Diagnosis and treatment of complicated tubercular meningitis.

A 41-year-old woman was seen in no acute distress with an infected ventriculoperitoneal shunt. She underwent several revisions of the shunt but was readmitted to the hospital with nausea, vomiting, and neurologic sequelae. Results of spinal fluid analysis were white blood cells 68/mm3 (25% neutrophils), glucose less than 20 mg/dl, and protein 513 mg/dl. Cerebrospinal fluid, aerobic and anaerobic, and blood cultures were negative. Three weeks later the patient suffered a seizure and was prescribed antitubercular agents for a presumed diagnosis of tubercular meningitis. One week later, chest wou

Multifocal tuberculosis simulating a cancer-a case report.

BACKGROUND: Tuberculosis is a disease that may affect any organ of the body. Multifocal tuberculosis involving multiple systems with associated symptoms are rare, which makes the diagnosis challenging. Distinguishing multifocal tuberculosis from lesions metastatic from system malignancy is difficult. Single detection method is difficult to make a diagnosis. A combination of multiple methods is essential. CASE PRESENTATION: A 17-year-old male presented with a 20 days weakness of lower limbs, which aggravated for 6 days. The PET/CT showed increased metabolism of ileocecal intestinal and termina

Simultaneous 18F-FDG PET/MRI in tuberculous spondylitis: an independent method for assessing therapeutic response - case series.

BACKGROUND: 18F-fluorodeoxyglucose positron emission tomography (18F-FDG PET) shows great potential for diagnosis and assessing therapeutic response of tuberculous spondylitis. Tuberculous spondylitis required long-term anti-tuberculosis (TB) medication therapy, and the optimal duration of therapy is controversial. There is still no clear way to tell when the anti-TB therapy can safely be discontinued. CASE PRESENTATION: Three patients with tuberculous spondylitis were evaluated for therapeutic response using 18F-FDG PET/magnetic resonance imaging (MRI). Clinical and hematological improvement

[Two cases of extrapulmonary tuberculosis causing differential diagnostic problems].

The decrease of tuberculotic diseases was continuous in the past years. This statement relates to pulmonalis as well as to extrapulmonalis tbc. The authors want to call attention to the possibility of the varied and more rarely appearing extrapulmonalis manifestation. In the first of their cases the burst of spondylitis tbc, and in the other one the very rare spleen tbc--which was made probably clinicum and later by the US abdomen examination--caused differential diagnostic problem.

Targeted Six-Week Intensive Physiotherapy for a Case of Tuberculous Meningitis With a Syndrome of Inappropriate Antidiuretic Hormone Secretion.

Tuberculous meningitis (TBM) is a severe form of extrapulmonary tuberculosis (TB) characterized by the invasion of Mycobacterium tuberculosis into the meninges surrounding the brain and spinal cord. It triggers an intense inflammatory response, leading to neurological complications if not promptly and adequately managed. TBM often precipitates muscle weakness, neurological deficits, respiratory challenges, swallowing difficulties, joint contractures, and pain. Physiotherapy intervention is essential in treating these problems by personalized treatment strategies and treatment plans to enhance

Early IFN-γ production together with decreased expression of TLR3 and TLR9 characterizes EAE development conditional on the presence of myelin.

Experimental autoimmune encephalomyelitis (EAE) is a model for the study of multiple sclerosis, which is an inflammatory and demyelinating disease of the central nervous system (CNS). Despite increased efforts to elucidate the function of toll-like receptors (TLRs) in autoimmune diseases of the CNS, the relative contribution of other factors, including the immunomodulatory properties of TLR signaling, role of the innate response and the presence or absence of myelin peptides remain unclear. The aim was to evaluate TLR expression in the CNS during EAE development by investigating the expression

Spinal epidural abscess in an adolescent male: an unusual etiology and the role of intraoperative ultrasound.

BACKGROUND: Pediatric spinal epidural abscess is considered to be a rare illness. Despite all the recent improvements in the diagnosis and the treatment of this rare condition, the morbidity and mortality rate is still high especially if the diagnosis was not achieved early on. PURPOSE: The purpose of this paper is to review the current literature on spinal epidural abscess (SEA) in the pediatric population, with a focus on understanding the clinical presentation, diagnosis, management strategies, and outcomes. This review aims to highlight the unique aspects of SEA and underscore the role of

Acute flaccid paralysis and neurogenic respiratory failure associated with enterovirus D68 infection in children: Report of two cases.

BACKGROUND: Acute flaccid paralysis (AFP) and neurogenic respiratory failure rarely occur in children. At the end of 2018, some children with such symptoms were admitted to our hospital. In this study, we aimed to assess two children with AFP and neurogenic respiratory failure associated with enterovirus D68 (EV-D68). CASE SUMMARY: Two children admitted to our hospital presented with symptoms and imaging results different from those of acute disseminated encephalomyelitis and hand, foot, and mouth disease. Their main symptoms were AFP and neurogenic respiratory failure. Magnetic resonance ima

Multiple Spinal Intramedullary Primitive Neuroectodermal Tumors Mimicking Acute Myelitis.

