Dual atrioventricular nodal nonreentrant tachycardia: a systematic review.

BACKGROUND: Dual atrioventricular nodal nonreentrant tachycardia (DAVNNT) is a relatively uncommon supraventricular tachycardia. The clinical characteristics and optimal management strategies are not well described. METHODS: A systematic review of all reported cases of DAVNNT was performed. Articles were included if they described spontaneous tachycardias resulting from 1:2 atrioventricular (AV) conduction as a result of two AV nodal pathways. Data sources included PubMed from 1950 to January 2011, article bibliographies, and the Google internet search engine. RESULTS: Ultimately, 44 citatio

Non-re-entrant atrioventricular nodal tachycardia.

Supraventricular tachycardia caused by AV nodal dual conduction is usually re-entry tachycardia. We report a case of a 40-year-old man with incessant supraventricular tachycardia caused by simultaneous conduction via slow and fast AV nodal pathway. The patient was successfully treated with catheter ablation of slow AV nodal pathway, the treatment of choice of this form of tachycardia (when proper diagnosis is made). Analysis of previously published cases is discussed.

Fronto-nasal dysplasia and atrio-ventricular canal in a fetus with trisomy 18 identified by absent nasal bones during first trimester screening scan.

Failed ultrasonographic visualization of nasal bones is associated with an increased risk of fetal malformations. Maternal ethnicity and chromosomal abnormalities influence the incidence and visualization rate of nasal bones. A case of absent nasal bones with fronto-nasal dysplasia and septated cystic hygroma identified at 13(+5) weeks' gestation in a trisomy 18 fetus is reported. The crown-rump length was 82 mm and the absent nasal bones were associated with micrognathia and a flattened face. The risks for trisomy 21 and 18 were subsequently calculated. The couple refused chorionic villus sam

Idiopathic left ventricular tachycardia with a change from left to right axis deviation during radiofrequency catheter ablation.

A 62 year-old-woman presented with a right bundle branch block (RBBB) and left axis deviation (LAD) tachycardia. Radiofrequency (RF) energy was delivered to the left posterior fascicle (LPF) where 2 presystolic Purkinje potentials (P1 and P2) preceding onset of the QRS complex were recorded. During RF energy applications, the QRS morphology of the tachycardia changed from RBBB and LAD to RBBB and right axis deviation without termination of the tachycardia. The Purkinje potential was observed following the QRS complex during the tachycardia because of conduction block of the LPF distal to the r

Influence of the randomized trials, AFFIRM and RACE, on the management of atrial fibrillation in two University Medical Centers.

The results of the Atrial Fibrillation Follow-Up Investigation of Rhythm Management (AFFIRM) and the Rate Control Versus Electrical Cardioversion for Persistent Atrial Fibrillation (RACE) study were presented in March 2002. These large studies showed no benefit of a rhythm-control strategy over a rate-control strategy in managing atrial fibrillation (AF). Cardioversion and atrioventricular junctional ablation are forms of rhythm control and rate control, respectively. The numbers of cardioversions and atrioventricular junctional ablations performed at the University of Virginia and the Medical

Adult heart block is associated with disease activity in primary Sjögren's syndrome.

OBJECTIVES: Congenital heart block occurring in the foetus and neonate may be associated with maternal anti-SS-A/anti-SS-B autoantibodies (anti-SSA/anti-SSB). The adult atrioventricular node is generally thought to be resistant to the damaging effects of anti-SSA/anti-SSB. However, case reports suggest that heart block developing in adult Sjögren's syndrome (SS) patients may be associated with these autoantibodies. Therefore, we investigated the relationship between serum antibodies and heart block in adult SS patients. METHODS: We abstracted data from clinic patient records. Diagnosis of pri…

Cross sectional echocardiographic assessment of the extent of the atrial septum relative to the atrioventricular junction in atrioventricular septal defect.

OBJECTIVE: To study patients with atrioventricular septal defect to determine the pathognomonic morphological features of the lesion and the relation between the septal structures and the atrioventricular junction. Setting : Tertiary level paediatric cardiology centre. METHODS: Cross sectional echocardiograms from 60 patients were reviewed using qualitative and quantitative analysis. The unifying feature was the presence of a common atrioventricular junction. The overall dimensions of the septal defect were determined and related to the plane of the common junction; the extent of both the atr

Atrioventricular conduction during long-term follow-up of patients with sick sinus syndrome.

BACKGROUND: It has been claimed that patients with sick sinus syndrome have an increased risk of developing AV block, but this has never been assessed prospectively. The aim of the present study was to evaluate in a prospective trial AV conduction during the long-term follow-up of patients with sick sinus syndrome. METHODS: Two hundred twenty-five consecutive patients with sick sinus syndrome and intact AV conduction were randomized to undergo single-chamber atrial pacing (110 patients) or single-chamber ventricular pacing (115 patients). Follow-up after 3 months and then yearly included meas

[The radiofrequency catheter ablation of occult accessory atrioventricular pathways].

OBJECTIVE: The aim of this study was to evaluate our results of radiofrequency catheter ablation (RFCA) of concealed accessory atrioventricular pathways (CP). PATIENT SELECTION: We treated with RFCA 19 patients, with 21 CP, 10 men and 9 women, mean age 37 +/- 16 years, with supraventricular tachycardia (SVT) and absence of ventricular pre-excitation in the electrocardiogram (ECG). These patients comprised 50% of the patients who underwent RFCA for SVT and had no ventricular pre-excitation in the ECG. The diagnosis of CP was made by electrophysiologic study, based on the demonstration of a pat

A simple in vivo model to evaluate the effects of antiarrhythmic agents.

