Repair of Achilles tendon ruptures with peroneus brevis tendon augmentation.

PURPOSE: To report 22 patients who underwent repair of compound Achilles tendon ruptures with peroneus brevis tendon augmentation. METHODS: Records of 6 women and 19 men aged 21 to 42 (mean, 28) years who underwent repair of compound Achilles tendon ruptures with peroneus brevis tendon augmentation were reviewed. All the wounds were transverse/oblique, minimally contaminated, and could be closed primarily. Patients were evaluated at months 3, 9, and 12, using the Foot and Ankle Outcome Score (FAOS) questionnaire. RESULTS: Of the 22 patients, 3 developed superficial skin complications that he

In Vivo Assessment of a Load-Bearing Biphasic Osteotendinous Braided Collagen Scaffold for Reconstruction of Massive Irreparable Rotator Cuff Defects.

Massive, irreparable rotator cuff tears affect a significant patient population and often require invasive procedures such as allograft patch augmentation, superior capsule reconstruction, and muscle transfers due to the limited regenerative capacity and inability to reattach the rotator cuff to the humerus. Current surgical techniques primarily focus on mechanically reinforcing the tendon without promoting biological integration, leading to high re-tear rates and functional limitations. This study aimed to evaluate OsTend, a biphasic osteotendinous scaffold designed to span the MIRC defect by

[Patellar imbalance and external rotary instability of the knee].

The importance of external hyperrotation and external rotary instability in patellar imbalance is examined. In view of the progressive nature of patellar imbalance and of the predominant part played by the muscular and capsulo-ligamentous elements in the stability of the extensor apparatus, the authors advocate an earlier and more radical surgical approach with a view to improving the patellar medialization forces (transplantation of the intermediate great muscle and retension of the lateral patellar retinaculum) as well as the elements of active internal rotation of the tibia (modified versio

Managing Severe Postburn Wrist Flexion Contracture by Proximal Row Carpectomy and Abdominal Interpolation Flap.

Introduction: Postburn upper extremity contractures can greatly diminish the quality of life. To successfully manage these contractures and achieve successful functional outcomes, an optimal surgical method should be planned to address all affected tissues on the extremities. Conventional soft tissue transfers after releasing the contracture, such as skin grafts or flaps, may be insufficient. In addition to capsulotomy, tendon release, and lengthening procedures, more aggressive modalities may be indicated. Methods: In this retrospective study, patients who developed wrist flexion deformities

Fascial flap reconstruction of the hand: a single surgeon's 30-year experience.

BACKGROUND: The reconstruction of complex hand wounds is challenging due to the requirements for thin and pliable coverage with a reliable vascular supply, potential for sensibility, and provision of a gliding surface. Fascial flaps represent an excellent option for the reconstruction of these complicated defects. METHODS: A retrospective review of fascial flap reconstructive procedures to the hand undertaken by a single microsurgeon was performed for operations occurring between 1979 and 2009. Both pedicled and free tissue transfer procedures were included in both pediatric and adult patient

[Application of sartorius muscle in the quadricepsplasty].

OBJECTIVE: Extension stiffness of knee joint is always treated by the quadricepsplasty, but the main deficiency of this method is that patient feels weakness of lower limb and easily kneels down. The aim of this article is to explore the method to resolve the complications after quadricepsplasty. METHODS: Since 1978 to 1997, on the basis of traditional procedures of quadricepsplasty, sartorius muscle was used to reinforce the extension of knee joint. The lower 2/3 of sartorius muscle was fully dissociated only with its insertion intact. A tendon-periosteal-bone flap, about 2 cm in width, was

Editorial Commentary: Interposition Graft Bridging Reconstruction: Good Mid-Term Outcomes for Massive Irreparable Rotator Cuff Tears.

Massive irreparable rotator cuff tears without glenohumeral arthritis are a common cause of shoulder pain and disability. Many surgical treatment options have been proposed, including debridement, partial repair, tendon transfer, superior capsule reconstruction, balloon spacer placement, bursal acromial reconstruction, and reverse shoulder arthroplasty. Interposition graft bridging reconstruction, as evidenced by the mid-term results of the current study, may also be considered, at least for now. However, let's see if this procedure will truly stand the test of time because all orthopaedic sur…

Radial head dislocation as a rare complication of obstetric brachial plexus palsy: literature review and five case series.

Radial head dislocation secondary to obstetric brachial plexus palsy is a rare complication that may occur a few years after birth. Five cases were examined and a comprehensive literature search was performed. Although it is a concern for parents, the dislocation resulted in mild or minimal functional impairment for all five children. Surgical interventions such as biceps tendon transfer, radial head open reduction or excision and annular ligament reconstruction were largely ineffective in significantly improving ranges of motion. Clinicians should be aware of the potentially futile outcomes a…

Surgical treatments to restore function control in spinal cord injury.

