Clinical presentations of acute pancreatitis vary from trivial pain to severe acute illness with a significant risk of death. Urgent endoscopic treatment of acute pancreatitis is considered when there is causal evidence of biliary pancreatitis. External biliary fistulas associated both with open and laparoscopic biliary surgery, need a quick solution to prevent local and general complications. Open surgical treatment was the only solution until the minimally invasive techniques, such as endoscopy, were developed. The authors present 10 cases of postoperative external biliary fistulas, successfully treated by endoscopic papillosphincterotomy alone, technique which is compatible with other endoscopic procedures of treatment, performed in association with endoscopic papillosphincterotomy or not.

Over the past decade, endoscopic therapy has been increasingly used as a less invasive alternative to surgery for the treatment of pancreatic duct strictures. The therapeutic goal has been the palliative relief of severe pain associated with chronic pancreatitis. Studies from several centers have shown that endoscopic stenting results in pain relief for a high percentage of patients. Future investigation needs to focus on the refinement and standardization of basic techniques, and different strategies need to be compared in well-designed trials.

Pancreas divisum patients make up a small but problematic portion of ERCP cases. Minor papilla cannulation techniques have been improved. Recurrent pancreatitis patients generally benefit from minor papilla therapy. Methods to select patients who are likely to respond to invasive therapy need refinement. Clinicians and endoscopists are strongly encouraged to be cautious and conservative with this patient group until stronger data indicate optimal management schemes.

A retrospective review of 56 patients operated upon for tumours of Oddi was performed in order to determine optimal diagnostic and therapeutic procedures. Common presenting symptoms were jaundice (86%) and anemia (21%). Mean size of the tumour was 2.3 cm. Five tumours were benign and 51 were malignant.