BACKGROUND: Primitive neuroectodermal tumors (PNETs) are rare malignant neoplasms in the spinal cord that comprise undifferentiated or poorly differentiated neural crest cells. PNETs imaging findings lack specificity, making diagnosis difficult. CASE DESCRIPTION: A 16-year-old male patient presented with progressive weakness and numbness of the lower extremities for 1 month, which occurred after a cold. Significantly elevated protein levels were detected in his cerebrospinal fluid. Magnetic resonance imaging revealed multiple patchy or spindle-shaped hyperintensities on T2-weighted imaging, w

Unusual circumstances to diagnose cervical Pott's disease.

Tuberculous spondylodiscitis usually affects the dorso-lumbar spine, and its cervical location is a rare condition that can mimic other diseases and consequently cause treatment delays. We report a case of tuberculous spondylodiscitis of the lower cervical spine discovered under unusual circumstances in a patient with severe polytrauma involving a cranio-cerebral trauma, a non-displaced fracture of the two laminæ and the spinous process of the C6 vertebrae as well as fibular and tibial shaft fractures. The patient underwent static tibial nailing, and a collar with occipital and chin supports w

[Clinical value of the posterolateral approach, supplemented with pedicle excision of the arch, to the anterior wall of the spinal canal within the thoracolumbar (Th11-L1) and lumbar spine].

UNLABELLED: The author validates clinically his own lateral approach to the lumbosacral (Th11-L1) and lumbar spine. In this approach an enlargement of the intervertebral foramen is achieved by the partial excision of adjacent pedicles or excision of one or several pedicles. The access to the anterior canal wall is gained in this manner. This surgical approach has been used in 48 patients with neurological injury (spinal cord, conus medullaris, cauda equina or the roots). An active tuberculosis was the causative factor in 12 cases, the sequelae of tuberculous infection in 10 cases, infection in

Multidrug resistant miliary tuberculosis and Pott's disease in an immunocompetent patient.

In a high tuberculosis TB prevalence country, mortality due to miliary TB is not unknown but the treatment outcome in general is good. We describe a previously healthy man with miliary TB who did not respond to 2-months antituberculous therapy with 4 drugs. Persistent complaints of backache, which antedated chest symptoms, resulted in a diagnosis of Pott's disease. Culture of bronchial aspirate yielded multidrug resistant Mycobacterium tuberculosis that responded slowly to streptomycin, ethionamide, cycloserine, clofazimine, ofloxacin, paraaminosalicylic acid and isoniazid. The association of

The IVBF dual-blade plate and its applications.

A recently developed simple device, the intervertebral body fixation dual-blade plate, was used in 88 cases of different spinal disorders. This patients in the first series were operated on from 1984 to 1986. The device is biomechanically simple and its application safe and easy. Using a proper bone grafting technique, it is a reliable device to establish spinal stability in spine surgery through an anterior approach. Its application in cases of fractures and fracture-dislocations of the thoracolumbar spine with paraplegia, tuberculous spondylitis, and primary tumors of vertebral bodies is pre

A case of afebrile miliary tuberculosis that progressed from tuberculous spondylitis with iliopsoas abscess.

We present a case of a 78-year-old woman who visited our hospital for chronic atrial fibrillation. She consulted an orthopedic surgeon for lumbar pain in August 2009 and has been followed up for osteoporosis. However, her lumbar pain became exacerbated. In December 2009, clinical examination revealed that the pain was caused by tuberculous spondylitis and iliopsoas abscess. Diffuse miliary shadow, which was undetected earlier, was noted on chest roentgenogram; she was diagnosed with miliary tuberculosis. Lumbar pain is common in elderly individuals and should be regarded as one of the tubercul

Cold tuberculous abscess identified by FDG PET.

We report FDG PET of two cases of cold abscess due to Mycobacterium tuberculosis. Case 1 had colon cancer; FDG PET showed high FDG uptake in the colon lesion and low uptake in the inguinal lesion. The latter was a tuberculous cold abscess confirmed by CT/MRI and biopsy. Case 2 received radiotherapy for lung cancer and presented with suspected vertebral metastasis. Further studies revealed tuberculosis of the vertebra and a tuberculous cold abscess in the iliopsoas muscle. FDG PET showed moderate uptake in the third lumbar spine and low uptake in the abscess center of iliopsoas lesion. Both tub

Fetal hydrops in GM(1) gangliosidosis: a case report.

UNLABELLED: GM(1) gangliosidosis is a rare disorder characterized by deficiency of the ss-galactosidase enzyme, with the resulting accumulation of glycolipids, oligosaccharides and especially GM(1) ganglioside. It can be classified into three clinical types according to the time of onset: infantile, juvenile and adult form. We report a case of GM(1) gangliosidosis presenting with fetal hydrops at 24 wk of gestation. The parents were consanguineous; the baby, born at 35 wk of gestation, was dysmorphic and presented severe generalized oedema. The most common cause of fetal hydrops was excluded.
