational practices of antenatal care providers toward pregnant women with sickle cell disease (SCD) and sickle cell trait (SCT), a survey was conducted among selected doctors and midwives who provide antenatal care at the outpatient clinic of the Obstetric Department of the Korle-Bu Teaching Hospital, Accra, Ghana. The study explored their practices of screening for and patient education about SCD and SCT. Of the 102 respondents, 100(98%) stated that they were knowledgeable in the medical and genetic aspects of the disease. Regarding screening, 82(80.4%) reported mandatory sc…

Point-of-care screening for sickle cell disease in low-resource settings: A multi-center evaluation of HemoTypeSC, a novel rapid test.

Sickle cell disease (SCD) is a common, life-threatening genetic disorder that is best managed when diagnosed early by newborn screening. However, SCD is most prevalent in low-resource regions of the world where newborn screening is rare and diagnosis at the point-of-care is challenging. In many such regions, the majority of affected children die, undiagnosed, before the age of 5 years. A rapid and affordable point-of-care test for SCD is needed. The diagnostic accuracy of HemoTypeSC, a point-of-care immunoassay, for SCD was evaluated in individuals who had SCD, hemoglobin C disease, the relate

Relationship between malaria incidence and IgG levels to Plasmodium falciparum merozoite antigens in Malian children: impact of hemoglobins S and C.

Heterozygous hemoglobin (Hb) AS (sickle-cell trait) and HbAC are hypothesized to protect against Plasmodium falciparum malaria in part by enhancing naturally-acquired immunity to this disease. To investigate this hypothesis, we compared antibody levels to four merozoite antigens from the P. falciparum 3D7 clone (apical membrane antigen 1, AMA1-3D7; merozoite surface protein 1, MSP1-3D7; 175 kDa erythrocyte-binding antigen, EBA175-3D7; and merozoite surface protein 2, MSP2-3D7) in a cohort of 103 HbAA, 73 HbAS and 30 HbAC children aged 3 to 11 years in a malaria-endemic area of Mali. Type I collagen as a marker of bone metabolism in sickle cell hemoglobinopathies.

Avascular necrosis of the skeletal system is a complication of sickle cell hemoglobinopathies. Type I collagen, which is synthesized by osteoblasts, comprises about 90% of the total organic matrix of bone. This preliminary study of skeletal changes in sickle cell hemoglobinopathies measured type I collagen formation and degradation using specific radioimmunoassays to determine the plasma concentrations of carboxyterminal propeptide of type I procollagen (PICP) and carboxyterminal pyridinoline cross-linked telopeptide of type I collagen (ICTP), respectively. Plasma ICTP concentrations were incr

Sickle cell disease, pneumococcal meningitis and paraplegia with recovery.

A 10-year-old boy without previous illness was found to have sickle cell disease and pneumococcal meningitis. On completion of 10 days of treatment with penicillin, the only abnormality still present was paraplegia. Movements of the legs recovered gradually, and after 3 months he was walking normally. Paraplegia is rare in sickle cell disease and the prognosis is not always poor.

Cholelithiasis in children with homozygous sickle cell anaemia in northern Nigeria.

The prevalence of gall stones was studied prospectively by abdominal ultrasound examination in 65 consecutive children with sickle cell anaemia (SCA) aged 5-13 years, 35 males and 30 females, in Jos, northern Nigeria. The prevalence of cholelithiasis was 4.6%. This rate was similar to previous studies from southern Nigeria and higher than that reported in American children with SCA. The different dietary habits of children in southern and northern Nigeria appear not to play any role in the prevalence rate. None of the children followed up for more than 12 months had upper quadrant abdominal pa