People with spinal cord injury (SCI) have a profound loss of control of their lives and abilities. Surgical procedures are of significant benefit in improving autonomy, self-care, and body function. Functional electrical stimulation (FES) is useful at higher levels of paralysis such as ASIA C5 or C6 where there are no remaining voluntary muscles for tendon transfer and can partially replace respiration, balance, and ambulation. Outcomes studies show that surgical care improves independence, strength of grasp, and measured quality of life. Those with tetraplegia should be referred for consultat…

Resolution of anterior segment ischemia after the removal of lateral fixation sutures.

Anterior segment ischemia is a rare but well-recognized complication after strabismus surgery, involving multiple extraocular muscles. We report a case of traumatic left sixth nerve palsy in a 32-year-old man who developed anterior segment ischemia after left medial rectus muscle recession and partial tendon transfer of the vertical rectus muscles to the lateral rectus, augmented by lateral fixation sutures. The corneal edema and anterior chamber reaction cleared upon removal of the lateral fixation sutures and institution of steroids and cycloplegics. The possible role of lateral fixation sut…

Microsurgical fibular graft for full-length radius reconstruction after giant-cell tumor resection: a case report.

In this article, we present the treatment of a recurrent giant-cell tumor of the radius with en bloc resection and full-length radius reconstruction with a 24-cm long microsurgical fibular graft. At time of 8-year follow-up, there was no evidence of tumor recurrence. A satisfactory range of motion of the elbow, wrist, and forearm was maintained. There was no instability in the joints, and grip strength measured 63% of the opposite side. With appropriate dynamic tendon transfer, this procedure can provide an alternative method for reconstruction of the full-length radius after tumor resection, …

The three in one procedure: how I do it.

Recurrent patella dislocation is common, and selection of an appropriate stabilisation procedure is important for a successful outcome. We describe a combined proximal and distal realignment procedure to adjust soft tissue tension around the patella. This procedure, the 'Three in One' involves a combination of a lateral release, vastus medialis obliquus muscle advancement, and transfer of the medial third of the patellar tendon to the medial collateral ligament. This is a safe, reliable procedure for patients with recurrent dislocation of the patella resulting from an imbalance of soft tissue

Brachial plexus dorsal rhizotomy in hemiplegic cerebral palsy.

Brachial plexus dorsal rhizotomy releases spasticity, improving the functional use of the hand. Grasping and pinch strength are augmented, together with movement speed and dexterity. Even when four dorsal roots have been sectioned, hand sensibility is largely preserved. Movement control is improved and equally advantageous in athetotic patients. Brachial plexus dorsal rhizotomy does not exclude the use of tendon lengthening or transfer procedures, but it is the authors' opinion that orthopedic procedures should be performed after dorsal rhizotomy. Children aged 5-6 years without muscle contrac

Salvage of failed resection of the distal ulna. Case report.

A 45-year-old woman completely lost the ability of active supination of the forearm after a Darrach resection for malunited fracture of the distal radius. A three-component reconstruction was performed to stabilise the distal stump of the ulna and prevent convergence between the two forearm bones. The procedure combined advancement lengthening osteotomy of the ulna, longitudinal intramedullary tenodesis of the extensor carpi ulnaris tendon, and dorsal transfer of the pronator quadratus through the interosseous space. Four months after the salvage procedure she again had full active supination

Opponensplasty through translocation of the flexor pollicis longus. Technique and indications.

Eleven cases of opponensplasty by translocation of the flexor pollicis longus are reported. Some technical points ought to be outlined: this translocation is mostly performed through a fused IP joint; the level of the reflection of the translocated tendon should necessarily be the neck of the first metacarpal bone. Nine cases were reviewed with eight very good results. There are very few indications for this technique: total palsy of thenar muscles with stiff IP flexion deformity of the thumb, whenever a fusion is indicated. Particular cases of high-level palsy of the upper limb, when no other

Treatment of Chronic Achilles Tendon Ruptures With Large Defects.

UNLABELLED: Background When Achilles tendon ruptures become chronic, a defect often forms at the rupture site. There is scant literature regarding the treatment of chronic Achilles ruptures with defects of 6 cm or larger. We examined outcomes from combining a turndown of the proximal, central Achilles with a flexor hallucis longus (FHL) tendon transfer to treat this condition. Materials Between September 2002 and December 2013, 32 patients presented with a chronic Achilles rupture and a defect of 6 cm or more. Twenty patients were male and 12 were female. Patient age was between 20 and 74 year…