Antiarrhythmic agents alter cardiac conduction, refractoriness, and action potential duration in a rate dependent fashion. A simple in vivo model was developed to measure these variables over a wide range of cycle lengths. Complete heart block was produced in dogs using an 8 French hexapolar ablation catheter, anterior skin electrode, and radiofrequency current (30 W, 13.6 +/- 2.6 seconds). Ventricular pacing and monophasic action potential recordings were performed using a single combination catheter. QRS and action potential durations were measured at multiple cycle lengths and after an abru

[Value of a provocation test with diprafenone in patients with bifascicular block].

The significance of provocative tests in patients with bifascicular block is not established. We studied 14 patients with bifascicular block, syncope and documented episodes of high degree AV-block. 1.5 mg/kg Diprafenon was given after a period of at last 12 hours 1:1 AV-conduction. Electrophysiologic evaluation was performed before and after diprafenon. AV-block III could be provoked in 1 of 14 patients with Diprafenon. Therefore a total AV block occurred in 7% of patients. The low sensitivity of provocative test with Diprafenon even in patients with documented high degree AV-block prevents i

Normalization of reverse redistribution of thallium-201 with procainamide pretreatment in Wolff-Parkinson-White syndrome.

Stress thallium-201 myocardial perfusion imaging was performed in a patient with Wolff-Parkinson-White syndrome. Reverse redistribution phenomenon was observed in the absence of coronary artery disease. This seems to be the first report of normalization of this phenomenon in association with reversion of accessory pathway to normal atrioventricular conduction after pretreatment with procainamide.

[Change in impulse formation and impulse transmission of the heart in relation to high magnesium concentration].

The effects of magnesium ions on the cardiac conduction velocity and on the refractory periods of the single parts of Langendorff perfused guinea pig hearts were studied using a high amplifying ECG recording and a special stimulation technique. Concentrations of 2.3 mM or 3.45 mM Mg++ had only little inhibitory effects on the cardiac conduction velocity and on the sinus node activity. The elevation of the magnesium ion concentration to 4.6 mM caused a reduction of the spontaneous sinus rate and a prolongation of the AV nodal and His bundle conduction time. During stimulation with stepwise incr

Intracardiac electrode catheter recordings of atrioventricular bypass tracts in Wolff-Parkinson-White syndrome: techniques, electrophysiologic characteristics and demonstration of concealed and decremental propagation.

UNLABELLED: Atrioventricular bypass tract deflections were recorded in five patients with the Wolff-Parkinson-White syndrome using standard, closely spaced (5 mm) electrode catheters. Three right paraseptal and two left-sided Kent bundles were recorded at the level of the tricuspid valve on the His bundle catheter and in the coronary sinus, respectively. Characteristics of the bypass tracts were studied during atrial pacing, programmed premature atrial stimulation, induction of supraventricular tachycardias and programmed ventricular stimulation. During atrial pacing, as pre-excitation increas

[Hemodynamic aspects of the transvenous electrodestruction of the atrioventricular junction].

Transvenous electrodestruction of the atrioventricular junction is an effective method of surgical treatment for supraventricular arrhythmias (paroxysmal tachysystoles) refractory to medication. However, it is central hemodynamic status alone that can be an objective indicator of favorable clinical effect. Major central hemodynamic parameters were examined in 52 patients before and after the operation. Considerable late postoperative improvement of the general condition and normalization of major central hemodynamic parameters were noted in most cases.

[Sotalol in supraventricular tachycardia. Electrophysiologic measurements in Wolff-Parkinson-White syndrome and AV node re-entry tachycardia].

The electrophysiologic effects of sotalol were studied in 11 patients with Wolff-Parkinson-White syndrome and 9 patients with AV nodal reentrant tachycardia. Electrophysiologic studies were performed before and after intravenous infusion of 80 mg sotalol over a period of 5 minutes. Sotalol prolonged the effective refractory period of the right atrium and the right ventricle. Both AV node and accessory pathway conduction were depressed by sotalol in antegrade and retrograde directions. Induction of reentrant tachycardia was prevented in 6 of 18 patients. The rate of reentrant tachycardia decrea

Pullout His bundle/electrophysiologic studies using leads previously inserted via the subclavian vein.

Conventional electrophysiologic studies involve the insertion of several catheter electrodes, positioned to pace and record from the high right atrium, His bundle region, and the right ventricular apex. The present study was undertaken to determine whether similar information could be obtained during removal of temporary pacing leads previously inserted via the subclavian vein, without the need for further invasive studies. Attempts to record a His bundle potential were successful in 157 of 200 patients (79%). A His bundle potential could be recorded during low right atrial pacing on 24 of 58

Antegrade conduction and AV node function in patients with unidirectional retrograde accessory pathways.

In 26 patients with unidirectional retrograde accessory pathways (URAP), antegrade conduction properties were evaluated. During electrophysiologic study the interval from the low septal right atrial potential to the His bundle potential (LSRA-H) in sinus rhythm (SR) was found to be less than 60 msec in 7 out of the 18 patients with left-sided URAP and in one out of two patients with septal URAP. Each of the six patients with right-sided URAP had an LSRA-H equal to or greater than 70 msec. During atrial extrastimulus testing, LSRA-H failed to prolong more than 100 msec (LSRA-H increment equal t

Cardiac sympathetic-parasympathetic interactions.

The cardiac response to neural activity in one autonomic division depends on the level of activity in the other autonomic division. The extent of this peripheral interaction varies considerably with the cardiac effector tissue under consideration. In sinoatrial nodal tissue the effect of efferent sympathetic activity is progressively attenuated as the level of vagal activity is increased. A similar type of interaction prevails in the regulation of atrioventricular (AV) junctional pacemaker tissue. Surprisingly, however, an analogous peripheral interaction cannot be detected with regard to the
